2016-2017 SNHSC Home Club Membership The Southern New Hampshire Skating Club, a member of United States Figure Skating, offers membership opportunities to its participants and supporters. Becoming a member of the club carries with it numerous benefits, as well as responsibilities for furthering the mission of the SNHSC. Please read through each page of the application, and sign as indicated. This will serve as a contract between you (or parent/guardian, if under the age of 18) and the Southern New Hampshire Skating Club. Gold Level Member - $135/year - Membership with USFS - $2.00 off/hour contract and walk-on rate - 10% discount on all club merchandise - Birthday gift from SNHSC - Registration waived for Holiday Exhibition - Opportunity to have a solo in the Annual Ice Revue contingent on meeting SNHSC ice use, test requirements, and all other soloist requirements - Additional Ice Revue group number (at discretion and need of Show Director) - Subscription to SKATING magazine - The opportunity to represent SNHSC at competitions and USFS test sessions - Link to USFS Rulebook - A copy of the SNHSC By-laws - Will receive priority over all levels and non-members when booking ice Blue Level Member - $85/year - Membership with USFS - $1.00 off/hour contract & walk-on rate - 5% discount on all club merchandise - Birthday gift from SNHSC - Registration waived for Holiday Exhibition - Additional Ice Revue group number (at discretion and need of Show Director) - Subscription to SKATING magazine - The opportunity to represent SNHSC at competitions and USFS test sessions - A copy of the SNHSC By-laws - Will receive priority over LTS members and non-members when booking ice

Important points regarding membership levels: - Skaters who are eligible for full solos in the Annual Revue must be Gold members, and skaters interested in taking USFS Moves in the Field/Freestyle/Dance tests as representatives of SNHSC must be Gold or Blue Members. - Gold and Blue members will have voting privileges. Those who are under the age of 18 at the time of voting may be represented by a parent or legal guardian, who will automatically be considered a subsequent member at no additional cost. Voting representatives will not be eligible for discount offers. - The rate for subsequent membership is $45/year. The subsequent member will garner the same benefits as the primary member. (For example, if you sign up one skater as a Gold Member, your second skater will also receive Gold Member benefits and you will pay $135 + $45 = $180.) - The membership year runs July 1 through June 30. - A list of the professional staff and program applications will be sent upon approval of membership. Only one copy of SKATING magazine will be sent per member household. Membership Process: The SNHSC Board of Directors meets on a monthly basis to consider new and renewed applications for membership. Upon receipt of a complete membership application, the Membership Chairperson can grant probationary membership if all requirements are met. Probationary membership expires at the following meeting of the Board of Directors. The decisions of the Board of Directors concerning membership shall be final. Application Due Date: Since the membership year begins on July 1, the Southern NH Skating Club membership chair must submit applications to US Figure Skating prior to that date for members to receive a full year s worth of benefits. Because of that requirement, completed membership applications must be postmarked by June 15, 2016. Late fees will be charged as follows: - Applications received between June 16 and September 1: $15 - Applications received between September 2 and November 1: $25 *NOTE: Late fees are waived for skaters who have never been home club members of the Southern NH Skating Club.

SNHSC Rules and Conduct Code As a member club of US Figure Skating, the Southern New Hampshire Skating Club is obligated to follow all policies and guidelines set forth by its governing body as stated in the USFS Figure Skating Rule Book. By signing this agreement, you acknowledge that you have read and discussed it with your skater (if a minor), and agree to abide by the following rules: 1. Courtesy is expected at all times on and off the ice. (Parents, remember you are the example here!) 2. All skaters must check in with the ice monitor (or a board member if the ice monitor is not present) and pay for walk-ons at the start of the session. 3. All walk-on skaters must be members in good standing of US Figure Skating. 4. No shoes are allowed on the ice. 5. Please do not sit or lie on the ice (unless injured or asked to do so by a coach). 6. Skaters doing a program will be given right of way. Other skaters need to be aware of the skater doing their program. 7. Skaters on lesson will be given priority with regard to music. Other requests will be played in the order received. 8. For safety reasons, the ice monitor and the Board of Directors will have the authority to remove a skater from the ice if their behavior or ability places the skater or other skaters at risk, regardless of level. Per USFS Code of Conduct, section GR 1.03, and per the SNHSC rules and regulations, applicants also agree to the following: I recognize that my participation in all competitions, exhibitions, and training camps hosted, supported, or sponsored by US Figure Skating is an honor and privilege that carries certain responsibilities. I agree to fully abide by the rules and guidelines set forth by US Figure Skating or its properly designated agents as a precondition to participation in US Figure Skating hosted, supported, or sponsored events, I will adhere to the following tenets of good faith: I will exhibit the highest standards of fairness, ethical behavior, and genuine good sportsmanship in all of my relations with others. I will not damage public or private property. I understand that I may be held financially responsible for damage deemed to be wantonly or willfully executed on my part, and that I may be subject to disciplinary action by the USFS and/or SNHSC. 2016-2017 Soloist Requirements In the event that an Annual Ice Revue is held, skaters who have passed the following combination of tests will be considered for a FULL solo, not to exceed 2 minutes or as the director determines show need dictates: Preliminary Moves in the Field and Preliminary Freestyle; OR Preliminary Moves in the Field and Preliminary Dance; OR Preliminary Moves in the Field and the Learn to Skate USA program level Freestyle 5. - Tests must be passed by December 31, 2016. Applicants must also meet the following requirements: - The skater must be a member in good standing with US Figure Skating, and a Gold Level Home Club Member of the Southern NH Skating Club. - Skaters must book at least two full sessions per week from October, 2016 through March, 2017. A full session is defined as booking a program or freestyle session for its entirety in length and duration. For example: A skater can book one 50 minute freestyle session and one 30 minute Learn to Skate session to meet the requirement. A skater cannot book one hour long freestyle session and a 15 minute power session to satisfy the requirement. - One of the booked sessions MUST be a freestyle session, and be booked for the length of the session. - The minimum length of a qualifying booked freestyle session is 40 minutes anything less will not count towards the required booking requirement. - Soloists are not eligible for Learn to Skate discounts when booked in conjunction with the Annual Ice Revue practice sessions. - Skaters eligible for a solo will complete the necessary application as distributed by the show director, and meet the necessary volunteer requirements. The Intro Solo Skaters who have achieved the following tests are eligible for an Intro Solo: Pre-preliminary Moves and Learn to Skate USA level Freestyle 2. Skaters may only be an Intro Soloist for ONE year. Once a skater has been an Intro Soloist, they will have to achieve the Full Soloist requirements to be considered for a solo in subsequent years. Tests must be passed by December 31, 2016. Skaters must also meet the following requirements: - The skater must be a member in good standing with US Figure Skating, and a Blue or Gold Level Home Club Member of the Southern NH Skating Club. - Skaters must book at least two sessions per week from October, 2016 through March, 2017. A full session is defined as booking a program or freestyle session for its entirety in duration. To meet the requirement, the skater must book a Learn to Skate session WITH practice ice, and one half hour of another program. This may include freestyle ice, dance, power/edge, or any combination thereof. - Soloists are not eligible for Learn to Skate discounts when booked in conjunction with the Annual Ice Revue practice sessions. - The length of the Intro Solo will be determined by the needs of the Show Director; roles may be shared (i.e. one skater performs in the afternoon show, and another performs in the evening). It is strongly recommended that all members of the Southern NH Skating Club also meet the requirements listed above, regardless of soloist eligibility. Skaters who skate at least two days a week will see a great difference in their skating and be better prepared for both Learn to Skate and USFS testing. Missed Ice Policy & Late Fees Requests for refunds or adjustments in fees due to extended illness or injury must be accompanied by a physician s document. Requests shall be directed to the SNHSC Board of Directors. There will be NO REFUNDS for occasional missed sessions due to school conflicts, vacations, or common illnesses (flu, colds, etc.) of less than three weeks duration. There will be a $25 late fee assessed for payments received more than 10 days following the due date. Payments can be made via cash, check, or credit card. If ice payment as indicated by the individual session applications is a hardship, the Board of Directors will work with you to set up a payment plan. We need to know that this is the case no later than the second week of a session. A skater s account will be assessed at the current bank rate for non-sufficient funds plus a $10.00 service charge for any checks returned to the club. After one check is returned for non-sufficient funds, payment must be made by cash or money order. I have read the club missed ice policy and late fee information. Required Fundraising Commitment As a nonprofit organization, increased fundraising has become a necessity to keep the SNHSC operating at the financial level it needs to provide a quality program for our skaters. We deeply appreciate your understanding of this policy. The fundraising goal for each member skater is $100. This may be met in two different ways: 1. The fee may be paid up front. This completes your fundraising obligation for the 2016-2017 season. 2. Your skater may participate in SNHSC organized fundraisers. We hold several throughout the year, which may include car washes, Yankee Candle product sales, calendar fundraiser, among others. Participation in group fundraisers (i.e. car washes, tag days) will be tracked by dividing the income by the number of participants. Monetary fundraisers (calendar raffle, Yankee Candle, Skate-A-Thon, etc.) will be tracked based on your skater s individual sales. 3. You can organize your own way to raise funds for the club - which can be done with the approval of the SNHSC Board of Directors. Each member s fundraising commitment will be tracked by the SNSHC Board of Directors. Please be aware that if the fundraising goal is not obtained after the last scheduled fundraiser during the membership year, then the remainder of that goal will be owed, and the skater (or parent, if the skater is under the age of 18) will be invoiced. If that amount is not received by the end of the 2016-2017 membership year, this will jeopardize the skater s standing with US Figure Skating for the 2017-2018 membership year. The Southern NH Skating Club is an organization run by volunteers, and the time and talents (on and off the ice) of our members will help keep the organization running for years to come. Please set the example for all SNHSC participants and dedicate your time to our programs, fundraisers, and events many hands really do make light work.

Waiver and Release of Liability, Assumption of Risk and Indemnity Agreement In consideration of participating in the Southern NH Skating Club s activities, I attest that I understand the nature of figure skating activities and that I am qualified, in good health and in proper physical condition to participate in such activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that this activity involves risk of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inaction, those of others participating in the activity, the conditions in which the activity takes place, or the negligence of the releases named below and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume such risks and all responsibility for losses, costs, and damages I incur as a result of participation in the activity. I hereby release, discharge, and covenant not to sue the Southern NH Skating Club, United States Figure Skating, its directors, officers, administrators, sponsors, volunteers, agents, employees, staff, instructors, trainers, other participants, and, if applicable, owners and lessors of premises on which the activity takes place (each considered one of the Releases herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or part by the negligence of the releases or otherwise, including negligent rescue operations; and I further agree that if, despite this release, waiver of liability, and assumption of risk, I or anyone on my behalf, makes a claim against any of the releases from any loss, liability, damage, or cost which any may incur as the result of such a claim. The Southern NH Skating Club has the right, but not the obligation, to provide rules, regulations and/or ice monitors for Club ice. We hereby acknowledge that the SNSHC shall not be responsible for the supervision of the members at Club ice. I have read this Release and Waiver of Liability, Assumption of Risk, and the Indemnity Agreement, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that any portion of this agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect.
