How would you triage these patients at an MCI? 1. Middle aged male not breathing. Upon opening the airway, breathing restored. Has large open bleeding wound on right upper leg. Weak thready radial pulse. 2. 13 year old male with bilateral femur fractures. Breathing at a rate of 20, alert and oriented, strong pulse at 90, and in extreme pain. 3. A crying infant covered in dirt and mud. No obvious injuries, strong pulse, adequate respirations at rate 28. 4. Young adult female, not breathing, with a serious head injury. Upon opening the airway, notice it is full of blood. No respiratory effort after airway is secured. 5. 45 year old female with marked deformity to both forearms. Walks to you for assistance. Breathing rate 18, strong pulse at 88, alert and oriented. 6. Elderly female on ground, moaning. No obvious injuries. Breathing rate 20, with adequate airway. Strong pulse at 70. She does not follow commands, and is unsure where she is. 7. Female runs up to you crying, covered in blood. She has an adequate airway, is breathing at a rate 24, with a strong pulse of 120. She is alert and able to follow commands. 8. 60 year old male with burns over his lower legs, making it impossible for him to walk due to pain. Notice soot around his nostrils. Breathing adequate at a rate of 20, pulse 100, alert, and oriented. 9. At an outdoor concert, lightning strikes the crowd during a sudden thunderstorm. You are the first unit to the scene. Upon arrival, you find 10 patients. 4 are unresponsive, including one teenage male with no pulse and no respirations after repositioning the airway. How would you triage this patient? Firefighter Training SCBA Check Skill Sheet 12-FF-2 Objective : Morning SCBA Check-Off Procedure Firefighter Name: Date: Directions Morning SCBA Check off Procedure. A typical structure fire IDLH atmosphere usually consists of a toxic air supply and high heat of varying degrees. The four key elements that allow us to operate in a IDLH environment are: SOPS, a strong strategic based Incident Command System, Water and tools with the ability to deliver it, and our PPE. The centerpiece of our PPE is the SCBA. Our ability to operate in a IDLH environment rely on the mechanical function of the SCBA. The morning Check-off of each individuals SCBA becomes the most critical step in our apparatus and equipment checks. Your life and your crew members life depends on the smooth and seamless operation of the SCBA. Once arriving for your assignment your first responsibility is to relieve your counterpart and inspect your SCBA. Full protective clothing including SCBA Equipment & Materials Criteria & Evaluation Comments Criteria After the Firefighter has completed the skill sheet, write comments below. Evaluator/Candidate Comments Pass Fail Evaluator Signature Date Firefighter Signature Date

Firefighter Training SCBA Check Skills Evaluation Checklist Objective : SCBA Morning Check-Off Task Steps Pass Fail 1. Remove SCBA from your riding position. 2. Visual Inspection of the overall physical condition of the unit 3. Make sure all straps are fully functional, fully extended and ready for use. 4. Remove the Dust Cover from the URC fitting and Inspect the fitting and interior of the dust cover. 5. Ensure the cylinder is closed and purge the system of any air using the bypass red knob. Once purged close the bypass valve fully. 6. Disconnect the High Pressure coupling from the bottle and check the condition of the nipple seal gasket or O-ring. Reconnect the coupling to the cylinder hand tight only ensuring the URC is aligned to the rear of the SCBA for filling. 7. Check the condition of the O-Ring on the mask mounted regulator 8. Insert the regulator into the face piece and check for proper engagement by pulling on the regulator to ensure the attachment of the regulator to the face piece. 9. Hold the face piece tight into the face and inhale to check for negative seal. Hold for approximately 5 seconds. 10. Exhale into the face piece to check for proper function of the exhalation valve. 11. Remove the facepiece from your face a depress the Stop button on the regulator that was caused by the exhale. 12. With the regulator still attached to the facepiece, open the cylinder valve fully listening for the audible low air alarm and PASS device Activation. 13. Ensure the activation of the Heads-Up display 14. Check the cylinder pressure and the Control Console pressure for consistency ensuring that there is no more than 200 PSI difference. 15. Check the PASS alarm by allowing it to go into pre-alarm mode and moving the PASS to silence to ensure operation. 16. Allow PASS device to go into full alarm mode by having the device remain motionless and then Reset the PASS 17. Activate the manual Full Alarm PASS by depressing the red button on the Control Console. Reset the Alarm after this is activated by depressing the yellow button on the Control Console twice separated by one second 18. Don Face Piece, inhale, and check the system for proper air flow. 19. Test the bypass by fully opening and closing the valve to ensure proper operation. 20. Doff Facepiece and depress the Stop on the regulator without removing the regulator from the facepiece 21. Close the Cylinder valve and watch the Control Console air gauge. If there is any drop in pressure there is a leak in the system.

Firefighter Training SCBA Check Skills Evaluation Checklist Objective : SCBA Morning Check-Off Task Steps Pass Fail 22. Open bypass valve slightly on the regulator while watching the control console, listening for the low air alarm and the vibra alert that is consistent with gauge 23. Bleed all residual air from the unit using the red bypass valve. 24. Turn off PASS device 25. Disengage regulator / facepiece assembly from unit and stow in proper place. 26. Replace rubber dust cover on URC 27. Check cylinder air pressure. If pressure is below 4500 PSI fill cylinder to appropriate level 28. Place unit back into the riding position adjusting shoulder and waist straps for next response. 29. Ensure same procedure is done after every response using SCBA unit

Firefighter I Firefighter Personal Protective Equipment Skill Sheet 5-I-1 Objective 21: Don PPE and SCBA for use at an emergency. (NFPA 1001, 5.1.1.2, 5.3.1) Student Name: Date: Directions For this skills evaluation checklist, students will don personal protective equipment and SCBA.
