Please read information below it is all important for you to know. General Information Basic Information The program includes 6 weeks of two-hour lessons there is no lodge/indoor time during the lesson session. Bus transportation is optional and limited seats are available. The lesson program is open to current Medina Elementary students (grades k-5). Transportation is only available for students in grades 1-5 (no bus for the kindies sorry!) Enrollment is limited to about 140 skiers and snowboarders, and program fees include an end of season (usually pizza) party for participants, instructors and chaperones. This program is a PTA-sponsored event and, for insurance purposes, each participating family must be a current PTA member. Class assignments (according to each participant s abilities) and instruction are provided by The Summit Learning Center at Alpental. Participants need to feel comfortable riding a chairlift. Though riding the chairlift might be a new experience for some participants, one of the goals of the program is to get all students, including the beginners, riding the chairlift by the second week. If a participant is unwilling/unable to ride the chair by the end of the second lesson, this program may be inappropriate and the parents will be notified. Please note that Medina Snowsports is a recreational program, which means that we may not be able to accommodate very high level skiers/snowboarders. Unfortunately, refunds cannot be given for any unused portion of the lesson package or bus transportation. Applications will be accepted beginning Tuesday, October 27 th, at 7:45 a.m. there will be a table set up under the covered area at school. Parent/guardian/nanny must drop off the application, you may not hand off your application to a friend in line. If you do not need bus transportation it is not necessary to come wait in line. Applications for just lessons can be dropped in the office. Please do not drop off any applications in the office prior to Tuesday, October 27 th at 7:45 no early applications will be accepted. Dates: Wednesdays: January 6, 13, 20, 27, February 3, 10, 2015. If the ski school cancels a day, the series will be extended until all 6 sessions are complete. If needed, the first make-up day will be Wednesday, February 24. Costs Bus Transportation, Lessons and Season Passes Bus Transportation- $140 check made payable to Medina PTA. Please complete the attached form and return it with payment. Note: some parents drive up to Alpental instead of using the charter bus. If you do not get on ski bus perhaps a carpool may work for you and your child. Lesson cost- $186.00 For children that went last year, there will be instruction on how to use roll over credit. Again this year: The Summit Learning Center has an on-line lesson sign up. If your son/daughter is accepted into the program, you will be notified by the Medina Snowsports coordinators (by NOVEMBER 13) and you will be given an access code to use to sign up for lessons on the Summit Learning Center webpage. Season passes - All students will need a season pass prior to the start of lessons. The season pass sign-up will be on-line you will be given an access code when you are notified that your son/daughter is in the program.

Season Pass Information Season passes are available to Medina Snowsports participants at a discounted rate - an access code will be provided to you once you ve been notified of your participation so that you can purchase your season pass online from the Summit at Snoqualmie. Daily schedule (times can vary due to weather conditions): School gets out at 12:15PM. Parents are responsible for making sure that the students are dressed in their ski clothes and ski/snowboard boots and have their gear ready prior to boarding the bus. Bus starts loading at 12:40, or sooner if possible. Roll call taken at 12:50PM. Bus departs at 1:00 PM and arrives at Alpental around 2:00 Lessons will be held from 2:15-4:15PM. The bus departs Alpental at 4:30 PM and arrives at Medina Elementary at approximately 5:30 PM. Parents need to be at school at this time, ready to help with equipment. Remember, no glass containers please or hot liquids. VERY IMPORTANT: Please check all clothing and equipment for proper fit and function prior to the first lesson. A dry run will insure that your child s first day goes smoothly! Rules and Regulations We plan on having an excellent year together!!! For the safety and enjoyment of everyone, we expect the students to be familiar with and follow the program rules. 1. Please be prompt for departure from Medina (buses leave between 12:50 p.m. and 1:00 p.m.) and for the pick up at Medina at approximately 5:30 p.m. (weather permitting). Parents are responsible for assuring that their child gets on the bus (dressed and with all equipment). 2. Participants need to have ALL gear ready to go prior to boarding the bus (with boots on!). If students do not have the proper gear prior to boarding they will not be allowed on the bus. Students are NOT able to rent equipment on the hill. 3. Make sure that everything is labeled clothing, boards, skis, boots, poles everything! 4. Students are ultimately responsible for walking from the bus to the lesson site in a safe and responsible manner. At the end of lessons students must walk with the group and chaperone or instructor back to the bus on the designated path. 5. If there are pass and/or ski area closures that necessitate canceling a trip, chaperones WILL ATTEMPT to notify all parents and will contact the school. Sometimes LAST MINUTE CANCELLATIONS OCCUR. If in doubt, call coordinators noted below or The Summit Ski School Cancellation Hot Line at (425) 434-6789. 6. Due to limited space on the bus, students may not bring school back packs on the ski bus. Please take school back packs home after school. Alternatively, backpacks can be left at the school until the return of the bus at 5:30. 7. Students are expected to show respect, at all times - to the chaperones, bus drivers, instructors and their fellow students. 8. Please remember that any misconduct (including free-skiing which is not permitted) deemed serious enough by the coordinators will result in expulsion from the ski/snowboard program without a refund. The chaperones and The Summit Learning Center have sole authority in these matters. BUS POLICIES/GUIDELINES 1. No skis/snowboards are to be leaned against the bus. The driver or Chaperones will load or unload all skis/snowboards. 2. Snowboarders and skiers should wear their boots onto the bus. 3. For safety reasons, no glass containers (pop bottles, jars, etc.) will be allowed on the bus. 4. No hot beverages or popcorn allowed on the bus. 5. No arms are to be extended outside of open windows and nothing is to be tossed from the bus or inside the bus. 6. Because extensive damage can occur, students are to keep off the seat backs and keep boots and equipment off the seats. 7. No yelling, screaming or shouting on the bus. 8. We strongly suggest leaving all electronics, etc., at home due to the possibility of theft or loss. 9. Any special arrangements with regard to a student returning to Medina by means other than the return bus trip are to be communicated to the CHAPERONES via email, by Tuesday evening if possible. 10. There are no refunds on trips not taken due to special ride arrangements. 11. Lost items, if left on the bus, can be found in the ski school Lost and Found in or near the office at Medina Elementary. 12. If your child causes damage to the bus (illness, property damage) the student s parent(s) may be held accountable for expenses incurred. *Please print and keep this information for future reference*

School Participation Agreement & Release I (we) recognize that skiing/snowboarding are hazardous sports that can result in serious injury or death. I (we) voluntarily chose to participate in this activity and accept the risk inherent in skiing/snowboarding and in the ski area/mountain environment. I (we) accept our responsibility to be informed, to behave prudently, to ski/snowboard safely and to read and abide by the Skier s Responsibility Code and all other posted behavior notices. I (we) agree to Release, Hold Harmless and Indemnify Medina Elementary School, Medina Elementary School PTSA, Bellevue School District #405, Ski Lifts, Inc., its parent subsidiary and affiliated companies, and the ski area and its/their employees, agents, volunteers, contractors, officers, members and real and personal property owners from all claims for any damage resulting from any cause which arise out of participation in or travel to and from this ski/snowboard program. This release is binding as to any other persons, including family members, heirs, and executors. This release does not apply to gross negligence or intentional acts. If I am signing on behalf of a minor I recognize that I may not release any claims the minor may have. However, I accept full responsibility for all medical expenses and/or damages incurred as a result of the minor s participation in or travels to and from the ski/snowboard program. I also agree to Release, Hold Harmless and Indemnify Medina Elementary School, Medina Elementary School PTSA, Bellevue School District #405, Ski Lifts, Inc., its parent subsidiary and affiliated companies, the ski area and its/their employees, agents, volunteers, contractors, officers, members and real and personal property owners for any claims brought by the minor. Students will be expelled from this ski/snowboard program without refund for being willfully disobedient, possession, use or sale of tobacco, alcohol, or controlled substances, vulgarity, stealing, gambling, fighting, leaving the ski area or for using non-approved transportation. If the ski/snowboard program is cancelled for a day(s), the program is automatically extended until the program is completed. If the program is not completed by March 26, 2014, any remaining on-snow lessons and transportation will be refunded minus trips and lessons taken. All other refunds are considered only if a student must withdraw based upon a doctor s written recommendation. Because of the preseason costs, a $50.00 processing fee is charged to issue all refund checks. I (we), the undersigned, have carefully read and understand this Participation Agreement & Release and agree it is binding on us. Student and Parent signature are required.
