Liberty Middle School Competition / Football Cheerleading Squad Try-Out Packet Forms are due no later than March 19th, 2018. Sign-up online then complete this form!

Mandatory Cheer Clinic and Try-Outs Who: Any 2018-2019 7 th or 8 th grader who will attend Liberty Middle School interested in cheering football/competition. When: Clinic: March 20-21-22, 3:30-5:00 pm Closed Try-Out: March 23rd 3:30pm until complete Where: Liberty Middle School Gym Attire: Clinic Days: t-shirt, shorts, and tennis/cheer shoes Friday Try-outs: White shirt, mostly black shorts, white socks, shoes, white ribbon/bow, hair worn out of face. In order to participate in clinic and try-outs, each cheerleader candidate must have the following forms completely filled out and signed-up online. All of these forms need to be placed in an envelope with candidate s name and delivered to the coach by March 19th, 2018. 1. Current Physical Form (from within the past calendar year and signed by a doctor) 4. Madison City Schools Athletic Insurance Waiver Form (left side on website, under Athletics) 5. Concussion Form (left side on website, under Athletics) 6. AHSAA Waiver (left side on website, under Athletics) 7. LMS Student Athlete Contract (left side on website, under Athletics) 8. Signed Current Class Average Form (in this packet) 9. Copy of Birth Certificate 10. 2 Separate Teacher Recommendation Forms* (in this packet) *The teacher recommendation forms should be sent from the teacher directly to the cheerleading coach. The teachers will not be giving these forms back to the candidate. All forms should be printed, completed, and returned by March 19th, 2018 to Danielle Dykes (Room 116), Liberty Middle School

Requirements for 2018-2019 Cheerleading Squad 1. You must be Liberty Middle School student entering either the 7 th or 8 th grade for the 2018-2019 school year. 2. You must have a 70 overall average in each subject area. This average must be maintained in order to remain an active cheerleader. 3. You must be in attendance to each cheer clinic date prior to try-outs. 4. You must be able to attend summer practices, camps and events involving the LMS Cheerleaders throughout the year. 5. You must be physically able to try-out. 6. You must accept all monetary obligations. 7. You must abide by the Liberty Middle School Student Athlete Contract. 8. You must turn all of the required forms in by March 19th, 2018.

Practice will resume July 23 rd.
