Please read the following pages regarding the above topics and then sign. By signing each section you are stating that you understand and will comply with the expectations and rules set forth at 3- Tigers Martial Arts. We also guarantee to provide high quality, professional martial arts training in a family atmosphere. Please print out the Tuition Agreement and bring with you next class

Our Quality Guarantee to You: To maintain quality instruction, we have created a comprehensive Leadership Training Program (LTP) and Staff Training Program (STP). Each participant, child and adult alike will learn how to build leadership skills that will motivate students without making harsh demands that decrease motivation. To maintain quality curriculum, we ve implemented a variety of programs and drills that target each student s needs. Our programs target each student s stages of development so that everyone s training experience improves their overall development physically, intellectually, emotionally, and socially. To maintain a positive atmosphere on the training floor we ve also created a 3-Strikes Rule, which will eliminate consistent distractions from disruptive students (or parents) and prevent any student from progressing if they are inconsistent in their attendance. We believe the students should have clear guidelines that are fair and reasonable, but at the same time prevent any student from decreasing the momentum of the school. All instructors and staff will have a background check preformed. All instructors and staff will be CPR and First Aid Certified. By providing a comprehensive educational system for our instructors and staff, incorporating a variety of agespecific programs, and enforcing the 3-Strikes Rule, we firmly believe that our school will maintain high standards for our members and their families. At any time if you feel that our Quality Guarantee has been compromised please do not hesitate to notify us immediately.

The 3-Strikes Rule We created the 3-Strikes Rule to help measure the effort and attitude of our students. These rules are meant to maintain certain standards that we believe contribute to the overall atmosphere of our school. If a student does not meet these standards, then the rules will be enforced. This will help us eliminate unnecessary distractions that prevent other students from learning and training. Unacceptable and distracting behavior, inconsistent attendance, and negative unreported comments are considered behaviors that can weaken our school s positive atmosphere. To maintain our Quality Guarantee, we will enforce the 3-Strikes Rule when necessary. The following standards are included in the 3-Strikes Rule along with the necessary procedures of enforcement: Unacceptable and distracting behavior If a student displays a poor attitude, inappropriate behavior or distracting behavior, it affects the entire class. Inconsistent attendance If a student becomes inconsistent in attendance, it slows down the entire class and takes the attention away from the hard-working and consistent students. (See the mandatory attendance policy for more details) Negative comments If a student, family member, or a friend talks negatively about another student (or family member), staff, instructor, or any other person and or situation related to the school it is considered unfair and inappropriate; and decreases the school s (and it s members ) overall morale. Any and all negative or devise comments should be reported to management first so an employee of 3- Tigers Martial Arts may address it.

Enforcement for the 3-Strikes Rule: 1. First time someone violates the 3-Strikes Rule, a verbal warning is given and a permanent red mark is made on his or her attendance card. 2. The second time someone violates the 3-Strikes Rule a student (parent if student is under 18) conference is scheduled and a second permanent red mark is made on the student s attendance card. 3. The third time someone violates the 3-Strikes Rule a final student (parent if the student is under 18) conference is scheduled and the student is forced to take a leave of absence with a term length from 4-weeks to one year or more depending on the severity of the violation. 4. Suspension or expulsion from a student s primary school can lead to suspension and or expulsion from our martial arts school. The instructor and parent will set up a conference immediately to discuss the next step for the student at our school. A student or a parent may be asked to leave without going through all 3-Strikes if the infraction or incident is deemed inappropriate and or malicious by Mr. Tibbetts at anytime.

Mandatory Attendance Policy: 1. As a student, you are required to attend 2 training classes each week (this does not include Black Belt or Masters Club). Testing is a class. Anytime an instructor is grading or teaching it is considered a class. 2. If you miss a class you may make it up during the tuition cycle. 3. An attendance mark on our attendance list is kept every class. You may ask if you are short on your required attendance at anytime. Attendance is taken at the beginning of every class. 4. Students must have all of their skill stripes to test. A student in the basic program needs 6 stripes to test. Black Belt Club members Green Belt through Red Black Belt need 7 stripes to test for their next rank. 5. All Ranks need 24 classes to test for their next rank. 6. If you miss or fail a stripe test it is up to you to practice the required material. You may test at the next stripe testing for any missing stripes. 7. We do not reschedule testing. If you are late for testing you may not test. If you have to miss a test please meet with your instructor so they may tell you when you re next testing is. 8. If you are sick or have to miss a class please call the school to let us know you will be out that day. Do not come to class sick (a cold, flu, pink eye, a persistent cough) show respect for your body and fellow students. We will have you go home if we notice you are ill to recuperate and get healthy. 9. If you are going on vacation please let your instructor know so you can set up a time to make up missed classes. Our goal is to help you reach your goals and to do this we must stay on track and part of that is through communication. 10. Snow days. If we are closed we will leave a message on our school voice mail, school facebook page, twitter and on instagram. If West Valley School District closes we will be closed due to inclement weather. 11. You should not be late to class. Allow time for traffic, trains, weather, etc. If you are late once a belt cycle that is fine, once a week or every other week is not acceptable. 12. If you are injured to the point you may not attend class then you must have a doctor s note to release you back to training. No note = no training. 13. Students must practice at home twice a week for 20 minutes. This is mandatory regardless of rank or ability. We expect the parents to set aside time for our students to practice. I know it already doesn t excuse practice at home. 14. Students are expected to tie their uniform, belt and pants correctly. This may need to be practiced at home by the student. Uniforms must be washed weekly and patches attached securely and properly.

The Martial Arts Code of Ethics 1. Use your training wisely. Martial art training is used to increase your physical, intellectual, emotional, and social skills. In any situation, martial arts should only be used as self-defense. 2. Real martial artists are respectful to everyone they encounter inside and outside of the studio. 3. To become the best you can be, train regularly and work on improving one step at a time. 4. Maintain the highest positive attitude possible. The better you act, the better you will train, and the better you will become. 5. Attend class regularly and avoid inconsistency. Make sure you practice good habits and work to become very organized. Missing class because you are unorganized is a habit that will negatively affect your progress and limit you from achieving your goals. 6. Train at your own ability and never force anything unnaturally. 7. To master the posture of a martial artist, eliminate negativity by controlling your temper, avoiding bad habits, and understanding the concept of empathy. 8. Practice being punctual at everything you do. This is the best way to earn the respect of others. 9. Make safety a priority inside and outside of the studio. 10. Show respect for yourself by eating right and staying hydrated. 11. Show respect to others by having good hygiene and not using profanity or negative comments in class or at the studio. 12. Avoid the temptation of teaching others outside of class. It is extremely dangerous and leaves not only you but also us liable for any incidents.

13. Approach us with any questions or concerns you have regarding our school. 14. Always wear a clean, well-pressed uniform to class and remove all jewelry before you enter the training room. 15. Set goals for yourself consistently and always review them to ensure that you are achieving them promptly. 16. Please do not practice anything not formerly taught by an instructor. 17. Let an instructor know immediately if you have an injury or feel ill. 18. Always ask for assistance when you are having a hard time with something or do not understand something. 19. Accept criticism as a way to help you improve your abilities. When an instructor corrects you, they are doing so to help you be the best you can be. 20. Support the Code of Ethics when you see others not following them.

3-Tigers School Rules & Regulations DO: *Bow EACH time you enter or exit the practice area (including water & bathroom breaks) towards the American Flag as a show of respect for your Instructor(s), and fellow students. *Address your Instructor(s) and all Black Belts as Sir or Ma am with respect. Answer all questions with Yes Sir, Yes Ma am, or No Sir, No Ma am. Answer up quickly & loud enough to be heard across the classroom. Do not talk during class unless instructed to do so. *Be on time and dressed for class. If you are late remain on the edge of the mat after bowing in with your hand raised an instructor will let you know when or if you can join class. *Pay very close attention in every class. Focus your attention and concentration as directed by your Instructor(s). *Take off your shoes & socks before entering practice area. Lay your shoes, socks & equipment neatly in the designated area outside of the practice area. (Socks tucked inside of shoes, etc.) *Wear your belt & full uniform to every class, unless instructed otherwise. (Jacket, Pants, Belt, Shirt) *Tie your belt and uniform correctly. Turn away from the class if you need to make any adjustments to your uniform while class is in session. (Jacket, Pants, Belt, Shirt) * If you are in school you must provide a S.T.A.R. Student report twice a year. *Put your name (or initials) on every piece of your equipment and weapons. *Bring your required protective gear to every class. You will not be allowed to participate in any contact drills if you are missing a mouthpiece, male groin protection, headgear, hand gear, or footgear there are no exceptions. *Trim all toenails and fingernails to insure safety in your partner drills. *Wear a clean uniform on a clean body. Wash your uniform weekly. * Tuck shirt, cords/draw strings of your uniform pants inside of your pants. *Come to class in your uniform. Please do not expect to use the dressing room because it may be off limits or unavailable. *You should have patches on your uniform by the following class. No exceptions. *Please cover all cuts with a bandage. *When participating in contact drills if you have long hair it should be pulled back in a ponytail, please no big clips, or barrettes. *Females should wear a tank top, or sports top, under your uniform when participating in contact drills. *Please place trash in the proper trash receptacle.

*Tuition must be current in order to test. *Bring something to keep you busy until your class starts. Please have a seat and keep yourself occupied until class starts. DO NOT: *Do not place your belt on the ground. This shows a lack of respect for your hard work & rank. *Do not wear shoes on the mats unless they are meant for martial arts. No one is allowed in the training area except students. If parents or family need to step on the floor, they must be shoeless and have permission from an instructor. *Do not run, wrestle, play or talk loudly in the spectator area or during class. *Do not curse or use swear words of any kind on or off the mat. *Do not wear any jewelry (watches, chains, facial piercings, rings, earrings, etc.) during class as this can cause injury to yourself or a fellow student. *Do not laugh or make fun of a fellow student. Never display a bad temper towards a fellow student. *Do not chew gum, eat candy, or have any other food in class. *Do not participate in contact drills without the instructor s supervision. *Do not leave gear/equipment or clothing lying around the class area. *Do not bring a student to class without a waiver filled out and signed.

3-Tigers Martial Arts Tuition Agreement (Please Print This Section & Bring With You Next Class) Today s Date: / / Your tuition is due on the of every month. Your monthly tuition fee is. Price is guaranteed until Black Belt Club (Green Belt). This tuition entitles you to attend classes per week. If you take a month off of class or more without paying tuition, you will forfeit any grandfathered pricing and comeback at our current monthly tuition. We accept cash, check, credit cards, online payment and PayPal. Checks are made payable to Brent Tibbetts. There is a $10 late fee charge for payments over 3 business days late from due date. This is not negotiable. There is a $25 NSF fee for returned checks. The only form of payment will be cash after a NSF incident. We do not pro-rate unless you will be gone 3 weeks in a one-month period. There are 52 weeks in a year. Our school will be closed for 4 weeks (which leaves 48 weeks) during the calendar year. Since some months have 5 weeks it will average out for you to attend 8 classes a month per calendar year on average. We offer classes 4 times a week plus an advanced classes. This will allow you to stay on track for testing and make up missed classes. Required attendance, Required Belt Stripes and Tuition need to be current to test. Testing fees are $ 0. At Black Belt there are testing fees and vary per Dan/Degree. At Green Belt you will need to buy sparring gear and the student will be eligible for Black Belt Club. If you buy sparring gear, weapons, etc. from any other source (ebay, a friend, off the internet) we will not allow it in class. We are very strict on safety and quality. Please see a 3-Tigers representative for details. At Black White belt you will be eligible for Master s Club.

Roughly every 3 months you have the opportunity test for a new belt. (if you have all required stripes on your belt). If attending school you will need to supply a S.T.A.R. report at testing twice a year. New Black Belt Club and Master s Club patches must be applied properly before your next class. Equipment, uniforms etc. must be paid for in advance before order will be placed. Orders are placed once a month on the 15 th. You must have required weapons for class prior to the start of a new cycle. Sparring gear that is too small or ripped/torn must be replaced immediately. We are not responsible for any equipment or valuables left on premises. Prices are subject to change. If that should occur you will be notified 60 day in advance. We reserve the right to change or terminate this agreement for any reason without refund or notice at any time.
