Exercise 2: Reviewing the Wrkplace Inspectin Checklist (30 min) p. 19-20 Part One: Behaviurs vs. Cnditins (15 min) Wrking individually: Think f 3 r 4 incidents that have ccurred r have the ptential t ccur n yur wrksite; Write each incident n a pst-it nte; Based n whether yu think the cause f the incident is an unsafe behaviur r an unsafe cnditin, pst the incident n the whitebard r flip chart under the apprpriate heading: Behaviur r Cnditin. As a grup, review each incident with yur instructr, discuss the categry and determine what t lk fr in the wrkplace inspectin. Fr example: Cnditin A maintenance wrker slipped n small puddle f water n the flr by the ht water heater. Behaviur A HCA hurt her back while transferring a resident withut a transfer belt. What wuld yu lk fr during the wrkplace inspectin? Check t see if the ht water tank was repaired r replaced (maintenance lg) and bserve the ht water tank area. Observe HCA's perfrming resident transfers t see if they are using transfer belts. Part Tw: Using the Wrkplace Inspectin Checklist (15 min) Wrking in pairs: Using the fllwing blank Resident Care Areas Wrkplace Inspectin Checklist, identify additinal cnditins r behaviurs in the spaces that wuld apply t yur facility. NOTE: we prvided ne example fr each standard. In the Additinal Observatins sectin, identify additinal standards, behaviurs and/r cnditins that wuld apply t yur facility. Be prepared t share the additinal cnditins and behaviurs yu have identified. As a large grup: Review each grup s additinal cnditins and behaviurs. Discuss the limitatins f inspectin checklists. 2012 Wrkplace Inspectins Wrkshp Handut 1

Check Yur Understanding p. 27 Take a minute t answer the fllwing questins based n the infrmatin presented s far. Be prepared t discuss yur answers. 1. What is a wrkplace inspectin? 2. Wrkplace inspectins shuld fcus n finding, nt n finding. (fill in the blanks) 3. What are the benefits f perfrming wrkplace inspectins n a regular basis? 4. What are the three types f wrkplace inspectins? (i) (ii) (iii) 5. Generally, wrk envirnments with fewer hazards require mre frequent inspectin than wrk envirnments with higher hazards. True False 6. Why is it imprtant fr wrkers t be part f the inspectin team? 2015 Wrkplace Inspectins Wrkshp Handut 4

Exercise 3: Planning fr a wrkplace inspectin p. 29 Take a few minutes t think abut planning an inspectin. If yu currently d frmal inspectins, think abut what yu d nw. If yu dn t, think abut hw yu d like t cnduct an inspectin. Cnsider the fllwing questins: Wh will cnduct the inspectin with yu? What areas will yu cver and hw will yu divide the inspectin areas? What infrmatin will yu review befre yu start? Hw much time d yu think it will take? Hw ften and during which shifts will yu cnduct wrkplace inspectins? 2015 Wrkplace Inspectins Wrkshp Handut 5

Hazard Assessment and Priritizatin p.36 There are several different methds used t evaluate risk. One methd invlves evaluating risk based n three factrs: Frequency: Degree f expsure t the hazard (i.e. number f times) Severity: Ptential cnsequences f expsure t the hazard Prbability: Likelihd f an incident ccurring The infrmatin is then applied t a risk evaluatin scale. Refer t the sample belw: Frequency Hw ften are yu expsed t the hazard while cmpleting this task? Risk Evaluatin Scale Severity Hw severe culd yur injury r illness be when expsed t this hazard? Prbability What are the chances that yu will be injured r becme ill by the hazard while ding this task? 4 One r mre times/day Serius injury/death Will very likely ccur (expected t happen) 3 One r mre times/week Medical aid Culd prbably ccur (has better than 50/50 chance f happening) 2 One r mre times/mnth Minr injury r illness/first aid Pssibility f ccurring (knwn t have happened) 1 Less than nce/mnth N injury/illness/damage Practically impssible t ccur (1:1 000 000) Add up the 3 numbers and rank the pririty based n the fllwing: Lw: 4-6 (L) Medium: 7 9 (M) High: 10 12 (H) Frequency (F) + Severity (S) + Prbability (P) = Ttal Risk 2015 Wrkplace Inspectins Wrkshp Handut 7

Eliminate Crrective Actins and Cntrls p. 37-38 When identifying crrective actins try t identify and crrect NOT nly the specific prblem (direct cause) but what is causing the prblem (indirect and rt causes). Engineering The first chice when dealing with hazards is t eliminate the hazard. If eliminatin is nt reasnably practicable, the hazard must be cntrlled. The Occupatinal Health and Safety Cde (Part 2, Sectin 9) require emplyers t apply the hierarchy f cntrls which are: Administrative Persnal Prtective Equipment Engineering Cntrls: Are ften thught f as hard cntrls. They deal with installatin r mdificatin f equipment that physically reduce the hazards. Examples include: ventilatin, guard rails, adjustable wrkstatins. Administrative Cntrls: These are ften thught f as sft cntrls. They deal with directing the activities f peple t reduce wrker expsure t the hazard. Examples include: practices and prcedures, training and instructin and rtating wrkers thrugh jbs t reduce expsure time. Persnal Prtective Equipment (PPE): PPE is intended t reduce the effect (severity) frm expsure t the hazard. PPE shuld nly be used t supplement engineering and administrative cntrls nt t be a substitute. Examples include: safety glasses, masks, gwns, glves and respiratrs. 2015 Wrkplace Inspectins Wrkshp Handut 8

What are Effective Cntrl Measures? Effective engineering, administrative and persnal prtective equipment cntrls satisfy the fllwing criteria: 1. They must adequately cntrl the hazard. 2. They must allw emplyees t d their jbs withut undue discmfrt r stress. 3. They must prtect every wrker at risk f being expsed t the hazard. 4. They must nt create new hazards. When the reprt is presented t management, each crrective actin will require an individual assigned t it t ensure implementatin is cmpleted and specific dates fr cmpletin must be determined. 2015 Wrkplace Inspectins Wrkshp Handut 9

Exercise 5: Identify Crrective Actins p. 39 Cntinue wrking in the same grups. Select three hazards frm the previus exercise, r frm yur wrk envirnment, and list them under Identified Hazards. Cmplete the hazard rating fr each hazard yu have selected. Nte: if yu have elected t select three different hazards frm yur wrk envirnment vs. frm the previus exercise, yu will need t cmplete the hazard rating befre determining the crrective actins. Then determine the apprpriate crrective actin(s) fr each hazard and cmplete the fllwing Crrective Actin sectin. Remember! The clser a cntrl is t the surce f the hazard the better. Hazards can be cntrlled: At the surce (where the prblem is created) Alng the path t the wrker (between the surce and the wrker) At the wrker (always the last chice) Identified Hazards Hazard Rating F S P TR P Crrective Actin 2015 Wrkplace Inspectins Wrkshp Handut 10

Exercise 6: Preparing the Inspectin Reprt p. 44-46 Cntinue t wrk in the pairs r small grups used fr the previus tw activities. Using the infrmatin btained during the inspectin, the hazards that were priritized and the crrective actins, cmplete as much f the fllwing Inspectin Reprt as pssible. Then: Determine wh the Inspectin Reprt shuld be presented t in yur facility; and, Determine wh wuld have the authrity t apprve the prpsed crrective actins and identify the individuals wh wuld be respnsible fr implementing the crrective actins. Be prepared t present yur Inspectin Reprt t the rest f the grup. As a grup, discuss hw crrective actins are typically implemented in the facilities. 2015 Wrkplace Inspectins Wrkshp Handut 11
