4 Guidance n the Cnstructin Phase Plan The Cnstructin Phase Plan January THE CONSTRUCTION PHASE PLAN Under the Cnstructin (Design & Management) Regulatins, it is a requirement f the Principal Cntractr t develp a Cnstructin Phase Plan, t an acceptable level, prir t being permitted t cmmence wrk n site. The Plan prvides a fcus fr managing and c-rdinating health and safety n the site. The amunt f detail in the Plan shuld depend n the nature and extent f the prject and n the cntract arrangements fr the cnstructin wrk. The Plan needs t explain hw key health and safety hazards will be managed, what the emergencies prcedures fr the site are, and the methds f cmmunicatin t be adpted. The Plan must be relevant t the particular prject and shuld build n the infrmatin prvided by the CDM Crdinatr and the Pre- Cnstructin Infrmatin. In additin, the Cnstructin Phase Plan shuld set ut the arrangements fr ensuring the health, safety and welfare f everyne carrying ut the cnstructin wrk and all thers wh may be affected by it. 5 Guidance n the Cnstructin Phase Plan Frmat fr the Cnstructin Phase Plan January FORMAT FOR THE CONSTRUCTION PHASE PLAN The fllwing lists what is expected frm the Principal Cntractr with regards the Cnstructin Phase Plan s that it satisfies the requirements f regulatin 20(1)(a) f the cnstructin Design and Management 2007,cmplies with regulatin 23(1)9a) and 23(2) and that the requirement f regulatin 22(1)(c) will be cmplied with during the cnstructin phase fr the prject prir t the start f the cnstructin phase. The sectin headings and their rder in the Plan fllw the recmmendatins given in the HSE's dcument 'Managing Health and Safety in Cnstructin - Apprved Cde f Practice. Hwever, it is nt a requirement that the Plan fllws this frmat, nly that all issues are addressed t a level that reflects the scale f the risks inherent n the prject. Descriptin f the Prject Prject descriptin List all the key elements f the cnstructin wrk being carried ut including any assciated infrastructure wrks and als demlitin and site clearance. Prgramme details State details f the anticipated cmpletin dates fr the prject and any intermediate stages. Prject team details List the names and cntact details f the prject team including the Client, Designers, CDM Crdinatr, ther Cnsultants, Cntractrs and Suppliers. Extent and lcatin f existing recrds and plans This sectin shuld reference available dcumentatin relevant t the existing envirnment such as: a) Drawings f existing structures t be demlished r incrprated int the prject e.g. steelwrk, brickwrk, reinfrced cncrete, pre-stressed cncrete, pre-tensined r pst-tensined cncrete etc. b) Asbests survey dcuments c) Sil survey dcuments d) Existing service details e) Any relevant Health and Safety Files if they exist f) any ther dcumentatin relating t the existing envirnment which culd be relevant t health and safety during the wrk activities assciated with the prject. These dcuments may have been issued at tender stage by the Client r have been btained by the Principal Cntractr thrugh site surveys. Cmmunicatin and Management f the Wrk This sectin shuld include details f the arrangements fr managing and rganising the prject. Management structure and respnsibilities Include infrmatin n wh is respnsible fr what, and the flw f respnsibility frm senir management thugh t site peratives. Fr example Prject Directr, Cmpany H&S Manager, Site Manager, Cntracts Manager, Safety Officer, Safety Inspectr, Site Fire Safety C-rdinatr, Site Freman, Site Operatives etc. Health and Safety gals Include infrmatin n the standards t which the prject will be carried ut e.g. t statutry requirements r higher if required by the Client r Principal Cntractr. Arrangements fr mnitring and review f health and safety perfrmance This sectin shuld include details n the mnitring and review arrangements fr achieving cmpliance with the health and safety standards set ut fr the prject. The Principal Cntractr shall set ut prcedures fr recrding and reviewing accidents, near misses, cmpliance with site safety rules, cmpliance with health and safety legislatin, cmpliance with safety methd statements and the Health and Safety Plan. The persns respnsible fr undertaking site safety inspectins and audits shuld be identified alng with the prpsed frequency t which they are t be undertaken. Arrangements fr regular liaisn between parties n site Include details f the arrangements that will ensure regular liaisn between parties n the site. This may be via direct cntact with the parties representatives r prject meetings. Arrangements fr cnsultatin with the wrkfrce Include details n the arrangements that are t be made fr cnsulting with and crdinating the views f wrkers n site. This might be via direct cntact with cntractrs' representatives r meetings with the fllwing n the agenda: a) Review f health and safety reprts. b) Reprting f accidents, dangerus ccurrences, near misses. c) Recmmendatins f any cntractrs. d) Pre-Planning f future wrk prir t the next meeting t include an update f the distributin and c-rdinatin f infrmatin t relevant parties. Arrangements fr the exchange f design infrmatin between the Client, Designers, CDM Crdinatr and the Cntractrs n site Describe the means f cmmunicating and passing design infrmatin t all members f the prject team e.g. via site meetings and prject reviews. Als describe the system fr assessing the health and safety implicatins f the design and the means f reprting back t interested parties. Arrangements fr handling design changes during the prject Include details f the arrangements fr the handling f design changes during the prject including any systems fr assessing the health and safety implicatins f any changes and reprting findings back t interested parties. Arrangements fr the selectin and cntrl f cntractrs and suppliers Set ut prcedures t: a) Judge that adequate prvisin will be made fr health and safety by cntractrs appinted by the Principal Cntractr. b) Ensure that suppliers f materials will prvide adequate details f their prducts, including apprpriate safety data sheets. c) Ensure that machinery and ther plant supplied fr cmmn use will be prperly selected, used and maintained; and that peratr training will be prvided. Arrangements fr the exchange f health and safety infrmatin between Cntractrs Describe hw health and safety infrmatin will be disseminated and exchanged between cntractrs (e.g. thrugh site safety meetings, prject meetings). Arrangements fr security Describe the arrangements fr securing the site and excluding unauthrised persns, including surveillance. The inductin shuld include basic infrmatin abut the prject including the main cntacts, plus relevant infrmatin abut the n-site hazards, emergency and first aid prcedures, and the duties and respnsibilities f all persns n site. Arrangements fr welfare facilities Describe the prpsed welfare arrangements and infrmatin abut any shared welfare in accrdance with the Cnstructin (Health, Safety & Welfare) Regulatins. This als needs t state wh is respnsible fr their maintenance and upkeep. Arrangements fr First Aid Include details n any first aid arrangements including the names f qualified persns appinted in accrdance with the Health & Safety (First Aid) Regulatins, hw this infrmatin is t be displayed and the lcatin f first aid bxes. Cnsideratin shuld be given t adequate cver during shift wrk. Arrangements fr the reprting and investigatin f accidents and incidents including near misses Include details f the prcedures fr reprting and investigating accidents, incidents and near misses. Accidents need t be entered in the Accident Bk B1 510 and if apprpriate reprted t the HSE under the Reprting f Injuries, Diseases and Dangerus Occurrences Regulatins. State wh is respnsible fr ding this. Arrangements fr the prductin and apprval f risk assessments and methd statements Include details f the prcedures/arrangements fr undertaking risk assessments in accrdance with the Management f Health and Safety at Wrk Regulatins and the prductin and apprval f methd statements. Describe hw infrmatin is t be btained frm ther cntractrs and hw essential health and safety infrmatin is t be cmmunicated t thse wh need t knw. State wh within the Principal Cntractr s prject team is respnsible fr this apprval prcess. Site Rules Give details f the site rules and the means f bringing them t the attentin f thse affected. Refer t and include site inductin infrmatin sheets. Fire and Emergency Prcedures Include details f the means f making peratives arriving n site familiar with the prcedures t fllw in the event f an emergency. The prcedures shuld be displayed, including details f the emergency assembly pints. Details f the Fire Safety Plan shuld als be included tgether with arrangements fr fire detectin, number f alarm and call pints, details f fire fighting equipment, interface between temprary and permanent installatins, training and instructin f wrkers with regard t fire and the testing/mnitring prcedures. Cnsideratin shuld als be given t emergency prcedures in excavatins liable t flding, tunnels, fr wrk near water and als fr the rescue f persns in harnesses. The emergency prcedures shuld als cver the immediate actins that need t be taken in the event f an accident when there is injury t ne r mre persns. Arrangements fr Cntrlling Significant Site Risks In rder fr an adequate Cnstructin Phase Plan t be in place, this sectin shuld include details f the cntrl measures in place t deal with the activities that have risks t health and safety, which are assciated with the early wrks packages. 8 Guidance n the Cnstructin Phase Plan The Cnstructin Phase Plan - A Live Dcument January THE CONSTRUCTION PHASE HEALTH & SAFETY PLAN - A LIVE DOCUMENT The Cnstructin Phase Plan is a live dcument and as such shuld be regularly reviewed and updated as wrks prgress n site. Where, during the cnstructin phase, the Principal Cntractr r ther cntractrs r Designers identify additinal hazards and risks they shuld identify the measures required t cntrl them and the Principal Cntractr shuld incrprate thse measures int his Cnstructin Phase Plan. If they affect the design, these variatins shall be discussed with the CDM Crdinatr. The Principal Cntractr shall als assess the cmpetence and resurces f all Designers and ther Cntractrs he appints, whether pre-selected by the Client r nt, and shall nly appint thse wh meet the requirements f Cmpetence Appendix ACOP. The arrangements fr undertaking this shuld be included in the Plan. The Principal Cntractr shall: Ntify the CDM Crdinatr f any design wrk that he r any ther Cntractr arranges t be undertaken. Arrange fr site safety inspectins and audits t be carried ut by cmpetent persns t review the health and safety aspects f wrk in prgress thrughut the cnstructin phase. Cntinually review and develp the Cnstructin Phase Plan, identifying and implementing any necessary changes frm any risk assessments, hazards identified and/r safety audits. Frmally reprt at each prgress meeting t the Client n health and safety issues arising during the previus perid including prpsed design changes and details f audits and inspectins and near miss incidents r incidents reprted under RIDDOR.
