4 1 PURPOSE The purpse f this prcedure is t prvide a mandatry prcess, guidance and infrmatin fr planning, managing and maintaining safe entry and wrk within cnfined spaces. This prcedure is designed t prvide clear guidelines that all wrkers are required t fllw when wrking in a cnfined space. Cnfined spaces are classified as high-risk areas and require specific planning, risk assessment, management and implementatin f cntrls in rder t minimise risks t safe levels befre entry fr wrk is authrised. Entry t cnfined spaces acrss all CS Energy sites is t ccur in cnjunctin with the CS Energy Permit t Wrk (PTW) Manual. NOTE: Where pssible, the need t enter r wrk within a cnfined space is t be avided. 2 SCOPE This prcedure is applicable t all persnnel requiring entry and wrking within a cnfined space at CS Energy wned and/r perated assets. A persn is cnsidered t have entered the cnfined space if their upper bdy r head is within the cnfined space. This prcedure is a cmpnent f CS Energy s verarching PTW Management System. Fr additinal infrmatin pertaining t the planning, administering and authrisatin prcesses relating t islatins and PTW prcess, refer t the PTW Manual. NOTE: The identificatin f, and wrk carried ut in Cnfined Spaces shall be in accrdance with Australian Standard 2865:2009 and the Permit t Wrk Management System 3 WHAT IS A CONFINED SPACE? A cnfined space means an enclsed r partially enclsed space that: Is nt designed r intended primarily t be ccupied by a persn; and Is, r is designed r intended t be, at nrmal atmspheric pressure while any persn is in the space; and Is r is likely t be a risk t health and safety frm: An atmsphere that des nt have a safe xygen level; r Cntaminants, including airbrne gases, vapurs and dusts, that may cause injury frm fire r explsin; r Harmful cncentratins f any airbrne cntaminants, r engulfment. Cnfined spaces are cmmnly fund in vats, tanks, pits, pipes, ducts, flues, chimneys, sils, cntainers, pressure vessels, undergrund sewers, wet r dry wells, shafts, trenches, tunnels r ther similar enclsed r partially enclsed structures, when these examples meet the definitin f a cnfined space in the WHS Regulatin. 5 4 CONFINED SPACE IDENTIFICATION Each CS Energy Site maintains a register f cnfined Spaces and is available t all persnnel fr reference in regards t the wrk they are carrying ut. The Site Manager shall delegate respnsibility t maintain the cnfined space register t the site Safety Team r ther grups as apprpriate. Any plant r structural mdificatin that ccurs in which a new cnfined space is created it shall be entered int the cnfined space register. As a minimum, the cnfined space register shall be reviewed every 2 years. 4.1 Cnfined S pace Identificatin A permanent Cnfined Space Entry sign shall be fixed t all penings fr each cnfined space; The sign must be legible, displayed in a prminent psitin and remain visible when the pening is pen; The sign must cmply with the requirements f AS/NZS Safe wrking in a cnfined space; and All penings shall be uniquely identified. 4.2 Identificatin f Cnfined S pace Nminated E ntry Pint Additinal signs are t be erected utside the nminated entry pint when the nminated entry pint is pen r when persnnel are wrking n/inside the cnfined space. The OIC is t apply an OIC Cntrl Pint lck and warning cntrl pint tag t every nminated entry pint. (The warning cntrl pint tag will be printed frm the nminated entry pint descriptins nminated by an OIC in the APTW and listed n the WCD r islatin sheet) 4.3 Identificatin f cnfined s pace nminated ventilatin pints Signage shall be installed t identify the cnfined space and t infrm wrkers nt t enter unless they have a cnfined space entry permit. Sign psting alne shuld nt be relied n t prevent unauthrised entry t a ptential space. Security devices such as lcks and fixed barriers shuld be installed. Cnfined Space pints are pened by the OIC wh applies the cntrl pint lcks (White) and tags and crdinates a secnd check by a cmpetent persn. 7 6 CONFINED ISOLATION If liquids, gases r vapurs culd enter the cnfined space, the pipe wrk shuld be physically islated. Psitive islatin is preferred if wrk inside a cnfined space is required while the unit is nline. There may be ccasins where it is nt pssible due t the plant design t utilise islatin with higher rder f cntrl. Under these circumstances it is imprtant that we ensure the islatin is: 1. Risk assessed t ensure the single pint islatin is safe while wrkers are inside the space. 2. Cnsideratin given t plant mdificatin t allw the psitive islatin in the future. Refer t Sectin 9 f the CS-PTW-ISO-01 Safe Mechanical Islatin fr mre infrmatin 7 CONFINED SPACE ENTRY The need t enter a Cnfined Space will be identified thrugh the cmpletin f a JSEA, which identifies the requirement fr a: A PTW with the SAP hazard Cnfined Space ; Cnfined Space Risk Assessment (refer t S1891); Emergency Respnse Plan (refer t S1889); and Atmspheric Testing (refer t S1890). All cnfined space entry will require: A JSEA and Cnfined Space Risk Assessment t be cmpleted; Unbstructed entry and exit penings large enugh t allw fr emergency access A standby persn assigned t cntinually mnitr the well being f thse inside the space, bserve the wrk being carried ut and t initiate apprpriate emergency respnse prcedures when necessary. A cmmunicatin system t enable cmmunicatins between inside/utside the cnfined space t summn help in an emergency. An Emergency Respnse Plan develped by the OIC in cnsultatin with the wrkers invlved; A rehearsal f the Emergency Respnse Plan t be cnducted prir t entering the cnfined space. Specific retrieval equipment (if required) and a cmpetent user(s) lcated at the entrance t the cnfined space. Initial atmspheric testing shuld be dne utside the Cnfined Space by inserting a sample prbe at apprpriately selected access hles, nzzles and penings (tp, middle and bttm shuld be tested). Onging testing requirements shuld be included in the risk assessment. Mnitring results (S1890) shall be attached t the Entry Permit. The Entry Permit has a dcumented acknwledgement by the OIC that all wrkers have left the cnfined space Cnsideratin shuld als be given n hw emergency services are ging t be ntified f the dangers f the cnfined space when they arrive. 8 CS Emergency respnse teams (ERT) must have an entry permit t enter a cnfined space in an emergency. Nte: Cnfined space entry is nt intended t prevent a persn frm inserting their hand r arm while hlding a test instrument r prbe int the cnfined space as part f the evaluatin prir t entry 7.1 Cnfined Space Openings Dependant n use at the time an pening may be a nminated ventilatin pint r a nminated entry pint r nt used. The OIC will nminate all the entry pints and ventilatin pints listed n the PTW Applicatin. After the OIC has been issued with the PTW every nminated entry pint and nminated ventilatin pint, is t be lcked pen by the OIC using an OIC Cntrl Pint lck and warning - cntrl pint tag. Warning cntrl pint tags will be printed with a clear descriptin f the entry pint t be pened and lcked. The key/keys t these lcks must be stred in the lckable cmpartment f the PTW bard as sn as the lcks are applied. Once the OIC Cntrl Pint lcks and warning - cntrl pint tags are applied the OIC must initial the step n the islatin sheet as cmplete. N entry is permitted int the cnfined space until the keys have been secured in the PTW bard and risks have been cntrlled (Refer t Sectin 7.6). Cnfined space nminated entry pints and nminated ventilatin pints shall nly be pened: a) By an OIC: Wh hlds an apprpriate PTW fr that piece f plant; and Wh has the nminated pints listed n their issued islatin sheet; and Wh has the apprpriate OIC Cntrl Pint lck and warning - cntrl pint tag; r b) By a PTWO (Only if it is designed and safe t be pened t meet peratinal requirements as a part f the islatin prcess): Wh is using the pening fr ventilatin; r Wh will lck the nminated entry pint pen with a red islatin lck and tag as a part f the PTW; and Will barricade the penings t prevent unauthrised persnnel frm entering. As sn as it is practical, the nminated entry pint(s) and nminated ventilatin pint(s) must be secured with an OIC Cntrl Pint lck and warning - cntrl pint tag (r red islatin lck and danger tag if undertaken by a PTW Officer). If an pening it is nt t be used as a nminated entry pint r nminated ventilatin pint, and requires the pening t assist with wrk (i.e. lighting, scaffld), it is t be barricaded and signed (Danger Sign) t prevent persnnel accidentally entering the space. Nminated Entry pints int cnfined spaces: Must permit rescue f all persnnel that enter the cnfined space; Must be kept clear f any bstructins; Page 8

9 Where necessary be prvided with scaffld platfrm, stairs r ladders t allw safe and cnvenient entry fr wrk and rescue purpses. Wrk party members must enter and exit frm the same cnfined space nminated entry pint t ensure a dynamic recrd is maintained at all times C nfined S pac e Nminated E ntry P int (withut the ptential t caus e entrapment) Where a cnfined space has a nminated entry pint that has n ptential t cause entrapment (e.g. pen pits/drains where entry/exit is nt cmprmised) is required t have: An additinal sign t be erected utside the cnfined space nminated entry pint C nfined S pac e Nminated E ntry P int (ptential t caus e entrapment) Where a cnfined space has a nminated entry pint that can be clsed t cause entrapment (e.g. drs, lids, hatches, manhle cvers etc NOT pen pits/drains where entry/exit is nt cmprmised) it is cnsidered as an Cntrl Pint and is required t have: An additinal sign/s t be erected utside the cnfined space nminated entry pint; and An OIC Cntrl Pint lck and warning - cntrl pint tag t secure pened drs. (The warning - cntrl pint tag will be printed frm the nminated entry pint descriptins by an OIC in the APTW and listed n the WCD r islatin sheet) 7.2 Multiple Wrkgrups Operating S imultaneusly in the S ame Cnfined S pace A cnfined space crdinatr is used when there are tw r mre PTW wrk parties cnducting wrk inside a single cnfined space and have the ptential t impact ne anther. A single cnfined space crdinatr will be appinted t crdinate all wrk between wrk parties being cnducted in that cnfined space. The cnfined space crdinatr: Will be appinted during Overhauls and Unit utages r under ther circumstances as deemed apprpriate. Will be nminated by the Overhaul r Outage Manager (r Maintenance Superintendent where there is n utage r verhaul). Will crdinate nminated cnfined space/s. The nminated cnfined spaces will be at the discretin f the Overhaul r Outage Manager. (e.g. during a majr verhaul, the area may cver the entire biler, back end, air heaters, flue paths n the precipitatrs that share the same atmsphere) Will crdinate effective cmmunicatin and activities affecting the atmsphere between all OIC s wrk in the cnfined spaces. Will have verriding authrity t determine if a planned wrk activity shuld r shuldn t cmmence r cntinue. Will ensure the cnfined space is cleaned t its riginal state. Manage the cnfined space bx-up and returning the tags and lcks. Each individual wrkgrup will still be assigned an OIC fr the wrk being cnducted in the cnfined space wh is respnsible fr ensure the safety f the wrkgrup in the cnfined space. This OIC will be required t liaise accrdingly with the cnfined space crdinatr. Page 9

10 A crdinatin meeting is required between all OIC s regarding multiple wrkgrups carrying ut different tasks in the ne cnfined space. The meeting may be held at: the start f each shift, r the transfer f OIC, r where the cnditins change within the cnfined space that intrduces the pssibility f new hazards, r where a new permit is issued minutes f the meeting are t be recrded. Where there are multiple tasks required t be carried ut in the same cnfined space there may be ne encmpassing PTW permit issued that will specify each task undertaken in the space. Individual JSEA s shall be cmpleted fr each task inside that space and the necessary cntrls implemented. 7.3 Cnfined S pace Islatins during Overhaul Cnditins During verhaul cnditins, where a number f cnfined space wrk areas are required t be accessed by numerus wrk parties simultaneusly, it can be difficult t issue and manage PTW s with the same cnfined space n multiple PTW s. It is permissible t manage these islatins by linking the cnfined space ventilatin pints and cnfined space access pints t a Wrk Clearance Dcument (WCD) acting as a multiple. The islating fficer is t lck the white POC lcks t the islatin and the white keys are t be maintained n the crrespnding islatin sheet rack (grid reference) in the PTW ffice. When multiple wrk parties are wrking simultaneusly in the same cnfined space a cnfined space crdinatr is t be appinted as per Sectin Cnfined space within a cnfined space The PTW will be issued allwing entry t the cnfined space. Every nminated entry pint and nminated ventilatin pint t be pened must be identified n the islatin sheet and be allcated its wn OIC Cntrl Pint lck and warning - cntrl pint tag. A cnfined space within a cnfined space shall be treated as an independent cnfined space and cntrls must be implemented. The JSEA shuld reflect the hazards identified fr the cnfined space and the additinal cnfined space within the cnfined space. (e.g. JSEA will incrprate additinal stand-by persn r atmspheric testing requirements) 7.5 S tandby Persns It is a requirement at CS Energy that all cnfined space entries have a standby persn fr that entry; It is permissible, where plant cnfiguratin permits (e.g. tw entry pints side by side, etc.), a cnfined space standby persn can fulfil the rle fr multiple cnfined space entry pints; Nte: Wrk Health and Safety Regulatin 2011 Regulatin 69 states; A persn cnducting a business r undertaking must ensure that a wrker des nt enter a cnfined space t carry ut wrk unless the persn prvides a system f wrk that includes:- (a) Cntinuus cmmunicatin with the wrker frm utside the space; and Page 10

11 (b) Mnitring f cnditins within the space by a standby persn wh is in the vicinity f the space and, if practicable, bserving the wrk being carried ut. 7.6 E quipment Us ed in Cnfined S paces All prtable electrical equipment shall be cnnected t, individually r cllectively t a safety switch Residual Current Device (RCD), with the device lcated utside the cnfined space. Ignitin surces must nt be intrduced int a cnfined space (frm utside the space) if there is a pssibility f the ignitin surce causing a fire r explsin in the space (flammable atmsphere). N cylinder f cmpressed r liquefied gas, ther than thse used fr self-cntained breathing apparatus, shall be taken int the cnfined space. The gas supply shall be turned ff at the cylinder valve when nt in use. The gas cylinders shall be secured. Hses supplying gas perated equipment, shall be inspected and tested prir t installatin, and guarded t avid accidental damage; All temprary lighting (including emergency lighting) shall be prtected against damage. 7.7 Risk Assessing Cnfined Spaces Prir t entry int a cnfined space a cmpetent persn must cmplete a JSEA, Cnfined Space Risk Assessment and Emergency Respnse Plan specific t the wrk required t be undertaken. The JSEA shall take int accunt the fllwing: The wrk required t be dne, including the need t enter the cnfined space; The range f methds by which the wrk can be dne; The hazards invlved and assciated risks invlved with the actual methd selected and equipment prpsed t be used; The cmpetence f the persns t undertake the wrk; Impacts n r frm wrks in adjacent areas; The hierarchy f cntrl methds; and The first aid and emergency respnse requirements; and The requirements fr entry/exit and ventilatin f the cnfined space. The Cnfined Space Risk Assessment shall take int accunt the fllwing: The nature f the hazards f that cnfined space and impacts frm wrks in adjacent areas; The hazards invlved and assciated risks invlved with the actual methd selected and equipment prpsed t be used; Atmspheric hazards and additinal required atmspheric testing; and Stand-by and emergency respnse requirements. The Emergency Respnse Plan shall take int the accunt the fllwing: Page 11

12 Emergency respnse requirements; Emergency equipment requirements; Trained Rescue Persnnel required; Rescue plan and rehearsal; First aid prcedures; and Assessed by the site Emergency Respnse Team. The JSEA, Cnfined Space Risk Assessment and Emergency Respnse Plan shall be reviewed: Each time a cnfined space is made available fr entry (prir t entry ccurring); When additinal r new hazards have been intrduced int the cnfined space; Any time the cnditins change inside the cnfined space; Whenever there is evidence that the initial assessment in n lnger valid. Fr impacts frm wrks in adjacent areas. 7.8 Cnfined S pace Islatins All islatins must be cmpleted in accrdance t the CS Energy PTW Manual. Yu must ensure that all ptentially hazardus services are islated prir t any persn entering the cnfined space. If gas, fumes r vapurs culd enter the cnfined space the pipe wrk needs t be physically islated. The minimum requirement fr islatin shall be ne f the fllwing: Remving a spl piece r expansin jint, blanking and capping the pen end and tagging t indicate purpse. Inserting a blank/spade between the flanges as clse as pssible t the cnfined space and tagging t indicate purpse. Clsing, lcking and tagging at least 2 valves in the piping leading t the cnfined space. A drain r vent valve between the 2 islatin valves shuld be lcked pen t atmsphere (duble blck and bleed). In all cases, yu shuld physically cnfirm that the islatin is effective. Islatin shall be undertaken t prevent; Inductin f cntaminants thrugh piping, ducts, vents, drains, cnveyrs, service pipes and fire prtectin equipment. The activatin r energising f machinery in the cnfined space The activatin f plant/services utside the cnfined space that culd adversely affect the space The release f any stred r ptential energy in the plant and The inadvertent use f electrical equipment. 13 The islatin shall nt be de-islated until all wrk is cmplete and all wrkers have left the cnfined space. 7.9 Air Quality Tes ting Air quality testing shall be cnducted prir t any entry r wrk inside a cnfined space. Initial atmspheric testing shuld be dne utside the Cnfined Space by inserting a sample prbe at apprpriately selected access hles, nzzles and penings (tp, middle and bttm shuld be tested). Onging testing requirements shuld be included in the risk assessment. Mnitring results (S1890) shall be attached t the Entry Permit. Testing must be undertaken in a manner that encmpasses all pssible atmspheric related hazards as specified n the Cnfined Space Risk Assessment f that particular cnfined space. The Air Quality Tester shall be cmpetent, authrised and have lcked and signed nt the Permit that allws entry t the cnfined space t cnduct an air quality test. This task shall be included n the relevant Cnfined Space Risk Assessment assessing all the assciated risks. Prir t the entry f the wrk party, the atmsphere f a cnfined space must meet the fllwing minimum requirements: The atmsphere must cnfirmatin that the cncentratin f any flammable gas, vapur r mist in the atmsphere f the space is less than 5% f its LEL (lwer explsive level) if this is nt reasnably practicable that when it is equal t r greater than 5% but less than 10% f its LEL that cntinuus mnitring is established r wrkers are immediately remved frm the space. If it is equal r greater than 10% f its LEL then all wrkers must be remved frm the space. Where air quality cnditins are mdified r changed in a cnfined space (e.g. release f airbrne cntaminants) a risk assessment must be cmpleted t identify the need fr retesting r cntinuus mnitring f the air quality. A persn shall nt wrk in a cnfined space where the air quality tests indicate an envirnment that has an unsafe r cntaminated atmsphere Purge and Cleaning Purging (ventilatin) and cleaning (e.g. hse-dwn) f a cnfined space prir t entry is an effective cntrl measure. Purging shuld be cmpleted using a ventilatin device cnsistent with the Cnfined Space Risk Assessment requirements. At n stage is pure Oxygen t be using fr the purging prcess as it may lead t an Oxygen enriched envirnment, which hlds the ptential t develp int an explsive envirnment. Page 13

It is mandatry fr the persnnel in cntrl f the cnfined space t develp an Emergency Respnse Plan (S1889 Cnfined Space Emergency Respnse Plan) fr each PTW cnfined space entry. This Emergency Respnse Plan can be generic r specific t a high-risk activity/wrk. The Emergency Respnse plan shuld be assessed by the site Emergency Respnse Team This Emergency Respnse Plan is required t be discussed prir t entry f the cnfined space and rehearsed. Specific cnfined space Emergency Respnse Plans will be managed at each CS Energy site as per their sites prcesses and resurces. This will be cnsistent with CS Energy Emergency Management Prcedures and their current rganisatinal arrangements. CS Energy Emergency/First Respnse Teams (ERT) must have an entry permit t enter a cnfined space in an emergency. The stand-by persn/s is at NO time allwed t enter a cnfined space. The key/s t these lcks must be stred in the lckable cmpartment in the PTW bard as sn as the lcks are applied. Once the OIC Cntrl Pint lcks and tags are applied the OIC must initial the step n the islatin sheet as cmplete. Ntifying the Standby Persn if there is a issue inside the space Exiting the space if atmspheric mnitrs indicate a unsafe envirnment (g int alarm), in the event f an emergency r if instructed by the Standby Persn. 8.4 S tandby P ers n Respnsibilities include: Identifying and testing methd f cntinuus cmmunicatin with OIC and persns wrking in the cnfined space; Ensuring any emergency recvery equipment, mnitring equipment and Emergency Respnse Plan are in place prir t wrk cmmencing; Ensuring they remain utside the cnfined space at all times; and Ensuring dcumentatin fr entry in t the cnfined space is lcated in prximity t the nminated entry pint f the cnfined space. 8.5 Cmpetent Air Quality Testing Persnnel Respnsibilities include: Undertaking Air Quality Testing: with calibrated equipment; challenging the test equipment prir t use; in accrdance with the hazards identified in the JSEA & Cnfined Space Risk Assessment.
