2 Cnfined Space Entry ENTRY The act by which a persn intentinally passes thrugh an pening int a permit required cnfined space. Any part f the bdy passing thrugh the pening is cnsidered entry. IDLH (IMMEDIATELY DANGEROUS TO LIFE OR HEALTH) Any cnditin which pses an immediate threat t the health f life n an entrant, r; Wuld cause irreversible adverse health effects, r; Wuld interfere with an individual s ability t escape unaided frm a permit space. ENTRANT The emplyee wh will physically enter the cnfined space t perfrm the wrk. ATTENDANT The emplyee wh remains utside the cnfined space and mnitrs the entrant(s); guards the space against unauthrized entry; warns the entrants f any unusual cnditins; and summns the rescue persnnel if needed. Permit-Required Cnfined Space A Permit-Required Cnfined Space is cnfined space that has ne r mre f the fllwing characteristics: Cntains r has the ptential t cntain a hazardus atmsphere; Cntains a material that has the ptential fr engulfing an entrant; Has an internal cnfiguratin such that an entrant culd becme trapped r asphyxiated; r Cntains any ther serius safety r health hazard. Entry Supervisr The emplyee respnsible fr crdinating the entry int the cnfined space. This must be a team leader r freman. 1-Cnfined Space Hazards Cnfined Space Entry

3 Respnsible Persn Cnfined Space Entry The persn directly respnsible fr the wrk being perfrmed in the cnfined space. This can be the Team Leader, Freman, jurneyman, r ther persn qualified by training and experience. Nn-Permit Cnfined Space A cnfined space that des nt cntain r, with respect t atmspheric hazards, have the ptential t cntain any hazard capable f causing death f serius physical harm. 5 Cnfined Space Entry Permits are valid fr up t 12 hurs. When wrk is cmpleted, permit and tailbard frm shuld be returned t safety. Cancelled permits must be kept n file fr at least ne year. Test the Atmsphere In this Order Check fr Oxygen Cntent: At least 19.5% and less than 23.5% Check fr Cmbustibles: Less than 10% f the LEL Check fr Txic Gasses: Mst cmmnly carbn mnxide (PEL <35 ppm) Or any ther hazardus materials as determined by the use f the space. NOTICE: Any time a limit is exceeded, n matter what the reasn, all persnnel shall immediately exit the space, and n thers shall enter until atmspheric cnditins are returned t safe levels. THERE ARE NO EXCEPTIONS TO THIS! Atmsphere Testing Shall Be Perfrmed: Prir t every entry when the space is vacant; After a 10 minute ventilatin perid (if ventilatin is necessary); At least hurly fr permit-required cnfined spaces. He shall be in cnstant cmmunicatin with the entrants while the jb is in prgress. All entrants shall sign the sign in lg when entering the space and sign ut when exiting. The attendant shall maintain the permit and sign in lg fr the duratin f the wrk. When the Jb is Dne: Remve all persnnel, tls, and debris frm the space. Sign ff the lg. Clse the space. Cancel the permit. Cntractr Cnfined Space Entry Cntractrs must be infrmed f the hazards within the space Cntractrs must fllw their wn established cnfined space entry prcedure and use their wn permit frms Cntractrs must supply their wn attendants One attendant is acceptable fr multiple cmpanies entrants Cntractrs must supply their wn air mnitrs Cntractrs must review entry after cmpletin f jb Attendant Respnsibilities T mnitr entrants during the jb and during entry & exit t help insure their safety. The attendant may nt abandn his pst fr any reasn while persnnel are in the space unless relieved by anther qualified attendant. T mnitr atmspheric cnditins in the space prir t and during entry. T cntrl access t the cnfined space. T summn emergency assistance as needed. T assess hazards in and arund the space, and take actin n the same. T immediately exit a space, withut questin, upn wrd f the attendant, n matter what the reasn. T fllw all safety rules and prcedures that applies t the jb. T be familiar with the wrk t be perfrmed and the prcedures that applies t the jb. T use the apprpriate PPE whenever necessary. Supervisr Respnsibilities T assure adequate prtectin is prvided t the entrants by verifying adequate lckut/tag ut and that all hazards are securely islated. T supprt the attendant s authrity in cntrlling access t a cnfined space. T verify that all persnnel have exit prir t clsing the space.
