2 1 Persnal prtective equipment Minimum requirements fr persnal prtective equipment are given in the dcument "General EHS requirements fr Cntractrs". Additinal requirements fr persnal prtective equipment may arise frm a safe jb analysis (SJA) r apply t specially marked areas. The fllwing may apply: Eye prtectin Safety gggles using angle grinders and at risk f chemical spills and dust Face shield/ mask at risk f chemical splash and when wrking with special chemicals Welding gggles/ welding shield fr welding wrk Respiratry prtectin Dust mask, minimum P3 filter, at risk f inhalatin f dust Gas/vapur filter r supplied- air respiratrs at risk f inhalatin f hazardus gases Supplied- air respiratrs in case f cntaminated wrking envirnment Hearing prtectin In nise znes (>85dB) Dispasable wrkwear Safety glves At risk f chemical spillage, dust and bilgical infectin Cut resistant glves fr ht wrk and at risk f mechanical damage Chemical glves when handling chemicals and infectius waste Fall prtectin When wrking at heights ver meters and at risk f falling Persnal gas mnitr At risk f hazardus cncentratin f H S, CO, O and LEL The list is nt exhaustive and prtective equipment shall be assessed in relatin t the identified risks. It is the Cntractr's respnsibility t have the required prtective equipment available. Persnnel shall have dcumented training in the use f persnal prtective equipment and the hazards it prtects against. The training shall als include strage, maintenance and cntrl f the equipment. 3 Lightning The fllwing is intended t ensure adequate wrking t ensure that wrk can be dne safely, cmfrtably and withut risk t persnnel r equipment. Respnsibility Purchaser: Cnnectin pints fr pwer Sufficient light in the premises Fr supplementatin when general lighting is weakened by prvisinal installatins Emergency lightning f sufficient strength Escape rutes shall be illuminated Respnsibility Cntractr: Use f apprved lighting fixtures and cables t ensure adequate wrking light Cnstricted r cnfined spaces shall be illuminated The vltage shuld be lwer 5V and with earth-leakage circuit breaker Lighting installatins in wrking rms, wrkplaces, access rads, transprt rads and carg area shall be placed in such a way that it des nt represent any risk t the persnnel Side 3 (14)
4 3 Entering and wrking in cnfined spaces Entering and wrk in cnfined spaces include entering and wrking in cntainers, bilers, tanks, sils, wells, pipelines and ditches deeper than meters and ther cnfined spaces with pr air exchange. The Purchaser shall have a list f permanent cnfined spaces n their facilities. Fr wrk in cnfined spaces, the Purchaser will prepare lcal instructins which cver this type f wrk. In additin, the fllwing shall be available befre entering and wrk in cnfined: Wrk descriptin Risk assessment / SJA Safety and rescue plan Requirements fr persnal prtective equipment and ther safety equipment Special requirements t equipment fr wrk in cnfined spaces Persnnel wh is ging t carry wrk in cnfined spaces shall have training required fr this type f wrk. The Purchaser shall perfrm gas measurement befre entering. Wrk certificate shall be psted at the entry pening. When entering, there shall always be at least tw persns present. An entry guard shall stand utside t mnitr the wrk. Entrance guard shall be able t cmmunicate with the persn (s) wrking inside the cnfined space. If, after a risk assessment, ne cnsiders the wrking atmsphere and ther factrs nt t represent any risk, the requirement fr a guard may be waived. This shall be reflected in the wrk permit. Additinal requirements fr prtective equipment are lanterns and gas meters fr H S, CO, O and LEL. In the event f an accident, the cntrl rm shall be ntified which then cntacts emergency services. Entrance guard may under n circumstances enter the cnfined space until emergency persnnel have arrived. Side 4 (14)
5 4 Wrking at height Wrking in height means wrk that is carried ut mre than meters abve grund / flr lever. Persnnel wh wrk at height shall have apprved training and shall be infrmed f the risk invlved. All wrking in height requires a wrk permit (WP) and shall be risk assessed using a safe jb analysis (SJA). Prtectin against falling shall be risk assessed at all wrk places, access and transprt rutes where there is a risk f a fall t a lwer level, even when the height is belw meters. Fall prtectin equipment shall be used when wrking ver meters at risk f fall. The equipment shall be hked n t a sufficiently slid anchr pint. Fall prtectin equipment shall be used when ther preventive measures are nt an ptin. Fall prtectin equipment shall always be used when wrking frm a lift. As a safeguard at least persns shall be present when fall prtectin equipment is applied. Buddy check shuld be perfrmed. The use f fall prtectin equipment requires cmpleted and dcumented training. Tls and wrk materials shall be secured t prevent fall t lwer levels. Barrier ff areas where there is a risk f falling bjects. Barrier shall be tagged with date and time fr startup and finish f the wrk, as well as name f respnsible persn, cmpany and mbile number. Upn terminatin f the barrier, all chains and tags shall be remved. Side 5 (14)
6 5 Lifting peratins and lifting equipment All lifting equipment shall be apprved and tagged in accrdance with applicable regulatins. Cranes and crane peratrs shall be certified, and certificates shall at all times be presented at inspectins. Rigger, slingers and signalmen shall have relevant and dcumented training. Planning f lifting peratins is carried ut by using safe jb analysis (SJA). Crane peratr shall prepare a wrk descriptin in cnnectin with blind lifts and tandem lifts. The lifting area / rute shall always be planned s that the lad is nt carried ver persnnel r ther critical structure r equipment. The lifting area / rute shall always be evacuated and barrier ff at a safe distance. Suspended lads shall be secured with wire straps and chains. Suspended lads shall never be left unattended. Fr cmplicated lifting peratins, a written safety plan shall be prepared. The plan shall be apprved by the Purchaser. Use f persnnel lifts requires valid dcumented training. Fall prtectin equipment shuld always be used when wrking n a lift. The crane t be used fr the lift shall be apprved fr this. Side 6 (14)
7 6 Ht wrk Ht wrk is defined as wrk that generates sparks r invlves the use f flames r ther heat surces that pse a fire hazard. Ht wrk include the use f pen flames, ht air, cutting and/r grinding equipment. Ht wrk shall nly be carried ut by persns wh have a valid certificate fr ht wrk issued by the Nrwegian Fire Prtectin Assciatin (Nrsk Brannvernfrening) r cperating rganizatin in the ther Nrdic cuntries. In additin t WP and SJA, Wrk instructin when perfrming ht wrk shall be cmpleted. The insurance cmpanies safety regulatin fr ht wrk shall be fllwed. A fire guard shall at all times be present and cntinuusly evaluate the risk f fire during the wrk, during breaks and at least n hur after the wrk has been cmpleted. A fire guard shall als have a valid certificate fr ht wrk. Additinal requirements fr persnal prtective equipment fr ht wrk are: Gggles Face shield / mask shuld be used nly with safety gggles r safety glasses with side shields Cut resistant glves (accrding t NS-EN 388 level 3) Angle grinders are nly permitted when they meet all safety requirements fr safety features, including auxiliary handles, kickback prtectin, safety switch and restart prtectin. Persnnel wh are ging t use this type f tls shall have training. Apprved extinguishing equipment shall be brught by the Cntractr. Minimum pcs. 6 kg pwder apparatus, with minimum pwer 34 A 33 B C, shall be easily accessible in the immediate vicinity f where the wrk is carried ut. A hand extinguisher can be replaced with a fire hse with an internal diameter f 19 mm. Side 7 (14)
8 7 Wrk n pressurized equipment The Cntractr shall btain a wrk permit (WP) and any safe jb analysis (SJA) prir startup. The wrkplace shall be islated frm the rest f the plant in gd time befre the wrk starts. Wrk shall nt start until temperature has reached 55 C. Check the pssibility f drainage and that the valves are tight. Pressure relief and drainage shall be perfrmed by certified perating persnnel. The Cntractr shall ensure that the system is cled and depressurized. In case f dubt, the wrk shall nt be started. At pening, at least tw persns shall be present. Please nte that water may has accumulated in the bttm f the bject. This water may still has a high temperature. When the wrk has been cmpleted, the Cntractr shall ntify the cntrl rm fr resetting t nrmal peratin. Side 8 (14)
9 8 EX wrk EX wrk includes wrk that takes place in an explsive atmsphere. Persnnel wh wrk in areas where explsive atmsphere can be frmed shall have explsin prtectin training. Wrk in EX znes requires wrk permits (WP) and shall als be risk assessed using a safe jb analysis (SJA). Wrk in EX znes shall be carried ut in accrdance with written instructin. Additinal requirements fr persnal prtective equipment fr EX wrk are antistatic wrk clthes. Mbile phne r cmmunicatin radi (walkie talkie) shall nt be brught int any EX zne if it is nt apprved fr use in the relevant zne. Wrk certificate fr gas measurement is required befre the starts and shall be placed well visible where the wrk site is entered. The measurement shall ensure that persnnel are nt expsed t txic substances. Gas measurement shall nly be perfrmed by qualified persnnel. It is expected that persnnel are familiar with the site s explsin prtectin dcument and fllw the requirements and guidelines set by it. Side 9 (14)
10 9 Chemicals Cntractr shall ntify which chemicals t be used at the wrkplace. Purchaser shall receive safety data sheets relevant chemicals fr review and apprval. Safety data sheets shall be available at the lcatin and the Supplier shall ensure that its persnnel have training in its use. Use f chemicals shall be included in risk assessments. All cntainers shall be labeled with labels indicating which chemicals they cntain. The Cntractr shall take measures t prevent accidents and spills f il, fuel and chemicals stred and used at the wrkplace. The Cntractr shall have all necessary permits fr the use f chemicals, as well as the transprt and handling f explsives. It is Cntractr's respnsibility t remve all chemicals when the wrk is cmpleted. Legal requirements fr use, strage and dispsal f chemicals shall be adhered t. Side 10 (14)
11 10 Cmpressed gases All use f gas bttles shall be agreed with the Purchaser. Cntractr shall ensure that safety data sheets fr gas cylinders in use in the area, is available and that user has familiarized himself with this. Damaged gas cylinders shall nt be used r stred in the area. Bttles shall be stred prperly and as instructed by the Purchaser. Placement shall take int accunt any emergencies that may ccur t enable rapid remval frm the area. Bttles shall be used and stred standing, and it shall be ensured that they cannt tip using a wall bracket, suitable fixed r mbile frame. Bttes cntaining gas and empty bttles shall be stred separately. Empty bttles shall be labeled. Tm/Empty. Bttles with different cntents shall nt be stred tgether (flammable gases, inert gases, xidizing gases etc.). Gas bttles shall be stred well away frm cmbustible materials such as fuel, il, paint r crrsive liquids. Prpane bttles shall never be stred clser than five meters frm ther gas cylinders. Prpane, and ther crygenic, liquid r cmpressed gases heavier than air shall nt be stred in r near lw-lying areas such as drains, basements and underpasses. Side 11 (14)
12 11 Access and barriers Ladders shall nly be used fr access. The ladder shall be well secured/anchred. Scafflding shall be prperly designed, cnstructed and maintained by qualified persnnel. Inspectins shall be perfrmed every 4 weeks and after each change. Scafflding shall be equipped with scafflding cards. Scafflding shall have a safe access and shall nt blck the escape rutes. All scafflding shall have kick plates and knee braces. In areas where ht wrk is perfrmed, wd shall nt be used. Scafflding users shall be trained in the use f the scafflding. The Purchaser shall ensure that a scafflding reprt is prepared if the flr is higher than 5 meters abve the grund. Barriers shall prevent peple frm entering hazardus areas (danger f falling, falling bjects, etc.) r prtecting equipment and installatins. Red/white chain is used when nly invlved persnnel can crss the barrier. Yellw/black chain is used when the barrier allws thers t pass, prvided that particular care is shwn. Side 1 (14)
13 1 Preparing bjects (lckut/tagut) The guidelines will ensure that machinery and equipment are emptied f hazardus energy and that it is lcked and labeled as lng as maintenance is in prgress. Hazardus energy are Electrical, Hydraulic, Pneumatic, Chemical, Radiatin, Fall r gravity, Mechanical and Thermal enery. Prcedure fr implementing lckut and tagut: 1. Map and prepare fr lckut and tagut: a. All energy in the system shall be mapped. b. Wrk instructins fr discnnectin shall be prepared. c. Wrk requires wrk permit (AT). d. Risk assessment thrugh a secure jb analysis (SJA).. Ntify all interested parties: a. What, why, hw lng, respnsible persn. 3. Shut dwn and stp the system: a. Accrding t the system's instructins fr shutdwn. b. Check that all switches are in the "ff" psitin. c. Check that all mving and rtating parts have stpped cmpletely. 4. Detect, discnnect and islate energy supply: a. Identify and check all dangerus energies listed in paragraph 1. b. Discnnect and islate energy surces and residual energies. Use written wrk instructins fr the system r prcess. c. Empty r islate residual energy and stred energy accrding t prcedure / instructins. 5. Lck and tag: a. Lck switches and valves in "OFF" by setting the padlck n the devices that islate the pwer supply. b. Similarly, lck stred residual energy blckages. c. Tag lck with discnnect infrmatin. 6. Ensure that lcks and tags are nt remved befre wrk is dne. 7. Check that the system is shut dwn and dead: a. Chse the methd that best ensures that the system is discnnected and emptied, r fllw the prcedure/instructins. 8. Perfrm maintenance. 9. Finish maintenance: a. Cnsider whether it is safe t put the system in peratin. Side 13 (14)
14 b. Wait t remve the lck and tags until the system is ready t be put int peratin again. c. Ntify affected wrkers that the system is in peratin again. Reference is made t the Safe Stp f Machines - "Lckut and tagut". Side 14 (14)
