This dcument has been develped t ensure the safety f persnnel required t enter and cnduct wrk in cnfined spaces. The prgram cntained herein describes reasnable and necessary plicies and prcedures fr any and all facilities, departments and individuals wh are assciated with cnfined space entry peratins. II. SCPE This Prgram shall cver all KSU Cllege f Agriculture emplyees wh enter and wrk in cnfined spaces during wrk activities and thse wh anticipate wrking in cnfined spaces during an emergency incident. This prgram and all parts f 29 CFR shall apply t all cnfined space entry peratins cnducted at KSU Cllege f Agriculture emplyees. As it is the plicy f Kansas State University t prvide its emplyees with the safest wrk envirnment pssible, the University requires cmpliance with the prcedures set frth in this manual. A site-specific prgram may be used, prviding it meets r exceeds the requirements set frth in this plicy. III. CNSEQSUENCES F DEVIATIN The Cnfined Space prcedure serves as an essential element in identifying and managing risk t faculty, staff and students. Ignring this prcedure culd result in serius injuries r fatalities. IV. CNTENTS This dcument has been rganized int fur sectins: 1. Identifying Cnfined Spaces-- Department Heads r their designated representatives shuld determine if any persnnel under their supervisin are required t enter r cnduct wrk in cnfined spaces as defined in this sectin. 2. Identifying Cnfined Space Hazards-- This sectin gives infrmatin n the types f hazards that may be present in a cnfined space. It shuld be reviewed whenever the hazards f a cnfined space are being evaluated. 3. Cnducting a Cnfined Space Entry -- If it is determined that department persnnel are required t perfrm duties in cnfined spaces, the prgram utlined in The Permit System shuld be implemented. 2 4. Respnsibilities and Training Requirements-- This sectin lists the respnsibilities and training requirements f each individual invlved in a cnfined space entry. V. PART 1: IDENTIFYING CNFINED SPACES Recgnitin is an imprtant aspect f making a safe entry int a cnfined space. All cnfined spaces lcated within a facility r under the facility's cntrl shuld be identified. nce the space has been identified as Cnfined, the KSU EH&S Department shall determine if a permit is required. All emplyees shall be made aware f these cnfined spaces thrugh training r instructin prvided by their supervisr r designated representative. Assistance in this training shall be prvided by EHS. All emplyees shall be instructed by their supervisr r designated representative that entry int a cnfined space is prhibited withut an authrized permit. T clarify what cnstitutes a Cnfined Space, the fllwing definitin will be used. A cnfined space is any space that has the fllwing characteristics: 1. It is large enugh r s cnfigured that an emplyee can bdily enter and perfrm assigned wrk. 2. It has limited r restricted means fr entry r exit. Cnfined-space penings are limited primarily by size and lcatin. penings may be small in size and may be difficult t mve thrugh easily. Hwever, in sme cases penings may be very large; fr example, pen-tpped spaces such as pits r excavatins. Entrance and exit may be required frm tp, bttm, r side. In sme cases, having t access the wrk area by a fixed ladder may cnstitute limited r restricted entry r exit. Size r lcatin will generally make rescue effrts difficult. 3. It is nt designed fr cntinuus emplyee ccupancy. Mst cnfined spaces are nt designed fr emplyees t enter and wrk n a rutine basis. They may be designed t stre a prduct, enclse materials and prcesses, r transprt prducts r substances. Because they are nt designed fr cntinuus ccupancy, frequently they will nt have gd ventilatin r lighting. Therefre, ccasinal emplyee entry fr inspectin, maintenance, repair, cleanup, r similar tasks, can be difficult and dangerus. The danger assciated with entry may cme frm chemical r physical hazards within the space. Nt all cnfined spaces will be cnsidered permit-required cnfined spaces, and being able t identify the difference between the tw is imprtant. A Nn-Permit Cnfined Space is a cnfined space that des nt cntain, nr has the ptential t cntain, any hazard capable f causing death r serius physical harm. Examples f nn-permit required cnfined spaces might include the interirs f HVAC units, certain air plenums and pipe chases, attics, walk-in freezers r refrigeratrs, and sme building crawl spaces. A Permit-Required Cnfined Space is a cnfined space that is ptentially hazardus. A permit-required cnfined space has ne r mre f the fllwing characteristics: 1. Cntains r has a ptential t cntain a hazardus atmsphere, such as sewage and manure pits r in sme instances in upright sils. Page 2 f 15

3 2. Cntains a material that has the ptential fr engulfing an entrant, such as pen trenches r grain bins. 3. Has an internal cnfiguratin such that an entrant culd be trapped r asphyxiated by inwardlycnverging walls r by a flr that slpes dwnward and tapers t a smaller crss-sectin, such as in sme grain bins; r 4. Cntains any ther recgnized serius safety r health hazard. Examples f serius safety r health hazards might include: Fall hazards Unguarded machinery Extreme heat r cld Steam pipes r chemical lines Hazardus nise levels Electrical hazards Presence f asbests Ptentially hazardus levels f dust (such as might ccur at the Feed Mill, seed cleaning r grain handling facility) Because f the lack f ventilatin in mst cnfined spaces, they will have the ptential fr a hazardus atmsphere. Therefre, they must be designated "permit-required," and the prcedures fr making entry int a permit-required space must be fllwed. Examples f permit-required cnfined spaces at KSU include sewers, electrical vaults, steam tunnels, sump pits, certain mechanical rms, sme excavatins, and ther types f enclsures. Any space that is accessed by lifting a manhle cver shall be cnsidered a permit-required cnfined space. Additinally, certain grain strage facilities, and equipment access areas may be designated permit-required cnfined spaces even thugh they dn't technically meet the definitin (i.e., they may nt really have limited r restricted means f entry r exit). These areas shall be clearly marked as permit-required spaces. KSU has tw blanket designatins cncerning permit-required cnfined spaces: Steam tunnels, regardless f access, shall be cnsidered permit-required cnfined spaces (i.e., as sn as yu step int a steam tunnel, yu are in a permit-required cnfined space, even if yu walked int it thrugh a mechanical rm). Attics are nt cnsidered t be permit-required cnfined spaces. Supervisrs are directly respnsible fr ensuring the safety f their emplyees in regards t cnfined spaces. It is their respnsibility t evaluate ptentially hazardus spaces within their facilities and areas t ensure that the prper precautins are taken fr safety. This includes clearly marking permit-required cnfined spaces, training emplyees, and ensuring prper entry prcedures are fllwed. These respnsibilities may be delegated t anther cmpetent persn prvided he/she is qualified. Facilities supervisrs are respnsible fr ensuring their emplyees are prperly trained t d the jbs they are sent t d. This includes recgnitin f cnfined spaces and prper prcedures fr making entry int permitrequired cnfined spaces whenever necessary. N Physical Plant emplyee shall be sent n a jb that ptentially invlves wrk in a cnfined space unless they have been prperly trained in cnfined space entry prcedures. Page 3 f 15

4 It may be determined that a cnfined space presents n real danger fr emplyees. Hwever, it is recmmended that all spaces be cnsidered ptentially dangerus until they have been evaluated and tested. nce a space has been evaluated, the Envirnmental Health and Safety ffice shall determine if the cnfined space requires a permit and will apply apprpriate labeling. VI. PART 2: IDENTIFYING CNFINED SPACE HAZARDS nce a space has been identified as cnfined, the hazards that may be present within the cnfined space must be identified. Cnfined-space hazards can be gruped int the fllwing categries: xygen-deficient atmspheres Flammable atmspheres Txic atmspheres Mechanical and physical hazards Every cnfined space must be evaluated fr these fur types f hazards. The three types f atmspheric hazards are ften the mst difficult t identify since they might nt be detected withut the assistance f a gas mnitr. xygen-deficient Atmspheres The nrmal atmsphere is cmpsed f apprximately 21% xygen and 79% nitrgen. An atmsphere cntaining less than 19.5% xygen shall be cnsidered xygen-deficient. The xygen level inside a cnfined space may be decreased as the result f either cnsumptin r displacement. There are a number f prcesses that cnsume xygen in a cnfined space. xygen is cnsumed during cmbustin f flammable materials, as in welding, cutting, r brazing. A mre subtle cnsumptin f xygen ccurs during bacterial actin, as in the fermentatin prcess. xygen can als be cnsumed during chemical reactins such as in the frmatin f rust n the expsed surfaces f a cnfined space. The number f peple wrking in a cnfined space and the amunt f physical activity can als influence xygen cnsumptin. xygen levels can als be reduced as the result f xygen displacement by ther gases. Flammable Atmspheres Flammable atmspheres are generally the result f flammable gases, vaprs, dust mixed in certain cncentratins with air, r an xygen-enriched atmsphere. xygen-enriched atmspheres are thse atmspheres that cntain an xygen cncentratin greater than 22%. An xygen- enriched atmsphere will cause flammable materials such as clthing and hair t burn vilently when ignited. Cmbustible gases r vaprs can accumulate within a cnfined space when there is inadequate ventilatin. Gases that are heavier than air will accumulate in the lwer levels f a cnfined space. Therefre, it is especially imprtant that atmspheric tests be cnducted near the bttm f all cnfined spaces. The wrk being cnducted in a cnfined space can generate a flammable atmsphere. Wrk such as spray painting, cating, r the use f flammable slvents fr cleaning can result in the frmatin f an explsive Page 4 f 15

5 atmsphere. Welding r cutting with xyacetylene equipment can als be the cause f an explsin in a cnfined space and shall nt be allwed withut a ht wrk permit. xygen and acetylene hses may have small leaks in them that culd generate an explsive atmsphere and, therefre, shuld be remved when nt in use. The atmsphere shall be tested cntinuusly while any ht wrk is being cnducted within the cnfined space. Txic atmspheres may be present within a cnfined space as the result f ne r mre f the fllwing: The Prduct Stred in the Cnfined Space When a prduct is stred in a cnfined space, the prduct can be absrbed by the walls and give ff txic vaprs when remved r when cleaning the residual material. The prduct can als prduce txic vaprs that will remain in the atmsphere due t pr ventilatin. The Wrk Being Cnducted in the Cnfined Space Txic atmspheres can be generated as the result f wrk being cnducted inside the cnfined space. Examples f such wrk include: Welding r brazing with metals capable f prducing txic vaprs, painting, scraping, sanding, etc. Many f the slvents used fr cleaning and/r degreasing prduce highly txic vaprs. Areas Adjacent t the Cnfined Space Txic fumes prduced by prcesses near the cnfined space may enter and accumulate in the cnfined space. Fr example, if the cnfined space is lwer than the adjacent area and the txic fume is heavier than air, the txic fume may "settle" int the cnfined space. Mechanical and Physical Hazards Prblems such as rtating r mving mechanical parts r energy surces can create hazards within a cnfined space. All rtating r mving equipment such as pumps, prcess lines, electrical surces, etc., within a cnfined space must be identified. Physical factrs such as heat, cld, nise, vibratin, and fatigue can cntribute t accidents. These factrs must be evaluated fr all cnfined spaces. Excavatins culd present the pssibility f engulfment. Emplyees shall be prtected frm cave-ins by slping, benching, r shring systems when the depth f the excavatin is mre than fur feet, in accrdance with 29 CFR In sme circumstances, air-mnitring may als be required. VII. PART 3: CNDUCTING A CNFINED SPACE ENTRY When a cnfined space must be entered, a permit shall be cmpleted and authrized by department heads, supervisrs, r their designated representatives prir t entry f the cnfined space. This permit shall serve as certificatin that the space is safe fr entry. The permit shall cntain the date, the lcatin f the space, and the signature f the persn prviding the certificatin. 6 A permit shall nt be authrized until all cnditins f the permit have been met. Supervisrs r their designated representatives shall instruct all emplyees t list their names n the authrized permit befre they will be allwed t enter a cnfined space. The permit t be used by Kansas State University persnnel can be fund in the Appendix f this manual. A. Plan the Entry The first step twards cnducting a safe cnfined-space entry is t plan the entry. This will allw fr the identificatin f all hazards, and fr the determinatin f all equipment necessary t cmplete the prject. 1. Gather general data: Identify the cnfined space. Give the name r lcatin f the cnfined space. Give the reasn fr entering the cnfined space. Be specific. Als, identify if ht wrk will be dne. Identify the cntents f the cnfined space. This refers t any chemicals r ther materials and energy that are usually present in the cnfined space. 2. Identify the Hazards: NTE: Atmspheric testing shall be cnducted prir t entering permit-required cnfined spaces. It is recmmended that the entry supervisr cnduct these tests; hwever, any cmpetent persn certified in cnfined space entry may d s. The entry supervisr will determine the xygen cntent and recrd this n the entry permit. The entry supervisr will determine flammable gas cntent and recrd this n the entry permit. The entry supervisr will determine levels f H2S and Carbn Mnxide and recrd this n the entry permit. If a txic substance is determined t be in the cnfined space during testing by the entry supervisr, Envirnmental Health & Safety shall be cntacted t assist in btaining a Safety Data Sheet (SDS) r ther chemical infrmatin t determine what type f persnal prtective equipment is required, the ptential health effects, the Permissible Expsure Limits, and any ther infrmatin needed t safely cnduct the wrk. Entry supervisrs will determine mechanical and physical hazards. They shuld list all items and energy that will require lckut/tagut, blanking and bleeding, discnnecting, r securing. Physical hazards shuld als be listed. 3. Ventilate the Cnfined Space: Indicate whether mechanical r natural ventilatin will be used. Describe the prcedures t be used. NTE: If mechanical ventilatin is t be used, the exhaust must be pinted away frm persnnel r ignitin surces. Als, mechanical ventilatrs shuld be bnded t the cnfined space. 4. Islate the Cnfined Space: Describe the prcedures fr discnnecting equipment r lckut and tagut. All mechanical, electrical, r heat-prducing equipment shuld be discnnected r lcked and tagged ut. This wuld als Page 6 f 15

7 include any pumps that pull fluid frm, r pump fluid int, the cnfined space. 5. Purge/Clean the Cnfined Space: Indicate if the cnfined space will be purged. Purging with inert gas is nt recmmended. If the space must be purged, describe the prcedures. Indicate the type f cleaning methds t be used. If chemical cleaners are t be used, name the type and describe the prcedures. The SDS fr the chemical shuld be cnsulted prir t use. NTE: When intrducing a chemical int a cnfined space, the cmpatibility f that chemical with the cntents f the cnfined space must be checked. If in dubt, cnsult EH&S. NTE: If steam is t be used, the hse shuld be bnded t the cnfined space. 6. Place Warning Signs: Indicate if warning signs r barriers will be needed t prevent unauthrized entry r t prtect wrkers frm external hazards. If the cnfined space will be left pen and unattended fr any length f time, warning signs, and barriers such as barricades and/r cautin tape will be required. 7. Identify All Persnnel: List all emplyees that will be required t prepare the cnfined space and cmplete the wrk inside the space. 8. Identify Necessary Equipment: List all equipment that will be necessary t cmplete the prject. Where practical, all persnnel entering a cnfined space shuld be equipped with a retrieval line secured at ne end t the entrant by a full-bdy harness with its ther end secured t a tripd lifting hist. B. Cnduct Pre-Entry Training nce the entry has been planned, supervisrs r their designated representatives must train all emplyees wh will be invlved in the entry. The training shuld be cnducted n earlier than ne day befre entry is t be made fllwing the prcedure utlined belw. 1. Identify the cnfined space, the reasn(s) fr entry, and the wrk detail: Assign each emplyee the jb(s) he/she is t perfrm in the entry prject (entrant, standby persn, etc.). If an emplyee is required t use a piece f equipment, be sure that he/she is capable f using the equipment prperly. Infrm all persnnel that n ne is t enter the cnfined space unless the attendant is present at the wrk site. 2. Infrm entrants f all knwn and/r suspected hazards: Infrm persnnel f any access r exit prblems. Infrm persnnel f all equipment that must be lcked ut r tagged ut. Infrm persnnel f the cntents f the cnfined space. Infrm persnnel f all atmspheric levels that must be maintained befre entering and while wrking in the cnfined space. Page 7 f 15

8 If a txic atmsphere r substance is present r culd becme present, the fllwing additinal training must be cmpleted: If respiratry prtectin is nt ging t be used, infrm persnnel f the maximum permissible expsure level (PEL) that can exist within the cnfined space, and the methd used t mnitr PEL. Infrm persnnel f the ptential health effects f expsure t the txic atmsphere r substance. Infrm persnnel f the signs and symptms f expsure t the txic fume. Infrm persnnel f the persnal prtective equipment (PPE) that they will be required t wear. If entrants are unaware f the prper use f the PPE, they must be trained in the prper use f this equipment. NTE: Supervisrs may request assistance frm EH&S in prviding the abve-mentined training. Persns shuld nt be assigned t tasks requiring use f respiratrs unless it has been determined that they are physically able t perfrm the wrk and use the equipment. A lcal physician shall determine what health and physical cnditins are pertinent. The respiratr user's medical status shuld be reviewed peridically (annually). 3. Identify Islatin Prcedures: Infrm the persnnel respnsible fr the lckut/tagut f all equipment that must be islated. Infrm the persnnel respnsible fr perfrming this functin f the methds t be used. 4. Identify Purging and/r Ventilatin Prcedures: Infrm all persnnel respnsible fr perfrming this functin f the methds t be used. 5. Identify All Equipment Needed: Infrm persnnel invlved in the prject f all equipment that will be necessary t cmplete the prject. Make sure that all emplyees are capable f using their assigned equipment prperly. 6. Determine Necessary Persnal Prtective Equipment: Infrm persnnel f all PPE that must be used t ensure their safety. Make sure that all persnnel required t use PPE are trained in the prper use f the equipment. 7. Establish Cmmunicatin: Infrm all entrants that they are required t maintain cmmunicatin with the attendant. Infrm attendant that he/she must maintain cnstant cntact with all entrants. Infrm persnnel f the type f cmmunicatin they are t use. 8. Prtect frm External Hazards: Infrm persnnel where signs and barriers will be placed t prevent unauthrized entry and prtect entrants frm external hazards. 9. Pre-Plan Rescue Prcedures: The designated attendant(s) shuld be infrmed f the rescue prcedures t be fllwed. Page 8 f 15

9 The attendant shuld be infrmed that he/she can have n ther duty but t maintain cntact with persnnel inside the cnfined space. Infrm the attendant(s) that they must nt enter the cnfined space under any circumstances. 10. Placing the Cnfined Space Back int Service: Infrm persnnel f the steps t be taken t place the cnfined space back int service. C. Preparing the Cnfined Space fr Entry nce the entry has been planned and persnnel have been trained, the next step is t prepare the cnfined space fr entry. The fllwing steps are t be fllwed when preparing the cnfined space fr entry: 1. Place warning signs r barriers arund the cnfined space t prevent unauthrized entry as necessary. 2. Place all tls, safety equipment, mnitring equipment, etc., near the cnfined space. 3. Islate all mechanical and/r electrical hazards as necessary. 4. Purge/ventilate the cnfined space as necessary. 5. Test the atmsphere using an apprpriate gas mnitr. If xygen cntent is less than 19.5% r greater than 21.5%, perfrm additinal ventilatin. Then shut ff ventilatin equipment and re-test the xygen cntent. If xygen cntent is between 19.5% and 21.5%, cntinue entry preparatin. 6. Test fr flammable gases. If the meter reading is less than 10% f the lwer explsive limit (LEL), cntinue entry preparatins. If the meter reading is abve 10% f the LEL, cntinue ventilatin f the cnfined space. Then shut ff the ventilatin and have the atmsphere re- tested. If the meter reading is still abve 10% f the LEL, the cnfined space must be cleaned befre entry is permitted. If the cnfined space must be entered fr cleaning purpses, the prcedures utlined in Item 9 f this sectin must be fllwed. 7. Test fr txics (If a txic atmsphere is present, n persn shuld be permitted t enter the cnfined space at a level exceeding the Permissible Expsure Limit withut prper Persnal Prtective Equipment. The Departmental Safety Crdinatr shuld be called t assist in identifying prper precautins and the prtective measures t be taken. 8. Assemble all persnnel invlved and review rescue prcedures. The entry supervisr will then add any needed infrmatin, then cmplete and sign the permit. Page 9 f 15

10 9. Ntify Department Head r supervisr that entry is cmmencing. If Department Head r supervisr is unavailable, ntify the Departmental Safety Crdinatr. VIII. PART 4. PERSNNEL RESPNSIBILITIES AND TRAINING Everyne invlved in a cnfined-space entry prject has certain respnsibilities and requires a certain amunt f training. It is very imprtant that every individual is familiar with his/her respnsibilities. This sectin utlines the respnsibilities and training requirements f each individual invlved in a prject. The EH&S Crdinatr r his/her designated representative shall be respnsible fr the fllwing: 1. Reviewing and updating the Kansas State University Cnfined Space Entry Prgram t cnfrm t current CFR standards. 2. Verifying cmpliance with standards set frth in the prgram by peridic inspectin f entry sites and canceling permits where unsafe cnditins are present. 3. Assisting Supervisrs with: Prviding training as set frth in the prgram, Identificatin f cnfined spaces, Identifying spaces that require a permit fr entry and Labeling Permit-Required Cnfined Spaces. 4. Perfrming a single annual review cvering all entries perfrmed during a 12-mnth perid t ensure emplyees participating in entry peratins are prtected frm permit space hazards. Supervisrs r Their Designated Representatives are Respnsible fr: 1. Identifying cnfined spaces within facilities r areas under their cntrl. 2. Identifying hazards within a cnfined space under their cntrl. 3. Dcumenting that all training requirements fr a specific cnfined space entry have been met by signing the pre- entry authrizatin space n the entry permit. Entry Supervisrs shall be respnsible fr the fllwing: 1. Ensuring that the required atmspheric tests are perfrmed at the cnfined space and results recrded n the permit prir t entry authrizatin. 2. btaining and maintaining all equipment necessary t cmplete the cnfined-space entry prject. 3. Authrizing entry by signing the Entry Authrizatin space n the entry permit after all cnditins fr a safe entry have been met. 4. Terminating the entry and canceling the permit when: Entry peratins cvered by the entry permit have been cmpleted. Page 10 f 15

11 A cnditin that is nt allwed under the entry permit arises in r near the permit space. 5. Determining, whenever respnsibility fr a permit space entry peratin is transferred, and at intervals dictated by the hazards and peratins perfrmed within the space, that entry peratins remain cnsistent with terms f the entry permit and that acceptable entry cnditins are maintained. Authrized Entrants are Respnsible fr and shall receive training in the Fllwing: 1. The knwledge f hazards that may be faced during entry, including the mde, signs r symptms, and cnsequences f the expsure. 2. Prper use f equipment, which includes: Atmspheric testing and mnitring equipment. Ventilating equipment needed t btain acceptable entry cnditins. Cmmunicatin equipment necessary t maintain cntact with the attendant. Persnal prtective equipment as needed. Lighting equipment as needed. Barriers and shields as needed. Equipment, such as ladders, needed fr safe ingress and egress. Rescue and emergency equipment as needed. Any ther equipment necessary fr safe entry int and rescue frm permit spaces. 3. Cmmunicatin with the attendant as necessary t enable the attendant t mnitr entrant status and t enable the attendant t alert entrants f the need t evacuate the space if required. 4. Alert the attendant (standby persn) whenever: The entrant recgnizes any warning sign r symptm f expsure t a dangerus situatin, r The entrant detects a prhibited cnditin. 5. Exiting the permit space as quickly as pssible whenever: An rder t evacuate has been given by the attendant r the entry supervisr; The entrant recgnizes any warning sign r symptm f expsure t a dangerus situatin; The entrant detects a prhibited cnditin; r An evacuatin alarm is activated. Persns authrized t perfrm duties as attendant shall be respnsible fr and receive training in the fllwing: 1. Knwing the hazards that may be faced during entry, including infrmatin n the mde, signs r symptms, and cnsequences f expsure. 2. Awareness f pssible behaviral effects f hazard expsure in authrized entrants. 3. Cntinuusly maintaining an accurate cunt f authrized entrants in the permit space and ensuring that the means used t identify authrized entrants accurately identifies wh is in the permit space. 12 4. Remains utside the permit space during entry peratins until relieved by anther attendant. 5. Attempting nn-entry rescue if prper equipment is in place and the rescue attempt will nt present further hazards t the entrant r attendant. 6.
