2 I. Scpe The University s creates plicies t be fllwed whenever an emplyee wrks at heights abve Six feet. These guidelines were established t prvide a safe wrking envirnment, and t gvern the use f fall prtectin measures and equipment. The written prcedures utline unifrm requirements designed t ensure that fall preventin training, peratin, and practices are cmmunicated t, and understd by, the affected emplyees (an affected emplyee is ne that must wrk in r arund areas where fall hazards are present). Therefre, nly trained emplyees may wrk in areas where fall hazards are present. II. Respnsibilities A. Risk Management and Safety shall; 1. Develp and maintain the written. Risk Management and Safety will re-evaluate this prgram peridically and will base the need fr changes upn suggestins by emplyees and their supervisrs, accidents and near miss incidents that have been recrded, rutine building safety inspectins and changes in standards and technlgy. 2. Prvide necessary training, which will primarily cnsist f elevated platfrm safety, scafflding safety, ladder safety and persnal fall arrest system safety fr thse affected emplyees. 3. Serve the rle f technical supprt and cnsultatin t departments f affected emplyees t interpret requirements and establish safe practices. 2
3 B. University departments shall; 1. Recgnize ptential fall hazards based n this plicy, ntifying Risk Management and safety f each fall hazard that their emplyees may face and t invlve their affected emplyees in this prgram. 2. Ntify Risk Management and Safety f the need fr apprpriate training, such as ladder safety, scafflding safety and persnal fall arrest system safety. 3. Evaluate, n an annual basis, the effectiveness f the prgram as it applies t the wrk that their affected emplyees perfrm and t prvide Risk Management and Safety with their cnclusins, cmpliance challenges and recmmendatins. 4. Cntact Risk Management and Safety fr technical supprt when questins arise regarding cmpliance and safe prcedures. 5. Ensure that prper safety equipment is supplied t their affected emplyees where needed, such as fall arrest systems, scafflding, prper ladders, guard railings, te kicks, etc. 6. Ensure that their affected emplyees perfrm applicable pre-wrk check lists and inspectins and t maintain the recrds f the cmpleted check lists fr at least ne year. 7. Ensure that all wrk places are safe t perfrm the wrk that their affected emplyees are expected t cnduct. T prevent slipping, tripping and falling, all lcatins where fall hazards are present must be kept clean, dry (where pssible) and rderly. Where wet prcesses are used, drainage will be maintained, and false flrs, platfrms, mats, r ther dry standing places are prvided where practicable. C. Affected Emplyees shall; 1. Fllw the prgram requirements utlined in this plicy and standard prcedures required by their department fr the wrk activities they are invlved with. 2. Ntify their supervisr when questins arise surrunding safe prcedures, the need fr fall preventin equipment, persnal prtective equipment and difficulties cmplying with requirements. 3. Attend any fall preventin training that is required f them. 3
4 4. Reprt all accidents and near misses that they witness r incur. This will help the University t imprve safe practices. 5. Perfrm checklist inspectins utlined in this written prgram prir t cnducting wrk activities. III. Fall Hazards There are a number f ptential situatins r cnditins that can present a fall hazard. This plicy may nt specifically address all pssibilities. Therefre, when emplyees r supervisrs recgnize a cnditin that may present a fall hazard nt specifically addressed by this written prgram r the University s Aerial Platfrm Lift Plicy, they shall invlve Risk Management and Safety prir t addressing the safety cncerns surrunding the cnditin. The fllwing list f cnditins addressed by this written prgram (nte that this is nt an all-inclusive list): A. Ladders, bth permanent and temprary (see Appendix A) Ladders shall exhibit the fllwing cnditins: 1. Meet OSHA specificatins fr design and safety. 2. The apprpriate type f ladder is being used fr the jb. 3. Metal ladders are nt used near expsed electrical surces. 4. All parts, rpes, fittings and cnnectins are secure and in gd cnditin. 5. Nn-slip surfaces are in place n ladder rungs. 6. Gripping safety feet are in place, secure and in sund cnditin. 7. Ladder has been set up safely: a) Flr/grund surface is firm. b) Flr/grund surface is flat. c) Flr/grund surface is nt slippery. d) Ladder is level. 4
5 e) Tp f ladder (unless using step ladder) is against a slid, fixed surface. f) Fr extensin ladders, the 4-t-1 principal is accmplished (the base f the ladder shuld be placed at a distance frm the wall that is equal t ne frth f the height that the ladder is extended. Ie; a ladder that is extended 20 ft high shuld have it s base apprximately 5 ft frm the wall). g) When emplyees are n extensin ladders at heights f 20 feet r higher, either a secnd persn is present t steady the ladders base r the tp f the ladder is effectively tied ff t a sund anchr pint. B. Stairs (permanent and temprary) All stairs shall exhibit the fllwing cnditins: 1. Meet OSHA specificatins fr design and safety. 2. All required cvers r guardrails are in place, including tp rails, midrails and te kicks r spindles. 3. All hand rails r guardrails are in place. 4. All treads and risers are in gd repair. 5. Nn-slip surfaces are in place. 6. Adequate headrm is maintained abve. 7. Stairs are clear f clutter and slippery materials. C. Elevated platfrms that are fixed All elevated platfrm lcatins shall exhibit the fllwing cnditins: 1. Tp guard rail in place, is between 36 and 45 inches frm flr, is in sund cnditin and anchred apprpriately. 2. Vertical rails (spindles), a slid surface r a mid rail is in place, is in sund cnditin and is anchred apprpriately. 3. If the railing is nt slid dwn t the flr, a te kick is present, in sund cnditin and anchred apprpriately. 4. The flring f the platfrm has n penings, is prperly attached t sund surface and is in sund cnditin. 5
6 5. Walking surfaces are clear frm bstructin and are nt slippery. D. Scafflding Once erected, scafflding is an elevated platfrm and shall meet the same safety requirements. Hwever, due t the cmplexity f parts and cnnectins, and due t the fact that they are rutinely assembled and disassembled, they are far mre cmplex and are ptentially mre dangerus. Therefre, all emplyees wh erect r use scafflding shall attend the Scaffld Safety Training Prgram spnsred by Risk Management and Safety prir t wrking with r n scafflding. See sectin VI f this written prgram fr specific requirements relating t scafflding. E. Flr and wall penings All flr and wall penings that lead t a fall hazard shall exhibit the fllwing cnditins: 1. All flr and wall penings are safely cvered r blcked frm access. 2. If nt safely cvered r blcked frm access, smene is assigned fr cnstant attendance t it. 3. Cvers shall be sund, slid and nt easily pened. 4. Barricades that are designed t prevent smene frm falling int the pening shall be visually nticeable, strng enugh t hld the weight f multiple peple and shall nt, in themselves, have additinal penings that create additinal fall hazards. If the barricade is nt slid frm it s tp rail t the flr, they shall meet the requirements f a guardrail n an elevated platfrm (tp rail, mid rail, te bard). 5. Flr surfaces surrunding the pening shall be clear f clutter r slippery material. F. Wrking withut a ladder r aerial platfrm lift at elevatins greater than 6 feet in height. Any time an emplyee wrks at elevatins higher than 6 feet abve the flr they shall use an apprpriate ladder, an aerial platfrm lift, scafflding r assemble an apprpriate elevated platfrm whenever feasible. If these are nt feasible, the emplyee shall be trained in the prper use f fall arrest systems and dn their arrest systems apprpriately 6
7 thrughut the duratin f time that they wrk abve 6 feet (see sectin V). Sme f the abve cnditins require the use f safety check lists prir t wrking with/n them. See Appendix A thrugh D fr apprved checklists. If any f the cnditins n the checklist are nt met fr the area r piece f equipment psing the ptential fall hazard, then emplyees may nt perfrm that wrk until the cnditin is safely met. If the cnditin cannt be remedied, a supervisr r Risk Management and Safety shall be ntified f the prblem. If the situatin calls fr the use f fall prtectin devices, such as harnesses, r psitining and restraining devices (such as belts) then the emplyee must dn such prtective equipment befre beginning the wrk and use it as intended thrughut the duratin f the wrk. If the emplyee has nt been trained in the safe use f fall arrest systems then he/she may nt use the equipment until they have been trained thrugh the University s Fall Arrest System training prgram spnsred by Risk Management and Safety (see sectin IV). IV. Training Prgrams The Risk Management and Safety Department will spnsr all fall preventin related training prgrams fr University emplyees wh require the need. The individual departments shall ntify Risk Management and Safety f the need fr training, which will include new hires and existing emplyees wh s jb has changed t require such training. All training recrds will be maintained by Risk Management and Safty. The fllwing training prgrams will be prvided as needed: A. Scaffld Safety: Fr all emplyees wh erect r use scafflding. B. Fall Arrest System Safety: Fr all emplyees wh use fall arrest systems. C. Ladder Safety training will take place peridically fr departments wh frequently use ladders. D. Nte that aerial platfrm lift training, thugh nt a part f this fall preventin written prgram, is als prvided and mandated fr all emplyees wh perate r ride n aerial platfrm lifts (see Aerial Platfrm Lift Plicy). 7
8 V. Specific requirements regarding persnal fall arrest systems Different types f persnal fall arrest systems are chsen depending n the nature f the wrk and the specific cnditins present in the area where the wrk will take place. A. Three basic types f persnal fall arrest systems include: 1. Persnal Fall Arrest system A system used t stp a fall nce it has begun. This system shall include an anchrage, full bdy harness, lanyard, lcking snaphks, lifeline and anchrage cnnectr, and it may include a descent cntrl device. 2. Psitining Device System A system that prevents falls by supprting the emplyee in a wrking psitin. This system supprts the emplyee, therefre, eliminating the chance fr a fall t begin. These systems may include a bdy belt, harness, cnnectr, lcking snap hk and prper anchrage. 3. Persnal Fall Prtectin Systems fr Climbing Activities A system that prtects the emplyee while he/she is climbing. This system anchrs at a pint that usually adjusts and mves with the climber. B. Equipment anchrage, tie ff and use Anchring yur fall arrest system is critical. The selectin f the anchring pint shuld be made carefully and when the emplyee is uncertain abut the anchring pint he/she is expected t cnsult with a supervisr r Risk Management and Safety. Anchring pints must be permanent, fixed bjects that are rated t hld frces several times the persn s weight, including the weight f the equipment they will have with them. There are ther criteria necessary fr an adequate anchrage pint that shall be cvered in training. When tying ff, the emplyee shall tie ff at such a lcatin where there are n bstacles in the ptential path f a fall and shall allw n mre than 6 feet f free-falling distance (hwever, 2-4 ft f free falling distance is recmmended). In additin t the free falling distance, the emplyee shall knw and accunt fr the additinal deceleratin distance f the fall arrest system they are using. The emplyee shall cmplete the anchring tie ff and equipment tie ff prcedures that are specified by the fall arrest system manufacturer PRIOR t getting int a psitin where he/she culd fall. 8
9 C. Bdy Belts and Harnesses Only full-bdy harnesses that are apprved by Risk Management and Safety may be used. All equipment manufacturer s prcedures shall be met. Harnesses can be attached either in the center f the back at shulder level r abve the head. The emplyee must use the fllwing prcedure t put their full-bdy harness n: 1. Inspect yur full-bdy harness befre putting it n (see part D f this sectin). 2. Hld the harness by the back D-ring and shake the straps int place. 3. Release buckled straps and slip them ver yur shulders with the D-ring in back. 4. Pull the leg strap between yur legs and cnnect it t the ppsite end. 5. Waist strap shuld be tight but nt binding. 6. Cnnect chest strap and psitin it in the middle f yur chest. 7. Check that the harness is snug but allws full mvement. D. Equipment Care and Inspectin Keeping equipment in gd wrking rder is essential. Fllw manufacturer s instructins and training prtcls fr equipment maintenance, cleaning and strage. Emplyees shall fllw the Persnal Fall Arrest System Check List in Appendix B f this written prgram prir t each use f their persnal fall arrest system. Equipment that has been subject t a fall must be inspected befre it is re-used. Nte that sme persnal fall arrest systems need t be discarded after a single fall ccurs. Any defective equipment must be identified and replaced at nce. E. Rescue After a Fall In the event that a persn falls and their fall arrest system engages, call 911. The Ntre Dame Fire Department will rganize a rescue effrt quickly. 9
10 VI. Specific requirements regarding scafflding Scafflds are cmplex systems with multiple cnnectin pints and are subject t a number f factrs that culd effect their stability and reliability. Therefre, nly a cmpetent persn (ne wh has received training spnsred by Risk Management and Safety) may erect and disassemble scafflding, perfrm a scaffld equipment inspectin (see Appendix C) and perfrm a scaffld wrkplace inspectin (see Appendix D). A. Basic requirements fr the safe use f standard scaffld systems 1. The wrking edge f the scaffld can be n mre than 14 inches frm the frnt f the building r structure withut additinal pre-cautins (see Risk Management and Safety if yu need mre than 14 inches). 2. Platfrms shall extend ver the end supprts by at least 6 inches and nt mre than 12 inches unless cleated r restrained by hks, nails r nine gauge wire. 3. All parts that are supplied by the manufacturer are t be used unless they are parts specifically designed fr ptinal uses which are nt being used during the prject. 4. All parts, including casters, pipes/ples, rails, te bards, platfrms, cams, lcking pins and all cnnectin devices must be inspected and fund t be in gd cnditin prir t each use (See Appendix C). 5. A wrk place inspectin shall be cnducted prir t and during the erectin f the scafflding (See Appendix D). 6. Guardrails shall be placed between 36 and 45 inches high and placed at the pen ends and sides f the platfrm and must be able t withstand a frce f 200 punds. Mid rails are needed between the tp rail and the te bard. Te bards shall als be in place. 7. Crss bracing and railing shall nt be used as a means f climbing t and accessing the platfrm. Use nly installed ladders. 8. Caster wheels shall be lcked and all lcking pins shall be in place prir t using scafflding. 9. Fall prtectin systems are required, when feasible, while emplyees erect and disassemble scafflding. 10. Hard hats shall be wrn when wrking n r arund scafflding (see Risk Management and Safety regarding exceptins t this requirement). 10
11 11. The area belw and arund erected scafflding shall have a barricade. 12. If planning t use a suspended scafflding system, cnsult with Risk Management and Safety first. 11

The wrkplace shuld be inspected at each new lcatin where a ladder is t be used. These are guidelines, therefre dcumentatin is nt a requirement. 15 Appendix D Fall Arrest System Checklist The fllwing checklist shuld be cmpleted prir t cnducting wrk using a fall arrest system. This is a guideline checklist that is required, hwever dcumentatin is nt mandatry. In additin t these items, be sure t inspect any ther items that the fall arrest system s manufacturer recmmends.
