E25 FATIGUE MANAGEMENT PURPOSE AND SCOPE This Prcedure prvides the requirements and guidance needed t manage fatigue fr Laing O Rurke railway peratins. It will need t be read in cnjunctin with the rail infrastructure managers fatigue risk management prgramme where apprpriate r it may be necessary t apply the RIM s fatigue management prgram in its entirety. Fatigue management applies t all rail safety wrkers. It is cnsidered that ffice based wrkers wh wrk rutine day wrk n weekdays nly will have a lw level f risk and n particular actins will be required. Their fatigue risk management will be aligned t Primary Standard Fitness fr Wrk Greater fcus is applied t situatins where higher fatigue may result. These include: Shift wrk, where wrkers are rstered t wrk during any perid that peple nrmally sleep; Lng perids f wrk, where emplyees may be required t wrk ver weekends as well as week days Perids f wrk in envirnments where the elements impact n a persn s fatigue levels PROCEDURES Laing O Rurke has a Rail Fatigue Management Plicy that utlines Laing O Rurke s cmmitment t managing fatigue as part f managing rail safety wrker fitness fr wrk. Implementatin f Fatigue Management will be verseen by the relevant Prject Leader with mnitring data included in mnthly reprts. Fatigue Risk Management Prgramme The fllwing requirements are thse determined by Laing O Rurke fr the railway peratins t be undertaken and must be applied unless a client s criteria r that f the prject itself especially thse in remte lcatins requires a variatin n wrking hurs, shifts, rsters breaks r travel times. Prjects need t cnduct a risk assessment fr their railway peratins t determine whether any exceptin t the shift patterns belw can be applied. Where the railway peratins are undertaken under the accreditatin f the RIM, apprval must be btained frm the RIM fr the revised Fatigue Risk Management Prgram t apply. Laing O Rurke shall apply the fllwing fatigue minimisatin cntrls t Laing O Rurke wrkers r thse f subcntractr / service prvider rganisatins wh may in the executin f their wrk be deemed t be rail safety wrkers in terms f the Rail Safety Natinal Law:
N mre than 12 hurs will be wrked at a time including travel except fr any unplanned event where wrk is required t exceed the 12 hur limit. This can als cver shutdwn wrk fr which the RIM requires 12 hur shifts at site and then travel abve that limit. An extensin f time can be authrised by the supervisr t a maximum f 14 hurs r the Prject Leader where an extensin t 16 hurs may be required. Shuld any extensin f the 12 hur rule be required a risk assessment must be undertaken and authrised by the respective persn prir t the extended perid being cmmenced In any extended perid the wrk perfrmed up t the maximum f 16 hurs at a time, must ensure wrkers are nt required t drive a mtr vehicle r perate heavy plant r equipment between the 13th and 16th hur Rest perids must ensure at least a 10 hur break between shifts Minimum break times per shift shall include a 30 minute meal break and tw 15 minute rest breaks in the perid f wrk befre and after the main meal break Maximum number f wrk days nt t exceed 12 wrk days in 14 cnsecutive days A reductin in night shifts must be applied with: 6 cnsecutive ccasins where 8 hurs are wrked at night (i.e. after 6pm) r 5 cnsecutive ccasins where 10 hurs are wrked at night r 4 cnsecutive ccasins where 12 hurs are wrked at night Ensure wrkers receive a minimum f 48 cnsecutive hurs free f wrk in a 14-day perid At least 24 hurs ff fllwing cmpletin f any blck f scheduled night shifts Have the capacity t replace r relieve wrkers where unplanned r unavidable extended hurs have created a risk t wrkers health and safety Fatigue Related Risks In accrdance with fatigue management prcesses it is necessary t undertake a risk analysis f the railway peratins t be undertaken which will include scrutinising the fatigue issues fr each task which includes the fllwing: The Envirnment: The Task The persn T assist in the fatigue risk management prcess use shuld be made f the (UK) HSE Fatigue Risk Calculatr (linked t this prcedure) which requires infrmatin t be inserted in the spreadsheet relating t: number f days, perid n duty, perid ff duty, jb type, breaks, cmmuting time, duty length, rest length and average duty per day which then relates t a fatigue risk scre being prvided. The Mean Daily Scre that results frm the data put in, is an indicatr nly and shuld be supprted by ther cntrls t assist in managing fatigue.
Risk Factrs fr Fatigue Risk factrs can be identified thrugh the fllwing: Findings frm incident investigatins Discussins with emplyees Emplyee surveys r questinnaires Walk thrugh inspectins Audit results Symptms f Fatigue The mst cmmn symptms assciated with fatigue are: Increased sleepiness Lack f cncentratin Temprary memry lss Slwed reactin times Irritability Headaches and general bdy aches Md swings Reduced physical strength and capabilities Reduced hand-eye crdinatin Pr judgement Effects t general health and well-being such as lss f appetite and weight Impacts f Fatigue Fatigue creates significant risks if engaged in hazardus tasks r changes in perfrmance which include: Lss f attentin Feeling apathetic and lethargic (lw energy) Inability t anticipate danger Shrt term memry prblems Pr decisin making Increased reactin time Lwer vigilance and alertness levels Reduced ability t slve prblems Impatience and increase risk taking Shrt unplanned naps (micr sleeps)
Pr perfrmance Self-Reprting During the peratin f the prject, steps will be put in place fr self-reprting f fatigue fr individuals wh may be affected, t accmmdate irregular r un-rstered wrking. This als includes wrk perfrmed by subcntractrs and fr vertime and extensin f shifts. With its 'just culture', Laing O Rurke encurages wrkers wh perfrm rail safety wrk t reprt if they are unfit t start r cntinue wrk as a result f suffering the effects f fatigue. Laing O Rurke will implement nn-punitive measures t address the situatin. Irregular r Un-Rstered Shift Wrk (including subcntractrs) Where it is nt pssible t rster wrk t predictable start and finish times and/r, where emplyees are rstered n-call, an alternative methd f assessment f fatigue shall be available. In rder t determine whether an emplyee wrking under these cnditins is fit t cmmence wrk, the fllwing guide shall apply: An emplyee can be cnsidered fit-fr-wrk if they have btained a minimum f 6 hurs sleep in the 24 hurs prir t cmmencing wrk. A minimum rest perid f 10 hurs shall apply between shifts. Overtime and Extensin f Shifts When unplanned situatins require vertime r extensin f shifts, the fllwing steps are required t select wrkers t extend time at wrk r prvide with vertime shifts n the basis f: Ability t wrk within rstering limits prvided in the rstering principles Recent rest perids, with pririty t thse with mst recent break f shifts Impact n future rstering. If wrkers are required t wrk vertime r extensin f shifts, an assessment f the wrkers fitness t wrk (E-C-8-1925 Fatigue Risk Assessment) will be carried ut by a Laing O Rurke representative wh has received fatigue management training prir t the extensin f wrk cmmencing. If a wrker is cnsidered t be at risk f fatigue then the fllwing risk mitigatins must be implemented: Wrk t be shared by mre than ne emplyee with the emplyees required t be vigilant fr symptms f fatigue Emplyee t be supervised Re-arrange wrk s that the fatigued emplyee is nt undertaking rail safety wrk. If nne f these steps can be put in place, unless it is safe t d s, the emplyee s shift must nt be extended r wrked as an vertime shift. Further arrangements may need t be put in place when emplyees wrk vertime r extended shifts, including:
Arrangements t get an at risk f fatigue emplyee safely hme Adjustment t future rsters s that fatigue is nt increased and adequate rest breaks is prvided. All such cases must be reprted t the Prject Leader with these reprts included in the mnthly safety perfrmance reprt. Mnitr Results Prject Leaders will arrange t have actual wrking hurs cmpared t rstered hurs n submissin f the emplyee's timesheet t ensure that rsters and fatigue levels are being managed. Where rsters are rutinely nt met, rsters will be reviewed and amended. Prject Leaders will review all cases where wrkers have reprted they were unfit t cmmence r cntinue Rail Safety wrk due t fatigue. Incident investigatin will be cnducted int incidents reprted under RSMS E21 Reprting Ntifiable Occurrences t determine whether fatigue culd have been a cntributing factr. Investigatin reprts that include fatigue as a factr in the incident will require a respnse frm the Prject Leader. Reprting Internally / Externally n Fatigue Related Issues The fatigue risk management prgram determined in terms f Laing O Rurke s accreditatin shall be applied in all instances at the respective prject. Shuld a breach f the fatigue risk management prgram ccur the Prject Leader r designated representative must advise the Rail Safety & Cmpliance Manager immediately as under the Rail Safety Natinal Law it is deemed t be a Ntifiable Occurrence. The Rail Safety & Cmpliance Manager must ldge the Ntifiable Occurrence with the Rail Regulatr within 72 hurs f becming aware that such a breach f the fatigue risk management prgram has ccurred. The Rail Safety & Cmpliance Manager shall include the nn-cnfrmance in the mnthly rail safety reprt submitted t the Hub Safety Bard.
