Please visit: dixonsdanceandfitness.com for a full list of the Policies and procedures 1. Monthly payments are due the 20 th of each month. Pay period starting August 20 th 2018 and ending May 20 th 2018. This applies to The Signature Competitive dancers only. 2. There is a $25 + HST registration fee payable on the initial registration in each school year. 3. There are NO refunds except in the case of cancellation due to insufficient registration or with a Dr. s Certificate. 4. ALL classes with the exception of the Creative Movement (3 years) require a full season commitment (Sept.-June). Therefore, you are responsible and required to fulfill this commitment financially. 5. This is NOT a Month-to-Month program, but a full year commitment. 6. Year End Performance: Friday, June 14 th and Saturday, June 15 th 2019 The Location will be at the Shallow Lake Community Church Perish. 7. There is a non-refundable $75 Performance and Costume fee- Due upon Registration. 8. There will be rehearsals that are Mandatory for ALL dancers participating in the Year End Performance to attend. The dress Rehearsal on Thursday, June 13th th 2019. 9. Studio Photography-Saturday May 25th, 2019 - All Dancers are to attend. Schedule and Location TBD. Individual & Group Packages will be available for purchase. These forms will be available and are to be filled out and handed in prior to May 25 th. 10. Classes cancelled throughout the year will try to be made up at during the season. This applies to cancelled classes due to the teacher s illness but does not apply to cancelled classes as a result of adverse weather conditions. 11. All registration forms must be signed and dated to commence classes. 12. Fees are due in full at the time of registration according to the payment policies.
Studio Copy 13. Family Discount 15% for the 2 nd child, 10% for 3 rd and 4 th etc. (before HST, must be mentioned at time of registration) for each additional child (discounts are applied to the lesser of the amounts, cannot be combined, cannot be applied to fitness classes). Second and third etc. class discounts may apply depending on the class as some already include discounted prices. 14. $45 +HST fee will be charged for all NSF cheques, all further cheques must be certified or cash only. 15. An additional $15+ HST, fee will be charged for late payments. 16. I require 21 days written notice if discontinuing from a program. Payments for the rest of the year will still be required.
