TIMBERVIEW MIDDLE SCHOOL ATHLETICS DEBBIE HOLT, ATHLETIC DIRECTOR KRISTEN SMITH, ATHLETIC SECRETARY 6, 7, & 8 INTRAMURAL SOCCER Timberview Middle School s boys and girls soccer program is committed to skill development, participation, team work, and good sportsmanship. The participation fee for soccer is $45. If this presents a hardship for you, please email Ms. Holt at Debbie.holt@asd20.org A valid current sports physical (within the past 12 months for the entirety of the sport season) MUST be on file before a player is allowed to practice or participate. Physical forms are available from the athletics office or on our website. A standardized form from your physician is acceptable. The signed registration consent form, fee, and physical should be turned in to the athletic secretary BEFORE NOON on the first day the student attends practice. Because of space, a limited number of students will be accepted into the soccer program. Students must submit a signed registration form and a current, valid physical to be accepted. An athlete who is injured, seen by a physician, and cannot practice for an extended period of time must provide a doctor s statement saying he/she is cleared to play all sports before he/she will be allowed to participate in any sport. Students who participate in soccer are expected to attend all practices and games and to be ON TIME for both. If a student must miss a practice or game, the coach must be notified in advance. You may leave a message for the coaches at 234-3621. Athletes may not ride home with friends without prior written permission from their parents. A replacement fee will be charged for all lost or damaged equipment/uniforms. Athletes will not be eligible to play another sport until all equipment and uniforms are returned and all applicable fees are paid. We offer INTRAMURAL soccer for students in grades 6-8 who wish to play, regardless of skill level. Our teams play one another during the soccer season, and there will be no interscholastic competitions or interscholastic teams. Coaches control the format and rule modifications in games. Practices will focus on skill development, tactical play, and technique. Groups may be skill-divided for specific practices. All players are assessed for skill level prior to the start of practices and games to make sure teams are evenly balanced with both new and seasoned players. Players may be assigned to field duty at least twice in the season where team members will serve in the positions of line referee, stat keepers, time keeper, or equipment manager. Athletes will attend games on a rotating schedule. Soccer is offered as an outdoor sport. Given Colorado s unpredictable weather, students should dress in layers and be prepared to play in anything from unseasonably cool temperatures to warm weather. In addition, players should come to practice and matches with shorts, a hat or ski band, gloves, their own water bottle, and lip balm. Athletes are divided into teams. Four teams will play on game days. If your athlete is not playing on a certain game day, he/she will have a bye. Students are excused from all game days if their team is not participating and if they are not signed up to help during that game.

All games will be played at TMS. Athletic Fee Refund Policy We understand circumstances may arise which make it necessary to withdraw. Refunds are only available during the first two weeks of practice. Athletes withdrawing during the first two weeks of practice will be given a refund ONLY after they have submitted a drop slip signed by a parent/guardian and coach. Drop slips are available in the athletics office. Athletes withdrawing after this time will not be issued a refund. Athletes must provide the following: Workout clothes (layering is recommended) Shin guards required and must be of adequate size for the height and age of the player. Players who do not have shin guards will not be allowed to attend practice or games. Long socks Soccer cleats (NO CLEAT ON TOE) or athletic shoes Lock for gym locker Water bottle Timberview will provide the following: Game uniform Soccer balls

SPORTS CONSTITUTION AND PERMISSION TO PARTICIPATE I, the parent or legal guardian of the above named student, hereby give my consent for my child to participate in Timberview Middle School s sports program for either the duration of the sport or until my student chooses to quit the sport, and I hereby release the school district and hold it and its employees harmless against any liability for injuries my student may incur as a result of participating in the sport. I have read and understand the rules and regulations of the sports constitution (located on the Timberview athletics website). I understand that my child will be held to these standards and consequences, and I have clarified any questions that I may have with the coaches. I understand that this document can be changed at any time at the discretion of the coaches or administration, and all decisions made are final. I have read and agree to the SOCCER registration information attached to this document.
