1 SELECT PLAYER COMMITMENT FORM Birth Years BY SIGNING THIS DOCUMENT, I/WE AGREE TO THE FOLLOWING: I/We are registering the player named above as a member of Classics Elite Soccer Academy for the year. I/We understand that this commitment is for June 1, June 1, I/We understand that the selection and team placement of the player shall be at the discretion of the Classics Elite Soccer Academy coaching staff and is subject to change. I/We agree to abide by the rules, agreements, and policies (including Parent and Player guidelines and responsibilities) of Classics Elite Soccer Academy and all the leagues that Classics Elite Soccer Academy team participates in. I/We agree that the continued participation of a player is contingent upon the player and his/her family continuing to adhere to the rules, agreements and policies of all leagues and tournaments we participate and those referenced above. FEES: Annual Fee ECNL/TCL Team (U13-U18) $ Annual Fee CL/Pre-ECNL $ Annual Fee USC $ Annual Fee USC D/Local Bracket $ A $ DISCOUNT is available if fees are paid in full at time of signing commitment form. PAYMENT SCHEDULE: The $ commitment fee (non-refundable) is due within 24 hours of acceptance. The process in GotSoccer MUST be completed within 24 hours of receipt of confirmation or player will be removed from the assigned team. If full payment is not made, the remaining balance is payable in eight (8) monthly installments, June 2018-January The monthly payments will begin the month after the commitment fee is received. Payment dates are on the 1 st or 15 th depending on the payment plan you choose. 2 PAYMENT PLANS MUST HAVE A CREDIT CARD or ACH ON FILE FOR AUTOMATIC PAYMENTS Checks for full payment should be made payable to the CLASSICS ELITE SOCCER ACADEMY. All monthly payments are the responsibility of the player s family. There will be a late fee charged of $25.00 for payments more than 15 days late if your credit card is declined or the draft is denied. Fees include: Team Pre-Season Camp League Registration Fees Club Administration Fees Field Usage Fees Club Training fees College Recruiting Platform (8 th -12 th grade) *USC CL/ECNL League Referee/Field fees *Athletic Trainer (ECNL) *USC CL/ECNL fees not previously included in fees. FAILURE TO PAY: Failure to pay your monthly fees within thirty (30) days of payment due date will invoke the NO PAY- NO PLAY. Players will not be eligible to participate in team practices, games, and tournaments until payment is received. CLASSICS ELITE SOCCER ACADEMY realizes that emergencies may result in a financial strain on the family. In order to keep the player from being ineligible the Club will work with a family if the family contacts the office in a timely manner. The 30-day rule applies to all payments except the team fees. All team fees must be paid by date due. Fees do not include uniform kit, team tournament fees, personal travel costs, trainer travel cost or spirit wear/accessories. TEAM FEES: Team fees must be paid directly to the team manager/treasurer by check. Team fees vary depending on the variable costs of travel, coach travel fees, or events planned and tournament/showcase entry. Every player is RESPONSIBLE for team fees regardless of attendance. There will be no refund of team fees. Team Fees include: Elective Tournament entry fee as determined by club/coach for each team *Coaches/Team travel expenses Team travel Team basket for the Golf Tournament Non-league referee fees Scrimmage referee and game fees *All ECNL teams and U14 and older TCL/CL will travel together as teams 2
3 Classics Elite Soccer Academy is committed to each player for a full year, and we expect each player to be committed to CESA for the year as well. Classics Elite Soccer Academy has a stringent refund policy because a player's acceptance has the effect of denying another player an opportunity to play. Fees are not refundable or transferable. The annual club fees will cover June 1, June 1, No refunds, partial or full, will be made to players who choose not to participate in our competitive program at any point after registration for any reason, including players suspended from the program. Fees will not be refunded or forgiven due to player absences, withdrawals, or early release from a team. Club fees remain the same regardless of the duration of the season. No fee payment or parts thereof will be refunded or forgiven because of player absences, withdrawals, or release from a team. The only exceptions are if a player has a season-ending injury. The player must provide the office with the documented proof of an injury from the treating physician or facility. CESA will consider a partial refund in these circumstances on a case-by-case basis. Please note that refundable amounts are time-sensitive as costs accrue to the Club for services provided to the team based on player count. Consequently, the refundable amount will be affected by the date you request the refund, not the date of the event. UNIFORMS: Mandatory Uniform Kit* - All players** Youth and Adult Sizes $ (separate fee) Players will receive their number from the team manager. *Adidas Uniform Kit include two game jerseys with player number, two game shorts, two pairs of game socks, two practice jerseys, one practice short, one pair of practice socks, one full warm-up kit (jacket and pants) one backpack all with club logos. Adidas Goalkeeper Kit is the same as the mandatory uniform kit but consists of two) GK Jerseys. **Travel polo (required for all ECNL players) ordered through Soccer Factory online separately. Payment for the uniform is required when the order is placed with Soccer Factory online ordering. Deadline to purchase uniforms is June 15 th, Please registration@classicselite.com if an extension is needed. Please include player name/team in subject line. FINANCIAL AID: If an alternate payment plan or financial assistance is needed, you must complete the Financial Assistance Application. A minimum nonrefundable deposit of $ must be turned in with your application. All paperwork for the club must be submitted at the same time. If award is accepted the money will be credited to the club fees. A decision will be sent in writing by from the financial aid/scholarship committee. Your account will not be credited until the form is signed and returned. You will are required to volunteer hours, ten (10) hours per child/per season. NOTE: If you have multiple players in the club and need assistance with payments, or are requesting an extension of payments, a request can be made for financial assistance. Completion of the financial assistance application must be submitted by the due date listed below. Please contact the Club Administrator at registration@classicselite.com for an application. All forms may be found on our website in the member resource area. Application deadline for Current CE Players is May 15 th, 2018 and New Players is June 15,
4 IMPORTANT DATES: Please refer to the Classics Elite Soccer Academy website, including the web calendar. This will be your primary source for information and important dates throughout the year. A tentative schedule for is attached. Pre-Season Team Camp: TBD Please note this is the ONLY week that there will be testing. No make-up day is scheduled. Players should make every effort to attend. 5 PLAYER AND PARENT AGREEMENT Commitment to the Classics Elite Soccer Academy is for a year-long period, unless granted a signed release by the Director of Coaching. THE PLAYER AGREEMENT: The PLAYER should maintain a level of performance at school and at home which is sufficient to allow a commitment to participate in all team activities. The PLAYER should make every reasonable effort to attend practice sessions, games, and tournaments. The PLAYER should call the team coach in advance if unable to be in attendance of any team event. The PLAYER should be on time with CE gear on, shin guards, and an INFLATED ball for all practices and games. The PLAYER should always give the coach and staff their utmost respect and attention. The PLAYER should treat every team event as a learning experience. The PLAYER should be open to instruction and evaluation with a growth mindset The PLAYER should make every effort to win within the rules of the game. The PLAYER should respect game officials and the opponents regardless of the result. The PLAYER agrees that grievous unsportsmanlike conduct can be grounds for suspension or dismissal. PLAYERS should refrain from inappropriate or negative behavior, through actions or words toward any REFEREES, COACHES, PLAYERS and PARENTS. PLAYERS shall not use profane language on the soccer field. PLAYERS will be expected to follow all club guidelines, policies, and procedures. Failure to comply with the aforementioned Code of Conduct will be grounds for potential PLAYER and/or PARENT disciplinary action. During that time suspension or expulsion could be considered. 6 THE PARENT AGREEMENT: PARENTS must understand that playing time is at the discretion of the coach in alignment with league rules and club values. PARENTS should provide encouragement and create a good learning environment PARENTS should NOT COACH PLAYERS during games as it may contradict the coach s wishes and numerous voices DO NOT help the PLAYER or the team. PARENTS should be their PLAYER S number one fan and not their coach. The PLAYERS get plenty of coaching from the staff what PLAYERS need from you is positive support and unconditional love. If your cheering involves direction it is coaching. PARENTS should refrain from inappropriate or negative behavior, through actions or words toward any REFEREES, PLAYERS, PARENTS, and COACHES. Any unlawful act by PLAYERS or PARENTS could be cause for suspension or expulsion. PARENTS shall not use profane language on the soccer field. PARENTS should be willing to support the team by volunteering time for chaperoning, transporting PLAYERS, fundraising as requested, etc. PARENTS should follow the chain of command when issues arise. The first point of contact is the team coach, if needed the next point of contact is the Program Director of Coaching and finally the Director of Coaching. PARENTS agree to follow this communication protocol with any concerns or issues. There is a 24-HOUR RULE when speaking to coaches. PARENTS and Coaches will wait 24 hours after a practice or game to discuss team or PLAYER issues. This allows for emotions to subside, so an effective conversation may take place. PARENTS should not talk negatively about any other child. Unrequested input from parents on coaching decisions will not be welcomed. PARENTS will be expected to follow all club guidelines, policies, and procedures. Failure to comply with the aforementioned Code of Conduct will be grounds for potential PLAYER and/or PARENT disciplinary action. During that time suspension or expulsion could be considered. All players and their families are expected to abide by and support the policies and procedures included therein. If you have any questions, please contact the Club Administrator. I understand that I am responsible for all club fees and team fees for the season. I understand that I am responsible for paying team fees even if I decide not to attend a game/tournament. All players are expected to meet a high standard of commitment to training and play. Players are expected to attend all practices and participate in all league games and tournaments. All players and parents are expected to read and abide by the Code of Conduct. If you have any questions, please contact your team coach or the Program Director. I understand that we are an Adidas club and agree that my player will train and play only in the Adidas gear that the club requires. Although we strive to provide quality-playing time for all players, there are NO guarantees of playing time in any single game. All players should expect to play in several tournaments on holiday weekends, including Labor Day, Thanksgiving and possibly others. I understand and accept my commitment to the Classics Elite Soccer Academy and agree to abide by and support the policies and procedures included therein. I hereby release any officers, coaches, sponsors, or owners/lien holders of properties used by the Classics Elite Soccer Academy from any and all claims and causes of action of any kind whatsoever which the undersigned has or might have, known or unknown, now existing or might arise in the future, directly or indirectly attributable to any injury or damage that might result from my child s participation in soccer activities. I agree that Classics Elite Soccer Academy may use pictures of my child for any lawful purpose for such purposes as player identification card, publicity, illustration, advertising, and Web content. I authorize Classics Elite Soccer Academy, its assigns and transferees to copyright, use and publish my child s sports photos in print and electronically. I have read and understand the above I understand that I am responsible for Club fees and Team fees as described above. I understand if these fees are not paid, then the player will be ineligible to participate in team practices, games and tournaments until payment is received. The Parent or Court-Appointed Legal Guardian must sign for any participating minor (those under 18 years of age) and agree that they and the minor are subject to all terms of this document, as set forth above.
