Santa Margarita Catholic High School Boys Golf MISSION STATEMENT Our mission is to provide an exceptional golfing environment that nurtures talented players who share a mutual respect for the sport, for each other, for opponents and officials. Thank you for your interest in the Santa Margarita Catholic High School Boys Golf Program. We would like to welcome back our returning players and are excited to meet the new players and their parents. We are looking forward to having another great year. The goal of SMCHS Boys Golf is to not only have a fun team experience but also prepare our student athletes for a life of leadership. This packet contains all of the information and approval forms that must be completed and signed by parents and players before players can receive their uniform and be allowed to participate in the SMCHS Boys Golf Program. It is very important that each parent and player thoroughly read and review all attached forms. By signing the approval forms, you are indicating that you 1) understand the risks that may be associated with participation in the SMCHS Boys Golf Program and agree to accept those risks. 2) understand the rules applicable to your son s participation in the SMCHS Boys Golf Program, and that you and your son agree to abide by those rules. The team fee for this season is $1,800 for all levels (for new players an additional $150 for a golf bag). As a reminder, we are billing Boys Golf sports fees for 2018-2019 on FACTS on Feb 15, 2019. This is the school credit card payment system. You do have the option to pay the entire amount with a check if you choose to do so at the Parent Meeting. Any further assistance regarding payment plans past the scheduled due dates should go through Juliana Treadway in the Business Department. All player forms must be printed, completed, and returned at the Player/Parent meeting. If you have any questions regarding any of the forms in this packet, we will be happy to answer them at the meeting. Thank you, Coach Chuck Morales Santa Margarita Catholic High School Boys Golf ATHLETES CODE OF ETHICS

Please remind all of your families to go to the school website under ATHLETICS, then SPORTS MEDICINE, the physicals
information will pop up. You need to print and fill out the physical form and then SIGN UP by clicking on the click on Purchase tickets here. Also, there will be a change in athletic registration as your sport specific contracts, etc will no longer be digitally signed during the registration process. This will allow parents of multisport athletes to register their student for multiple sports with just a few clicks. This change will also allow us to move students from one sport to another as needed throughout the course of the year without forcing them to complete the registration process for the additional sport(s) if they did not originally register for them. 2018-19 athletic registration IS NOW OPEN!
