GENERAL POLICY This dcument applies t academic use f the facilities and equipment within Hgue. The safe cnduct f labratry activities within Hgue is the respnsibility f all faculty, staff, and students. This dcument applies t all students (undergraduate and graduate), faculty, and staff wrking in departmental shps/labs/facilities and turing field lcatins. DEFINITIONS Authrized An authrized lab participant can help thers during a lab emergency and has met the fllwing requirements: has received applicable lab safety training has passed applicable lab safety tests has submitted a signed cpy f the ETSC Lab User Plicy Agreement (Appendix A) t the curse instructr, which is kept in the lab-specific safety ntebk 1

Cmpetent A cmpetent persn can identify existing and predictable hazards in surrundings r wrking cnditins which are unsanitary, hazardus, r dangerus t peple, and wh will take prmpt crrective measures t eliminate them. Cmpetence is determined by dcumented training and/r experience. Qualified Buddy System In a qualified buddy system, at least tw trained and authrized peple must be present in a lab when the lab is in use. The respnsible/cmpetent curse instructr r cmpetent designee fr the specific lab must be n the premises f Hgue and peridically check n lab users when Qualified Buddy lab participants are students. Safety Data Sheet (SDS) A dcument required fr hazard cmmunicatin purpses, that is used t cmmunicate hazards f chemicals in a cnsistent user-friendly, 16-sectin frmat. SDS shall be available fr every chemical and mixture used in the ETSC Department. SDS explain the prperties f each chemical; the physical, health, and envirnmental hazards; prtective measures; and safety precautins fr handling, string, and transprting the chemical. Trained A trained lab participant has received training frm lab staff r staff-apprved individuals. The training will be apprpriate fr the specific tls r equipment t be used in each lab, and includes knwledge f all lab safety prcedures, emergency respnse prcedures and cntact infrmatin. All ETSC lab users must cmplete and sign the ETSC Lab User Plicy Agreement prir t cnducting wrk in an ETSC labratry (see Appendix A). Generally, labratry use is nly during business hurs. Fr the purpse f this plicy, regular business hurs are defined as 8 am t 5 pm, Mnday thrugh Friday, during the academic year. 2

All after hurs r hliday use f this building must be arranged fr in advance and scheduled with the rm crdinatr. See the ETSC Building Use Plicy fr additinal infrmatin. After hurs r hliday use f labratries shall require the supervisin f cmpetent instructr(s) r staff. The respnsible/cmpetent curse instructr r cmpetent designee fr the specific lab must ensure users are trained in equipment r tl peratin and verify that the Qualified Buddy System is being used. Cmpetent instructrs r staff must peridically check the students in the lab t ensure cmpliance with lab safety requirements. When the wrk is cmplete, the cmpetent instructr r staff must inspect and lck the lab. SAFETY The ETSC Department is cmmitted t prviding an academic envirnment in which recgnized hazards are eliminated r cntrlled. The bjectives behind this cmmitment t safety and health are as fllws: Wrk shall be cnducted s that it enhances the persnal safety and health f students, faculty, staff, and visitrs. Classrm, labratry, and prject wrk shall be cnducted in a manner that prtects peple, the envirnment, the equipment, and the public. Everyne is expected t cnduct their daily tasks in a manner that is cnsistent with the bjectives f this plicy as well as any envirnmental, health, and safety regulatins r guidelines practiced by CWU. ETSC Department safety plicies and standard perating prcedures are intended t supplement and enhance existing university safety plicies and prgrams, and will be reviewed regularly. Faculty and staff shall cmply with all aspects f this plicy. Failure t d s culd result in disciplinary actin up t and including terminatin. Safety Cmmittee In rder t fulfill the abve bjectives, the ETSC Department has established a safety cmmittee. The ETSC Department Safety Cmmittee s (ETSC-SC) missin is t act as a resurce t ETSC, thereby assisting with prtectin f the safety and health f students, faculty, staff, and visitrs, as well as ETSC equipment and facilities (see cmmittee by-laws in Appendix B). A representative f the CWU envirnmental health and safety department will be a member f the ETSC-SC. Labratry Safety Training Guidelines Infrmatin, training, and supervisin are prvided as an integral part f instructin fr curses with a lab cmpnent. Lab- and task-specific training n ptential hazards and accident preventin shall be prvided by the cmpetent instructr r designee. Fllwing all training, the instructr shall verify student cmpetence in safe perating prcedures (SOPs) fr equipment and tls prir t allwing the student t use the Qualified Buddy System. If a student is unable t shw cmpetence in perating equipment r tls accrding t the SOP r manufacturer s instructins, the instructr shall retrain the student under questin. 3

All training shall be dcumented using a Lab-Specific Training Sign-Up Sheet r equivalent frm. This sign-up sheet shall be kept n file by the curse instructr in the lab-specific safety ntebk. A sample Lab-Specific Training Sign-Up Sheet is prvided in Appendix C. Each signed cpy f the training sheet is t be kept in each respective lab during the curse f the class, and maintained in the lab-specific safety ntebk. Student nn-cmpliance will result in suspensin frm the labratry and may be cause fr revcatin f labratry privileges. Exceptins t these regulatins will require the prir written apprval f the Department Chair r designee. General Safety Requirements fr Labratries General safety requirements fr ETSC labratries will be cntinuusly enfrced by the cmpetent instructr r designee. Nn-cmpliance may result in suspensin frm the labratry, at the discretin f the cmpetent instructr r designee. The ETSC Department believes understanding inherent hazards and learning hw t wrk safely is an integral and imprtant part f the academic prcess. All persnnel (faculty, staff, student emplyees and students) are expected t: Read, understand, and cmply with the ETSC Department Safety Plicy. Knw the lcatin f exits, evacuatin rutes, and emergency equipment (e.g., eyewash, fire extinguisher, spill kit, safety shwer, SDS) and hw t use them. Wear apprpriate clthing fr hazardus labratry envirnments, which includes lng pants and clsed te shes. Wear the apprpriate persnal prtective equipment (PPE) fr the specific labratry and task being cnducted. PPE requirements are psted utside the drs f each lab, when applicable. Maintain unbstructed access t all exits, fire extinguishers, electrical panels, emergency shwers, and eyewashes. Understand that hrseplay r ther acts f carelessness are prhibited. Perfrm n unauthrized experiments, tasks, r jbs, and perfrm given experiments, tasks, r jbs nly accrding t the instructr s directins. N hazardus equipment may be used withut prper training and demnstrated cmpetency. Nt damage, mutilate r willfully misuse equipment, tls r facilities. Cperate with safety requests frm university faculty and staff. Use the crrect tls and equipment fr the jb. Fllw all tl and equipment-specific perating prcedures as well as specific safety prcedures and/r plicies as required. Operate nly the equipment that yu have been trained and authrized t use. Reprt defects/breakage in labratry equipment and equipment safeguarding t rm crdinatr. Equipment must be used in accrdance with manufacturer guidelines. Understand that willful vilatin f lab safety rules r safe wrking practices will result in suspensin frm the labratry and may be cause fr revcatin f labratry privileges. Use the required safety equipment and PPE, including glves when handling any hazardus r txic agent. Glves shall be remved befre leaving the lab and entering public areas. 4

Never vilate r ignre any warning signs r barricades. Never walk r stand under suspended lads. Ensure that all chemicals are clearly and currently labeled with either the manufacturer s riginal label, r the substance name, cncentratin, date, and name f the individual respnsible. Secure all cmpressed gas cylinders and ensure the labels are legible. Ensure that all electrical equipment used near a water surce is GFCI-prtected. Avid using extensin crds whenever pssible; if yu must use ne, inspect it prir t use and ensure the grunding plug is nt damaged. When using cmpressed air, use nly apprved nzzles and never direct the air twards any persn. Understand that if yu are leaving a lab unattended after regular business hurs, yu must turn ff all ignitin surces and lck the drs. Be cautius when lifting heavy bjects lift cmfrtably, aviding any unnecessary bending, twisting, reaching ut, and excessive weights. Reprt any incidents, near misses, injuries, r illnesses t the curse instructr r department chair. Incident and Hazard Reprt frms are als available thrugh CWU Envirnmental Health and Safety nline reprting. Reprt and clean up any spills immediately. Understand that if yu are nt sure hw t perfrm yur assigned task safely, stp and cntact yur immediate supervisr r curse instructr. Lab use during nn-instructinal days must be crdinated with the cmpetent instructr r designee and the rm crdinatr. Building and lab maintenance will be perfrmed during the clsures. During nrmal instructinal days: All lab drs will be lcked when the lab is nt in use. All required PPE shall be wrn fr wrk cnducted in specific labratries, as indicated n the labratry dr. PPE assessment fr labratries and tasks is the respnsibility f the cmpetent instructr r designee. Cnsideratin shuld be given t ther activities that culd pse a hazard. 5

Other requirements f labratry use are detailed belw. Each lab shall have safe perating prcedures (SOP) r jb hazard analysis (JHA) dcumentatin, as well as equipment-specific SOPs that are used fr training each equipment user. Labratries will be available fr trained and authrized student use n instructinal days during regular class time. Additinal lab time may be scheduled with instructr permissin (r with knwledge f rm crdinatr). The curse instructr is respnsible t brief students n all SOPs, enfrce the use f SOPs, clean-up activities, and rm/equipment security. Students are respnsible t cmply with all SOPs and PPE requirements. When a lab is being used fr a class r ther instructinal use, it will nly be available fr students currently enrlled in the curse that is in sessin unless permissin is prvided by the instructr. Students wanting t use the lab utside f scheduled class/lab time will need t crdinate such use with the individual instructr. Use will be restricted t thse students having cmpleted and passed curse wrk incrprating specific lab equipment. The curse instructr(s) shall be respnsible t maintain a list f trained and authrized students meeting these requirements in each lab-specific safety ntebk, in cmpliance with FERPA requirements. Lists will be active fr a minimum f fur (4) years n a revlving basis fllwing the grade pstings frm the previus quarter. Lab use by students at any given time is permitted nly fr students using the Qualified Buddy System and with permissin f the instructr. The persn perating the equipment must be trained, authrized, and cmpetent in equipment peratin. The Qualified Buddy System requires that the secnd persn present in the lab is trained and authrized as well. Cmpetent instructrs shall ntify lab users if the Qualified Buddy System des nt apply t a specific teaching labratry (fr example: the Machine Lab has additinal requirements beynd the Qualified Buddy System). Any additinal lab use requirements shall be explained t lab users in applicable curses. The cmpetent instructr r designee that unlcks the dr t a lab fr student use utside f scheduled class time is respnsible fr verifying lab users are trained and authrized in safety, relevant SOPs, clean-up, and rm/equipment security. The cmpetent instructr r designee is als respnsible fr mnitring lab users peridically and t enfrce applicable safety plicies and prcedures. All lab users (faculty, staff, students, and visitrs) have a respnsibility t stp any unsafe acts and reprt any unsafe cnditins bserved in ETSC labratries. Any hazardus material brught int the lab must be apprved by the rm crdinatr r cmpetent instructr. Material must be labeled with the wner s name and cntact infrmatin. Apprpriate SDS fr the substance must be prvided and the material/substance must be prperly dispsed f at the end f the quarter. Any material left past the end f the quarter becmes the prperty f the university. Students will be charged fr items lst r nt returned. Students shall nt be issued lab keys under any circumstances. 6

Lab Physical Cnditins Each lab will be equipped with a labeled and accessible first aid kit and applicable spill respnse materials. Each lab will be equipped with a safety ntebk (physical r electrnic) cntaining the fllwing: 1. Safety Data Sheets 2. List f students qualified t use lab fr each curse (Respnsible party: Curse Instructr) 3. ETSC Building Use and Safety Plicies 4. Lab specific safety prcedures and SOPs (Respnsible party: Curse Instructr) 5. The cmpsitin f the ETSC-SC will be as fllws (five members): ETSC Department Chair One CWU Envirnmental Health & Safety Department representative One faculty representative (can be ETSC-SC Chair) One faculty member frm the Safety and Health Management Prgram (can be ETSC-SC chair) One department lab technician Meetings: The Cmmittee will meet at least nce per quarter fr ne hur t review safety-related cncerns and injury/illness data fr activities within ETSC. Written minutes shall be maintained by the Cmmittee Chair and cpies will be sent t ETSC persnnel. Cpies f minutes shall be maintained in the Department ffice and shared netwrk drive. Meetings will be held at Hgue Technlgy Building, 3 rd Flr Cnference Rm (300N). A qurum f >50% is needed fr a meeting t prceed. Inspectins: Safety inspectins f ETSC facilities shuld be cnducted n at least a quarterly basis by ETSC- SC members r their designee. This will be dne prir t the quarterly meeting and the results will be discussed during the meeting. Inspectins shall include, but are nt limited t: fire extinguishers, first aid kits, eye wash statins, student PPE cmpliance, hazard cmmunicatin, facility signage, chemical safety, etc. Safety cncerns shall be recrded using the ETSC Safety Inspectin Frm, which is prvided in Appendix D. Cpies f the inspectin frms will be frwarded t the ETSC Department Chair. Training: Training sessins fr ETSC persnnel will be cnducted accrding t state and federal regulatry guidelines. Training sessins may include tpics such as: evacuatin prcedures, first aid and CPR, fire safety, building safety, equipment safety, lab safety prcedures, hazard cmmunicatin, etc. 9
