Informed Consent to Medical Treatment: I request that in my absence the above-named minor be admitted to any hospital or medical facility for diagnosis and treatment. I request and authorize physicians, dentists, and staff, duly licensed as Doctors of Medicine or Doctors of Dentistry or other such licensed technicians or nurses, to perform any diagnostic procedures, treatment procedures, operative procedures and x-ray treatment of the above minor. I have not been given a guarantee as to the results of examination or treatment. I authorize the hospital or medical facility to dispose of any specimen or tissue taken from the abovenamed minor. I assume full responsibility for all costs of such treatment. SOCCER CODE OF CONDUCT At no time may a player or spectator: Approach an official, timekeeper or scorekeeper in a confrontational manner Enter the field area unless you are playing. Spectators are to remain in the bleachers or off the field. Engage in unsportsmanlike conduct, including negative cheering, yelling towards an opposing team s spectators, players or coaches or referees Allow his/her young children to roam free or play at any facility, including but not limited to the playing field, on top of or under bleachers, in hallways, foyers, restrooms, on other equipment /buildings at the field or in the parking lots of practice or games. St. Francis of Assisi Soccer 2019 Dear Parents, The St. Francis of Assisi Sports Program is a parish organization led by the sports director, sports board and the pastor. This will be our 2nd season entering a girls soccer team in the VCAC League (Va. Catholic Asso. Conference). Our objectives for the teams at St. Francis are: LEARN THE SPORT WORK TOGETHER AS A TEAM UNDERSTAND AND GOOD SPORTSMANSHIP HAVE FUN Each year our teams have done remarkably well. Much of this has to do with the dedication, time and patience of the coaches, some of whom are parents of players. With this in mind, please remember that any help you can offer your child s coach/team is greatly appreciated. The cover page lists all headings contained in this sports packet. By signing this page you agree that you have read all the information contained in this packet and will abide by and agree to all rules, procedures and fees due. The Cover Page, Permission and Medical slip, Code of Conduct, and t-shirt order form must be completed and returned before your student can participate. FEE Registration: $50.00 T-Shirt: $20.00 Please make out separate checks made payable to: St. Francis of Assisi Memo: Soccer TRANSPORTATION Parents are responsible to get their child to and from practices and games. SCHEDULE The practice / game schedule is attached. Coaches will notify parents if there are any changes in the schedule by text/email. Remember, if your email is down, you must notify your coach. Games are on weekdays. GAME LOCATION: Games are in Pr. William, Stafford and Spotsylvania counties. GOOD SPORTSMANSHIP All players represent St. Francis of Assisi parish and thus are expected to exhibit respect and good manners when they are at practice, games or guest at other parishes or public fields. Parents, remember these players are elementary/middle school students learning the sport, not professional players. UNIFORM Soccer t-shirt (to be ordered through sports program) and PE shorts To be purchased by parent: Shin guards Soccer Cleats Socks maroon or red SPORTS AWARD BANQUET The awards banquet will be held in the Spring. The date and details will follow later in the season. Basketball, Cheerleaders, Volleyball, Track and Cross Country and Soccer will be honored at the banquet. ELIGIBILITY A child is eligible to participate in Soccer if they are in the 5 th 8 th grade and attending the parish school. SPORTS POLICY The St. Francis of Assisi Parish Sports Director and respective coaches, have sole discretion on team composition and player placement. The parish sports director and parish sports advisory committee do not entertain complaints regarding a parent s differing opinion or evaluations of their child s skill level/team placement. COMMUNICATION Coaches will do most if not all of their communication through text messaging and or email. Occasionally a phone call will be made in case of last minute practice or game cancellation. Parents and coaches should communicate with each other when there is a concern about a player. Parent should talk to the coach after practice or through email. They should not approach the coach with their concern after a game. PARENT PARTICIPATION/INVOLVEMENT Parents are an important part of any sports program. Without your involvement, children would not be able to get to and from practice and games, there would be no coaches, sports director, sports committee etc. Remember, those who volunteer to coach need your cooperation and support. If you feel a need to talk to a coach about his/her coaching approach, please consult with the coach and see if a different approach would work better for your child. A good coach is open to any concerns from the parents. Both parties should be respectful of one another and never demeaning to each other.

ST. FRANCIS OF ASSISI AND GAME RULES 1. Player who misses practice without a valid excuse will be ineligible to play subject to your coach s discretion, in games that week. Missing practice or a game because of another sporting event, dance lesson/recital, scouts etc. is not a valid excuse. If you miss one of your practices each week due to a parish activity it will be up to the coach s discretion as to whether your child will play. 2. Parent is to make sure player calls the coach if they are unable to attend practice or game. 3. Player is expected to arrive at games 20 minutes prior to the scheduled game time. 4. Parents attending practices or games are to refrain from sideline coaching such as calling to a player from the sidelines or stands on how the play should be executed. 5. All siblings or young fans, not participating in the sport, must be sitting in the stands with an adult or parent at all times. No small children should be on the field or the sidelines due to the possibility of injury. No child should be playing during game breaks or between games on the field. 6. Parents must pick up their player on time after practice. Parents, siblings or other adult will come to the coach to pick up their student. No player will be allowed to leave the field without an adult with them. 7. No parent/adult is to drop off a player without first checking to see if the coach is at the field. 8. All concerns/complaints should be addressed first to the coach. If they cannot be resolved, parents are to contact the parish sports director who will meet with the parents and coach. If no solution can be found, parents must submit in writing their concerns/complaints in a signed letter to the parish sports advisor board with a phone number and time to contact them. They may send the letter to the school addressed to the Sports Advisory Board. PLEASE NOTE: This is a PARISH organization under the leadership of the pastor.
