LEVEL 5-8 BOYS BASKETBALL 2017-2018 Approved by: Participant Registration & Release Form LEVEL 5-8 BOYS BASKETBALL Cost: $75 Basketball Activity Fee and $45 uniform fee Coaches: Level 5 & 6 TBD, Level 7-Coach Richardson, Level 8-Coach Warda Roster size: 12 players, registrations are accepted on a first-come, first-served basis. A minimum of 8 registrations are needed to form a team. All forms and fees must be submitted together by Friday, October 27, to secure a roster spot. Keller will host a Level 5-8 boys basketball team in Ridge Park Basketball League at the Ridge Park District (9625 S Longwood Dr. Chicago IL 60643) this year. Level 5 games will be held on Sunday, Level 6 games will be held on Saturday, Level 7 games will be held on Friday and Level 8 games will be held on Monday. Games will tentatively start in early December. Each team in the league will play 12 games. Please note, details and fees for local boys basketball tournaments are not finalized at this time. If the Keller team chooses to enter a local tournament, then a separate registration form will be forthcoming to cover those fees. BASKETBALL UNIFORMS: Select one jersey size and one shorts size. We encourage you to order a size up so jerseys can be worn by your child for two years. ELIGIBILITY REQUIREMENTS: 1. All participants must be active members, in good standing, of Keller Athletic Association. Annual dues for KAA membership are $20 per family for 2017-2018. 2. All participants must be in compliance with the academic and disciplinary requirements of Keller. 3. Any student who is on probation for academic or disciplinary problems will be deemed ineligible until such situations are rectified to the satisfaction of the school s administration. 4. To participate in a future sport, fees from prior sports must be paid. 5. For league participation, no cuts will be made except when mandated by roster limitations. 6. Basketball registration is on a first come, first served basis until rosters are filled. A player s name will be added to the roster only when the following registration materials are submitted: *2017 Basketball Participant Registration & Release Form *2017 Player and Parent Guidelines *Basketball activity fee (either $75 or $120) *2017-2018 KAA Membership Form *$20 KAA membership fee *Optional shirt/hoodie order form(with fees) PARTICIPATION REQUIREMENTS: 1. Participant s family agrees to pay the following activity fee: $75 (if using last year s uniform) or $120 (if purchasing a new uniform). Activity fee includes all league fees, referee fees and uniform (if purchasing a new uniform). This fee does not include any tournament fees that an individual team may opt to participate in. Payment and all forms must be received by Friday, October 27, 2017. Please make checks payable to: Keller Athletic Association. 2. Activity fee is non-transferable and non-refundable. If Keller Eagles are not able to field a complete team(minimum of 8 players), payment will be returned. 3. Regular attendance, participation, effort and cooperation are required at ongoing practices and games. 4. Playing time of each student athlete is at the coach s discretion. 5. Each participant s family will be responsible for helping to provide Adult Service as needed. 6. Each participant s family will be responsible for their own transportation, allowing for timely pre-game arrival and prompt pick-up from ongoing practices and games. Repeated/excessive late pick-ups may result in forfeiture of privileges and fees. Supervision is not available after scheduled activities. 7. Excessive, unexcused absences, from practices or games, may result in forfeiture of privileges and fees. 8. All players and parents are required to sign the Parent and Student Code of Conduct. 9. Any player who forfeits participation privileges may be deemed ineligible for participation in future activities.

Player and Parent Guidelines and Responsibilities Player Guidelines 1. Playing sports at Keller is a privilege. 2. I will put academics first. 3. I understand that sports are a commitment. 4. I will attend all practices and games and if it is necessary to miss a practice or a game, I will notify the coach. 5. I will respect the coach and assistants and my teammates. 6. I will learn the rules of the game and always try my best. Failure to comply with the guidelines below may result in reduced playing time, game suspension or removal from the team. I have read and understand the Player Guidelines. STUDENT SIGNATURE: DATE: Parent Guidelines When attending a Keller sporting event, I will remember that I am an ambassador to Keller and will maintain a positive image. I hereby pledge to be responsible for my words and actions while attending a Keller sports event and shall conform my behavior to the following code of conduct: 1. I will not engage in unsportsmanlike conduct with any coach, parent, participant, official or any other attendee. 2. I will not engage in any behavior which would endanger the health, safety or well-being of any coach, parent, player, participant, official or any other attendee. 3. I will not engage in the use of profanity. 4. I will allow my child to decide if he/she wants to participate in sports. 5. I will teach my child to play fairly and do their best. 6. I will not encourage my child, or any other person to engage in the use of profanity. 7. I will treat any coach, parent, player, participant, official or any other attendee with respect at all times regardless of sex, creed, color, national origin, sex or ability. 8. I will not engage in verbal or physical threats or abuse aimed at any coach, parent, player, participant, official or any other attendee. 9. I will be responsible for the behavior of all those attending a Keller sports event on my child s behalf. 10. I will not shout instructions, coach or direct players on the field from the stands nor the sidelines. 11. I will not address the officials from the sidelines in any manner. 12. I will respect coaches for the time they donate and will not publicly confront or criticize them. 13. I will not approach a coach with a complaint or criticism for 48 hours after a game. I understand that any violation of this code of conduct could result in being banned from attending any Keller Athletic events.
