Mail-in registration begins Dec. 14 and is available on a first-come, first-served basis. Each trip requires registration - the registration/conduct form MUST be signed by the participant and their parent. COMPLETE FORM ( with signatures ) AND MAIL YOUR REGISTRATION to: Plymouth Parks & Recreation 3400 Plymouth Blvd, Plymouth, MN 55447 Registration must be received at the Parks & Recreation office by 6:00pm on the Tuesday prior to that Friday s trip. City of Plymouth Page 3 Page 4 Refund & Transfer Policy If circumstances arise preventing your participation, you may cancel with the Parks and Recreation department at 763-509-5200. If you cancel before the Tuesday 6:00 p.m. deadline you may transfer to another trip, or receive a refund minus a $5 withdrawal fee. If you cancel on Wednesday or Thursday by 4:30 p.m. ( t he week of the trip ), you will receive a refund of the lift and rental fee only. No refund or transfer if cancelling after Thursday 4:30 p.m. Trip Cancellation Policy A trip may be cancelled due to insufficient enrollment or if the wind-chill is 15 degrees below zero ( based on the National Weather Service wind-chill formula ). If there is insufficient enrollment, the decision will be made on Wednesday. Weather decisions are made on Friday at 10:00 a.m. based upon the forecast for the trip location and on transportation considerations, utilizing the National Weather Service and the Weather Channel. If a trip is cancelled, a notice will be posted after 10:30 a.m. on the Parks & Recreation weather line 763-509-5205 and the city website www.plymouthmn.gov. Special Needs Please note any special needs ( ex. ADHD, asthma, hearing impaired, Equipment Storage A locked storage area for equipment will be available on the day of the trip at West Middle School in the lower level by Industrial Tech. Equipment drop-off is 7:55-8:15 am and pick-up is 3:10-3:25 pm. The WWMS Ski & Snowboard club is not responsible for lost, damaged, or stolen items. Bus Departure Meet on trip days in the lower level in the Industrial Tech hallway. The bus PARTICIPANT RESPONSIBILITIES ON THE HILL Always show courtesy to others, and be aware that there are elements of risk in skiing and snowboarding that common sense and personal awareness can help reduce. Observe the code listed below: Stay in control and able to stop. People ahead of you have the right of way. It s your responsibility to avoid them. You must not stop where you obstruct a trail or are not visible from above. At the Hill Upon arrival, announcements will be made by trip advisors and chaperones - ask questions if you did not hear the announcements. It is your responsibility to know when the bus leaves for home - a time for departure will always be announced. Individuals who return late from the slopes may have their membership suspended for a trip. Chronic tardiness will lead to membership termination. Memorize the bus name and number you are arriving on. Please note that all members will remain on the bus while club leaders collect our lift tickets and rental forms, and unload our equipment. City of Plymouth Page 5 Page 6 Emergency Procedures Please indicate a home, work and cell number for parent/guardian and an alternate emergency contact other than the parent. In the event of an injury, the area ski patrol will provide immediate first aid and the ski area s emergency procedures will be followed. If the injury is not severe but requires transportation, a parent/guardian may pick up the child at the ski area, or the student may be transported on the bus. A chaperone will stay with Stolen, Damaged or Lost Gear You are responsible for your equipment! Rip-offs occur each season and it is disappointing. Invest in a ski or snowboard bag to protect your equipment while it bumps along in the trailer. Lock your skis and boards! A ski/board lock is a good idea. Make use of the Ski and Board check at the areas we visit. All Bus Return The bus will return by 11:00 pm. A late pick-up fee of $5 will be assessed, plus an additional $5 for every 15 minutes you are late for pick-up. Program registration will be denied after the 2nd late pick-up. The late fee must be paid before participation in the next trip. The school will be locked Wear a watch - Buddy up - Bring a bag dinner Wear a helmet - Lock up your stuff Bring quarters for lockers - HAVE FUN! Wayzata West Ski & Snowboard Club Trip Leaders Trip Leaders are student volunteers who help with trips and equipment storage. If interested, speak to Ms. Gallagher or CODE OF We are a non-smoking, alcohol and drug free club. Zero tolerance is our attitude - please read over your Code of Conduct again if this is not a clear statement. We expect all members to abide by our rules. Parents/guardians/schools will be notified for smoking offenses and the authorities will be called to handle alcohol or drug offenses. Termination of membership will apply if you are caught smoking, using drugs or alcohol. It is the desire for all members to enjoy the passage of time with courteous, kind and friendly traveling companions. We re here to have fun skiing and snowboarding. If at any time you feel negatively affected by someone else s behavior you have the right to speak freely and confidentially to a chaperone about your issues. On the bus everyone is responsible for themselves. No food, drinks or pop. Noise must be held to acceptable levels. To reduce excessive noise, only music/gaming players with earphones ( ie. IPod, PSP, MP3 ) will be allowed on the bus.. Your chaperones are a friendly bunch who look forward to each outing with as much enthusiasm as you. Be warned, they are empowered to enforce our rules and will expect the following covenant with you.
