Dear Students and Parents: Welcome to the Sangre de Cristo School of Dance Summer Intensive Levels 4-7 Registration Packet The summer intensive for levels 4-7 will be for two weeks from July 31-August 11 from 10:00 a.m.-6:00 p.m. weekdays and will culminate in a demonstration for friends and family on Saturday, August 12th at 2:00 p.m. Registration for summer intensive will be open until Monday, July 10th at 5:00 p.m. At that time we will close registration and determine if we have sufficient enrollment to host the summer intensive. Confirmations will be sent out 48 hours after. Please fill out this packet and bring it to the School of Dance office by Monday, July 10th or fill out the online registration link provided below. Online registration link: goo.gl/esfghv Summer Intensive Pricing: $200.00 per week, $400.00 full session This summer intensive is designed to combine classes with professional workshops to accomplish three goals throughout the two weeks. 1. Classes Ms. Nan and Ms. Brittany will host classes that will focus on learning concepts and tool that will encourage technical improvement that will be carried forward to all future classes, auditions, rehearsals, performances, and beyond. 2. Choreography and Composition Workshops Ms. Nan has some big plans for public performances throughout the school year. The content of these performances will be based on choreography that is developed by the students with the assistance of Ms. Nan and Ms. Brittany during the intensive. Students will work individually and as part of a team to compose movement that tells a story and expresses emotion. 3. Performance Technique Ms. Nan will focus on practicing some essential tools to enhance presentation and artistry onstage and prepare participants for the rigors of auditions, rehearsals, and performances. Sangre de Cristo School of Dance Summer Intensive 2017 STUDENT/PARENT POLICY AND AGREEMENT Required for registration at Sangre de Cristo School of Dance ( School of Dance ) RELEASE OF LIABILITY MEDICAL EMERGENCY The School of Dance will not be held liable for any injuries or property damage. The enrolled participant and/or the parent/guardian recognize and understand that dance/fitness training is a potentially hazardous activity and the risks inherent in dance training include the possibility of serious physical injury. I/we hereby agree to indemnify and hold harmless the School of Dance, its instructors, officers, directors, agents, and employees against any liability resulting from any injury that may occur to the participant while participating in School of Dance activities. The participant and/or parent/guardian agree to indemnify the School of Dance for any damages incurred arising from any claims, demand, action, or course of action by the participant. I/we authorize any representative of the School of Dance and/or Sangre de Cristo Arts and Conference Center to seek medical treatment for the participant if needed during his/her participation in activities of the School of Dance. I/we agree to pay all costs associated with medical care and transportation for the participant. Any special medical/health problems, i.e. allergies, physical restrictions, injury history, learning disabilities, and/or medical emergency information (EpiPen or inhalers) that staff should be aware of must be listed on the registration form. DISCLAIMER CONSENT All teachers at the School of Dance use a "hands-on" approach to teaching. In order to facilitate the student's learning, the teacher may physically guide a movement or correct body placement by touch as necessary. This will be in a professional manner and the intention of the contact will be made clear. The participant or parent/guardian understands and gives consent for our teachers to use this helpful teaching method during the participant's lessons and rehearsals. PHOTOGRAPH LIKENESS VIDEOTAPE RELEASE The enrolled participant and/or the parent/guardian authorize the School of Dance and/or its representatives, agents, or employees to photograph and/or videotape and use any photograph/likeness of the participant for publicity, choreographic archives, and promotional materials, both online and printed. INCLUSION POLICY The School of Dance does not discriminate on the basis of race, color, religion, sex, age, sexual orientation, or national/ethnic origin. ATTENDANCE There are no makeup classes for summer intensive. The student assumes the responsibility of attending all classes. No refunds will be made except in the case of illness or injury causing the student to miss all or most of the session. SCHEDULE CHANGES There are no expected schedule changes for the Summer Intensive. However, if it becomes necessary, you will be notified. However, please note that these changes do not alter the cost of the semester. No refunds or credits will be given in this situation. WITHDRAWAL DROP ADD Withdrawal : This is from ALL classes for a semester or the season, resulting in the enrollment cancellation of the student. A form must be filled out and turned into the School of Dance office. Summer Session and Summer Intensive do not have a drop or add periods. ILLNESS INJURY Students who are ill (infectious) should not come to class. Extended illness requires a doctor's note upon return to class. Injured dancers are encouraged to observe class in order to remain familiar with the curriculum. Students will be asked to write their observations of that class and give to the instructor after class. TUITION Payment is due by first class of the Summer Intensive. If tuition is not received, the student will not be allowed to attend class until a payment is made. If the student's name cannot be found on the attendance sheet by the instructor, then that student must see the office administrator to pay before they can attend class. Payment is collected at the School of Dance office in the form of card, cash, or check. Card payments can also be collected over the phone.

Reporting an Absence: Please contact the office either by phone or e-mail to report class absences due to illness or other valid reasons. This will be an excused absence. 3. Contact School of Dance Office for: Tuition, fees, class schedule, rehearsal schedule, etc. 4. If you need to speak with the Interim Artistic Director, Nan Wainwright, about an issue of a sensitive nature, please call the office and a time will be scheduled. ATTIRE: Students should wear outer clothing (sweat pants, jackets, etc.) over tights/uniforms to and from classes. This is for several reasons - safety, weather, wear and tear on uniforms, etc. THE "BLUE DOOR": When closing the "Blue Door," please push hard and check to make sure it is closed. SAFETY: All students (or parents/guardians if the student is too young to sign themselves in), regardless of age, must be signed in and out of the book located on the table in the theater building lobby. This is a security measure taken to insure that we know when students have left the building. PAYMENT & CLASS CANCELLATION POLICY Payment for the Summer Session and Summer Intensive must be paid in full by the 1 st day of the class. There is no drop period for the Summer Session or Summer Intensive. Scholarships are not available for the summer intensive. We reserve the right to cancel the intensive if it does not meet minimum numbers of enrollees. If the intensive is cancelled, all enrollees will be notified via email and/or phone call. For Girls: Sangre de Cristo School of Dance Summer Intensive 2017 Dress Code For Ballet Class: Solid color leotard, pink tights, pink ballet slippers, short solid-colored dance skirt permitted after barre. For Contemporary/Jazz/Modern: We recommend bringing a change of clothes for these classes. Leggings, tank tops permitted. No midriffs. Hair: All female students must prepare their hair in a tightly-fitted bun that is secure enough to withstand the quick head movements associated with turns for ballet. Students may wear ponytails in Jazz and Modern/Contemporary. Female students with short hair who are unable to secure their hair in a bun must wear a headband and use hairpins to make sure that the hair remains off the forehead, face, and the back of neck.
