JOB HAZARD ANALYSIS (JHA) JHA Preparation Instructions: JHAs are to be prepared by the Supervisor or Group Manager in charge of the work, with work group member participation. The goal of preparing a JHA is for those responsible for or performing any work to review the risks and possible complications of that work and to consider ways of mitigating those risks and complications. A JHA shall be completed for all work executed. The JHA## is to be incremented when a procedure requires more than one JHA form. Standing should be marked if job or task is performed regularly and work group members are always the same individuals. Supervisors are responsible for getting the work group members sign off completed and a copy sent to the safety officer for each JHA. JHA Change Instructions: If changes to the content of this JHA are required, follow the KPNO ETS Engineering Change Process. The master copy of the document must be checked out of the Document Vault, an Engineering Change form must be completed, and the Revision Table (see last page) must be updated. Note that periodic reviews of this JHA that do not result in content changes require only entry of the review date and the name of the reviewer in the Revision Table; the document revision should not be advanced in this case. EL Electrical ER Ergonomics FB Fall to Below FH Fire Hazard Item No. 1 2 BASIC JOB STEPS 1.1 Ensure the telescope drives and hydraulic pumps are off and tagged out according to the Lockout/Tagout Standard Operating Policy (1000-AD-015-0001) and the Lockout/Tagout Standard Operating Procedure (2100-AD-015-0065). 1.2 Refer to the Fall Protection Plan SOP (1000-AD-015-0003). 1.3 Refer to the Rescue Plan SOP (1000-AD- 015-0004). Ensure the telescope is at Zenith position. Note: Place a Maintenance in Work placard in the Control Room. HAZARDS PRESENT IN EACH JOB STEP EL, SE CB, CI, CO, ER, FB, SB CB, CI, CO, ER, FB, SB EL, SE CORRECT AND SAFE PROCEDURES AND PRECAUTIONS Note: These procedures must be reviewed prior to starting the process of installing and removing the Zenith stay bars. -Electrical equipment must be handled with care to prevent shocks, burns and other injuries from occurring. -Workers must always review the Lockout Tagout SOPs (1000-AD-015-0001 and 2100-AD-015-0065) before handling electrical equipment. 1000-AD-015-0059-JHA Item No. 3 4 5 6 7 8 9 BASIC JOB STEPS Install the Hour Angle (lower stay bar) to its corresponding attachment points using the attached push pins. Note: A minimum of two workers is required to complete this step. Position the 5-ton dome crane to a predetermined spot to attach the Man Basket to the crane hook. Note: A minimum of two workers is required to complete this step. Connect the 4-point lifting cables to the Man Basket. Note: Connecting the lifting cables incorrectly will cause the basket to lift unevenly. Connect the Declination (upper stay bar) to the Man Basket using the attached web sling assembly and its lifting ring. Lift the Man Basket slowly and move it towards the North end of the telescope, as needed. Note: Begin from floor position. Only one person is able to use the Man Basket at a time. Install and secure the lower attachment point of the stay bar using the attached push pins. Note: Ensure the upper attachment point has been installed and the individual in the Man Basket has been cleared before completing this step. Lower worker in the Man Basket to the Main Floor. HAZARDS PRESENT IN EACH JOB STEP CO, FB, SB CO, FB, SB CO, FB, SB 10 Unhook the basket from the crane hook. FB, SB CORRECT AND SAFE PROCEDURES AND PRECAUTIONS -Inspect push pins for rust and wear to ensure a proper fit in the equipment. the crane hook, as there may be a low head clearance. -Examine the lifting cables for abnormal wear. lifting cables on the equipment, as there may be a low head clearance. -Examine the web sling assembly and its lifting ring for abnormal wear. the web sling assembly and its lifting ring, as there may be a low head clearance. -Fall protection must always be worn and secured properly before any movement of the basket occurs. -All radio communication must be isolated between the crane operator and the person inside of the basket. -Confirm no workers are standing under the Man Basket as it is being elevated. -Inspect push pins for rust and wear to ensure a proper fit in the equipment. -Confirm no workers are standing underneath the Man Basket as it is being lowered. the crane hook, as there may be a low head clearance.
