Activity: FUN IN THE SUN Code: BF Students: Reception & Year 2 Max. Number: 100 This activity is designed for Junior Primary students only. The students will have activity with sand play, cricket, soccer, frisbees and general fun play. NOTE: Reception -Year 2 only This is a pre-paid activity with a donation of $20 Southern side of Henley Beach Jetty 8:45 am 9:00 am. Southern side of Henley Beach Jetty (next to Surf Life Saving Club) 12:00 pm. Collect from the southern side of Henley Beach Jetty (next to Surf Lifesaving Club) All students must report to the southern side of Henley Beach Jetty at 8:45 am. Students will then gather for a roll call. Students are expected to be dressed suitably for beach activities, but please be aware that no child will be allowed to swim. Students must participate in all activities and are not permitted to leave the beach area. Students to wear sneakers/shoes no thongs. Sunscreen, hat, light snack and bottled water (disposable or clearly labelled) to be carried in a small back pack, and appropriate clothing for the weather of the day. Follow teacher instructions carefully and DO NOT leave the designated area. Students are to make their own way to and from the Henley Beach Jetty and report to class teacher on arrival.

Activity: MAD MOVIE SESSION Code: MM Students: Years R-2 Max. Number: 100 Note: Years R-2 only This is a pre-paid activity with a minimum donation of $20 Movie: Odeon Star Semaphore Cinemas - 65 Semaphore Road, Semaphore Sherlock Gnomes Gathering point 9:15 am (sharp) at the War Memorial Clock near the Semaphore Jetty. Students will make their way to the cinema by foot and under teacher supervision to see the 10:00am session of the movies. 12:00 noon from the Odeon Cinema Students will be provided with a snack and drink at no extra cost. This is a lock-in activity and students will not be permitted to leave the facility until the designated finishing time. Neat casual dress - appropriate clothing for the weather of the day. General safe and sensible behaviour in such a venue is expected. Students are responsible for their own travel arrangements to the gathering point and from the venue.

Activity: MINI-GOLF & BEACH PLAY Code: MG Students: Years 3 to 6 Max. Number: 80 This activity is open to students from the Primary Campus who are keen to Play some mini-golf and enjoy some play on the sand at West Beach. Students will be provided with a complimentary drink and hot snack. NOTE: Years 3 to 6 only This is a pre-paid activity with a minimum donation of $30 West Beach Mini -Golf 9:30am for roll call at West Beach Mini-Golf for 10.00am start 11:45am (please be prompt) from West Beach Mini-Golf Students can bring their own/additional snacks/water etc (students will be responsible for their own) This is a lock-in activity and no students will be permitted to leave the facility until the designated finishing time. Neat casual dress - appropriate clothing for the weather of the day and for active. General safe and sensible behaviour in such a venue is expected. Students are responsible for their own travel arrangements both to and from the venue.

Activity: FISHING (HENLEY JETTY) Code: FH Students: Years 3 to 6 Max. Number: 50 This activity is open to students from the Primary Campus who are keen fishermen and have access to their own fishing equipment. They should also be relatively skilled and independent when fishing. NOTE: Years 3-6 only This is a pre-paid activity with a minimum donation of $30 Henley Jetty 9:00 am for roll call at Henley Jetty, Henley Beach 11.45am (please be prompt), Henley Jetty, Henley Beach Students must have access to their own fishing gear, including bait and must obey all Fisheries Laws, as designated on the Jetty. They must also be relatively independent as fishermen. Student should also bring snacks/water etc, and must take all rubbish with them when they leave. Do not bring plastic bags. Sunscreen, hat, water etc, generally appropriate clothing for the weather of the day. Students should also take great care re: the safety of other students whilst they are fishing. No student will be permitted to swim. Students are responsible for their own travel arrangements both to and from the venue.

Activity: INDOOR SPORTS (FINDON) - SOCCER Code: IF Students: Years 3 to 6 ONLY Max. Number: 60 NOTE: Years 3-6 only This is a pre-paid activity with a minimum donation of $30 Adelaide Indoor Sports Centre, 316 Findon Road, Kidman Park Students should assemble at the Adelaide Indoor Sports Centre at 8:45 am for a 9:00 am start. Students will be dismissed from Adelaide Indoor Sports Centre at 12 noon. This is a lock-in activity and students will not be permitted to leave the facility until the designated finishing time. Soft drinks and snacks may be purchased at the venue. Students will be expected to participate in as many games as possible. Good sportsmanship is also expected. General safe and sensible behaviour in such a venue is expected. Appropriate footwear and clothing and any protective equipment specific to Indoor Soccer should be worn. The facility supplies all necessary equipment. Students are responsible for their own travel arrangements both to and from the venue.

2018 Mission Action Day Friday 6 April 2018 NOMINATION FORM PRIMARY only Please return this nomination form to your Class Teacher by Friday 9 March 2018. If you have chosen a prepaid activity, the form and money must be returned together. Students are required to return the nomination form and pay the minimum donation to their Class Teacher by the same date Friday 9 March. COMPLETE THE PAYMENT FORM BELOW AND RETURN TO YOUR CLASS TEACHER IN AN ENVELOPE MARKED WITH STUDENT NAME AND CLASS ID. Students are asked to nominate TWO activities, in the event their first activity is either already filled or does not proceed.
