Fit Test Prtcls I. Purpse The purpse f this prgram is t ensure the prtectin f all UW-Green Bay emplyees frm respiratry hazards in the wrk place thrugh prper use f respiratrs. Respiratrs are t be used nly where accepted engineering cntrls are nt feasible t limit expsure t harmful air cntaminants, while engineering cntrls are being installed, r in emergencies. II. Prgram Administratin The UW-Green Bay Safety Manager has respnsibility fr the verall administratin f this prgram and has authrity t make necessary decisins t ensure success f this prgram. Supervisrs wh have emplyees cvered by this prgram will have respnsibility fr assuring emplyee cmpliance with the prgram elements. III. Wh Is Cvered All UW-Green Bay emplyees wh use a respiratr n the jb are cvered by prgram elements listed in part IV. Emplyees wh use respiratrs n the jb are cvered whether they are required t use a respiratry, vluntarily use a respiratr supplied by UW-Green Bay r vluntarily use a respiratr supplied by the emplyee. UWGB Respiratry Plan respiratrs invlves the vluntary use f filtering facepieces (dust masks) are nt cvered by this prgram. Supervisr apprval is required fr vluntary use f respiratr in wrkplace if respiratr is supplied by the emplyee. IV. Prgram Elements A. Respiratrs used in the wrkplace will be selected based n the respiratry hazard(s) t which the wrker is expsed and wrkplace and user factrs that affect respiratr perfrmance and reliability. Only NIOSH apprved respiratrs shall be used. B. All UW-Green Bay emplyees cvered by this prgram will underg a medical evaluatin t determine the emplyee's ability t use a respiratr befre the emplyee is fit tested r required t use the respiratr in the wrkplace. Prevea WrkMed, a licensed health care prvider, will perfrm these medical evaluatins fr UW-Green Bay. Prevea WrkMed will prvide the UW-Green Bay Respiratry Prtectin Prgram administratr a written recmmendatin regarding the emplyee's ability t use the respiratr. Cst f medical evaluatins is the respnsibility f the budgetary unit t which the emplyee is assigned. Additinal medical evaluatins will be dne when: an emplyee reprts medical signs r symptms that are related t ability t use a respiratr Prevea WrkMed, the supervisr r the prgram administratr determines a reevaluatin is needed infrmatin frm the respiratry prtectin prgram, including bservatins made during fit testing and prgram evaluatin, indicates a need fr emplyee reevaluatin a change ccurs in wrkplace cnditins that may result in a substantial increase in the physilgical burden placed n an emplyee C. Befre an emplyee uses a tight-fitting facepiece respiratr in the wrkplace, the emplyee will be fit tested using the same make, mdel, style, and size f respiratr that will be used. Fit testing will be dne prir t initial use f the respiratr, whenever a different respiratr facepiece (size, style, mdel r make) is used, and at least annually thereafter. Fit testing will be dne by the Safety Manager using ne f the OSHA accepted prtcls cntained in Appendix A f 29 CFR 1910.134. D. A tight-fitting face piece respiratr shall nt be wrn by an emplyee wh has (1)facial hair that cmes between the sealing surface f the facepiece and the face r that February 2016 2

Emplyees shall perfrm a user seal check each time they put n a tight-fitting respiratr. Prcedures fr the seal check are fund in Appendix B-1 f 29CFR1910.134. F. Supervisrs will prvide sufficient surveillance f the wrk area s that if there is a change in wrk area cnditins r degree f emplyee expsure r stress that may affect respiratr effectiveness, the supervisr will ensure that emplyee leaves the respiratr use area and reevaluates cnditins and respiratrs befre allwing emplyee t return t the wrk area. G. Maintenance and Care f Respiratrs - The emplyee has primary respnsibility fr care and maintenance f respiratrs issued fr their use. The area supervisr will ensure the emplyee meets this respnsibility. Mandatry cleaning prcedures are fund in Appendix B-2 f 29-CFR 1910.134. Respiratrs issued fr the exclusive use f an emplyee shall be cleaned and disinfected as ften as necessary t be maintained in a sanitary cnditin. Respiratrs issued t mre than ne emplyee shall be cleaned and disinfected befre being wrn by different individuals. Emplyees issued a respiratr fr use in the wrkplace are respnsible fr string the respiratr t prtect it frm damage, cntaminatin, dust, sunlight, extreme temperatures, excessive misture, and damaging chemicals. In additin, the respiratr must be packed r stred t prevent defrmatin f the face piece and exhalatin valve. Emplyees shall inspect respiratrs befre each use and during cleaning. Inspectin will include a check f respiratr functin, tightness f cnnectins, cnditin f the varius parts, pliability f elastmeric parts and signs f deteriratin. Emplyee shall reprt any cnditins that require respiratr replacement t the supervisr. Respiratr shall nt be wrn if there is any questin abut cnditin f the respiratr fr use in the wrkplace. H. Supervisrs will ensure that all filters, cartridges and canisters used in the wrkplace are labeled and clr cded with the NIOSH apprved label and that the label is nt remved and remains legible. I. All emplyees cvered by the Respiratry Prtectin Prgram will receive peridic training. Training will be dne by the Safety Manager. Training will be cnducted as t ensure that emplyees can demnstrate knwledge f the fllwing: why the respiratr is necessary and what the limitatins and capabilities f the respiratr are hw t use the respiratr effectively hw t inspect, put n and remve, use and check the seals f the respiratr prcedures fr maintenance and strage the general requirements f 29 CFR 1910.134 February 2016 3

When changes in wrkplace, type f respiratr, r inadequacies in emplyee's knwledge r use f respiratrs indicate, additinal training will be dne. J. The Safety Manager with supervisr assistance will d a regular prgram evaluatin t ensure that the prgram elements f this prgram are prperly implemented. K. The Safety Manager will maintain prgram recrds including recrds f required medical evaluatins and fit testing V. Designated Respnsibilities A. Safety Manager Update and evaluate UW-Green Bay Respiratry Prtectin Prgram as needed, with assistance f supervisrs. Crdinate scheduling f medical evaluatins with Prevea WrkMed D fit testing as required. Prvide respiratr training fr cvered emplyees Prvide assistance in identifying and evaluating respiratry hazards in the wrkplace. Prvide assistance in respiratr selectin. Maintain required recrds including recrds f medical evaluatins and fit testing. B. Supervisrs Ensure that all emplyees wearing respiratrs in the wrkplace (with the exceptin f emplyees vluntarily using filtering facepieces) are identified and cvered by this prgram Apprve vluntary use f respiratr use in wrkplace if respiratr is supplied by the emplyee when apprpriate criteria are met. Select and assign respiratr use in accrdance with the requirements f 29 CFR 1910.134 Ensure cvered emplyees receive required medical evaluatins. Ensure cvered emplyees attend mandatry training Prvide sufficient surveillance f the wrk area t ensure emplyee cmpliance with all prgram elements February 2016 4

D nt wear a tight-fitting respiratr if facial hair (r ther cnditins) interferes with the sealing surface f the facepiece and face. Clean and disinfect respiratrs using prcedures fund in Appendix B-2 f 29 CFR 1910.134. Stre respiratr in accrdance with established prcedures. Inspect respiratr befre each use and during cleaning. Reprt any cnditins that wuld require respiratr replacement befre next use. Reprt any wrk cnditins that may require change in respiratr selectin and use. Emplyee safety is the primary cncern. Any questins and cncerns an emplyee has n the effectiveness f a chsen respiratr t prtect against harmful air cntaminants shuld be directed t the area supervisr r Safety Manager. February 2016 5
