Sanction #IN16107 Host Club: Maverick Aquatics (MVA) Location: Facilities: Eligibility: McCutcheon High School 4951 Old U.S. 231 Lafayette, IN 47909 Six-lane, 25-yard pool for competition, turbulence control lane lines, Colorado timing system, six-lane scoreboard, touch pads, backup buttons and watches. 202.4.9 C - Pool depth ranges from 11 feet at the start end to 3 and ½ feet at the turn end. No warm-up and cool down area will be available during the meet. 202.4.9 - The competition course has not been certified in accordance with 104.2.2C(4). 202.4.9 E & 205.2.2 & 202.4.9 F Swimmer(s) must be registered with USA Swimming to be accepted into this meet. Age as of shall determine the swimmer s age for the entire meet. Indiana Swimming does not process on- site memberships. Spectators: Per USA Swimming Rules and insurance regulations only athletes, registered coaches, certified officials volunteering to officiate and meet volunteers are allowed on the pool deck. Balcony seating for spectators, parents, and family members overlooks the pool. Competition will be conducted in the timed finals format. MVA reserves the right to use fly-over starts for all events. 8 & Under 25 Yard Events will start in the water from the shallow end of the pool. This meet will be PRE-SEEDED. If the meet becomes full, MVA reserves the right to change to a Positive Sign-In. Clerk of Course will be offered for 8 & Under events only. For all other events, including 12 & Under, there will be no Clerk of the Course. It is the swimmer s responsibility to appear at the correct starting block ready to swim in time for the start of his/her heat. This meet will not be scored. Ribbons will awarded for 1 st 6 th place finishers in 12 & U and 8 & U individual events. 12 & Under Events will be broken down into 11-12, 9-10, and 8 & Under Ribbons will NOT be awarded for relay or 13 & Over events.

Executed release and hold harmless form (included in this packet). Check for payment of all entry fees. Entry Deadline: Entries will be accepted starting: October 23, 2015. Entry deadline will be: November 6, 2015. Deadline will be extended if the meet is not full. Entry Fees & Surcharges: $4.00 per individual event. $6 per relay event. An additional $2.00 Indiana Swimming surcharge will be collected per athlete. Fees must accompany all entries. Deck entries, if accepted, will be double the regular fee. Please make all checks payable to Maverick Aquatics. Refunds: MVA reserves the right to limit entries in an event in order to maintain a reasonable timeline. In any meet sanctioned by Indiana Swimming other than Championship Meets, the host team may limit the number of swimmers in any one or more events to maintain a reasonable length of the meet. In the event that entries are limited, the host club must return the entry fees to the club(s) whose swimmers are not allowed to compete in the specific event(s). Entry Limits: Athletes may compete in a maximum of 4 individual events and 2 relays. Officials: MVA welcomes any officials who would like to work during the meet. Rules: Current USA Swimming and Indiana Swimming rules will govern this meet. 202.5.2 At a sanctioned competitive event, USA Swimming athlete members must be under the supervision of a USA Swimming member coach during warm-up, competition, and warm-down. The Meet Director or Meet Referee shall assist the athlete in making arrangements for such supervision. 202.4.9 D Any swimmer entered in the meet, unaccompanied by a USA Swimming member coach, must be certified by a USA Swimming member coach as being proficient in performing a racing start or must start each race from within the water. It is the responsibility of the swimmer or the swimmer s legal guardian to ensure compliance with this requirement. 202.4.9 H Use of audio or visual recording devices, including a cell phone, is not permitted in changing areas, restrooms or locker rooms. 202.4.9 I Changing into or out of swimsuits other than in locker rooms or other designated areas is prohibited. Facility Information: Coaches MUST constantly display their current USA Swimming coach credential to gain deck access. The meet referee reserves the right to ask for coach credential display and/or deny deck access if coach does not comply or card is no longer valid/current. In swimming competitions, the competitor must wear only one swimsuit in one or two pieces except as provided in USA Swimming rule 205.10.1. All swimsuits shall be made from textile materials. For men, the swimsuit shall not extend above the navel nor below the knees, and for women, shall not cover the neck, extend past the shoulder, nor extend below the knee. As guests of McCutcheon High School, we are expected to treat the property as if it were ours. Swimmers, coaches, officials and spectators should note the following: 1. Ample free parking is available at the school 2. Please enter through door #7 3. McCutcheon is a smoke free campus and no smoking is permitted on the school grounds. 4. Please keep all areas clean. 5. Do not go or let children go to any unauthorized areas (ex. Gyms, Hallways, Etc). Anyone caught abusing the facilities will be asked to leave the meet immediately.
