FY15 ULTIMATE FIGHTING CHAMPIONSHIP (UFC) BOWLING LEAGUE UFC themed bowling league is developed to increase participation, lineage, and excitement in your bowling center! This short season league is designed to fill gaps in the summer targeting all UFC/Mixed Martial Arts (MMA) enthusiasts. The UFC League is exclusive to the Armed Forces and is not available in commercial centers. The only opportunity to obtain a UFC Bowling Ball is by participating in a UFC League hosted by a military bowling center. This event is a limited time offer and the only opportunity to participate will be the summer of 2015. OVERVIEW UFC Bowling League will run May through August The League Champions from 1 CAT B and 1 CAT C bowling center that generate the most participants per lane bed will be awarded with an all-expense paid trip (not to exceed 4 people per center) to an UFC event in Las Vegas, NV (TBD). The trip includes airfare, hotel, and VIP tickets to the event. Additionally, Centers will receive UFC Promotion Kits from Leading Edge Promotions League Participants will receive a custom UFC bowling ball (order forms included in Promotion Kits provided by Leading Edge). Winning Bowling Center (1 CAT B and 1 CAT C) with the most participants per lane bed will be provided with an official UFC shirt for their staff. RULES OF ENGAGEMENT A. LEAGUES Bowling Center managers can choose to have league teams consist of doubles or quads. In the case of a doubles league both the 1 st and 2 nd place teams would earn the trip and in the case of a 4 person team only the League Champions would earn the trip. Teams can be mixed or matched gender, doubles or quad team, 1 person constitutes a legal lineup, pre-bowling permitted, no post bowling, no subs, vacancy score will be 150 with handicap of 56 (80% of the difference of the vacancy average 150 and 220), the league will bowl a minimum of 12 weeks and conclude NLT 31 August 2015. Handicap is based on 100% of 220, absent bowlers use average minus 10 pins, its 3 games per week, 4 points available each week, winning team game is worth 1 point and team total is worth 1 point. UFC BOWLING BALL Recommend charging $15 per person per week for league fees. Bowling ball cost will be amortized over a 15 week period ($6.75 per ball, $8.25 lineage/drilling fees) to cover the cost of the ball. You can run the league as many weeks as you like just be sure to amortize the cost of the ball, $100, over the number of weeks bowled and include that in your fees. Bowling Centers will submit their bowling ball orders to Leading Edge Promotions and pay by P-Card. All order information is provided in your Leading Edge Promotion kit. C. AFSVA WILL FUND AFSVA will fund the cost of the following items: UFC Promotion Kits from Leading Edge Promotions Two (2) grand prize trips for winning teams (1 for CAT B and 1 for CAT C not to exceed 4 persons) to a UFC event in Las Vegas (TBD). The trip includes airfare, hotel, and VIP tickets to the event. Winning bowling center (1 CAT B and 1 CAT C) with the most participants per lane bed will be provided with an official UFC shirt for their staff. To calculate: # of bowlers / # of lanes = participants per lane. D. FINANCE Each grand prize winner is required to fully complete, sign and return an Affidavit of Eligibility and Liability/Publicity Release ("Affidavit/Release"), together with any additional information (such as social security number or resident alien identification number) that may be required by promotion entities. The information provided in potential winner's affidavit/release must be correct and mirror the information provided in winner's registration information or the potential winner will be disqualified. Except where prohibited by law, a potential winner must return all required documents within fourteen calendar days following attempted notification or prize may be forfeited. If a potential winner cannot be contacted, is ineligible, fails to claim a prize or fails to return the properly completed and executed affidavit/release as required, or if the prize notification or prize is returned as undeliverable, prize will be forfeited and awarded to the next highest placing participants. Each Grand Prize Winner will be issued an IRS Form 1099 which documents the value of the prize for tax purposes. AFSVA will send the appropriate tax reporting forms to the winners. Ensure your installation winners understand they are responsible to provide their social security number (SSN) for tax purposes. If SSN is not provided the prizes are invalid and offered to the next 2 P a g e
team. AFSVA reports the value of the prize winnings to the Internal Revenue Service (IRS) as income at the end of the league. ADVERTISING UFC leagues will run May through August; begin advertising this event in March (See 2015 Timeline) UFC Promotion Kits from Leading Edge Promotions will include the following items: 6 Posters (22 x 28 ) 1 Counter Card 1 Standee (31 x 68 ) Order Forms 1 Display Bowling Ball Marketing materials will also be available on the download central on usafservices.com website. Flyer 2015 TIMELINE UFC BOWLING LEAGUE February February (mid) March April May May August DCO (TBD) Leading Edge Promotions ships display ball and marketing kit to field. Begin advertising and promoting UFC Leagues. Recruit league members. UFC League competition. 1 June Orders must be submitted directly to vendor, Leading Edge Promotions, in order to guarantee UFC bowling balls to be delivered by 1 Aug 15. Activities are responsible for payment of bowling balls to Leading Edge Promotions. 15 June Bowling Centers submit league sheets for participant per lane calculations to AFSVA via MyPers Link: https://gum-crm.csd.disa.mil/app/login/redirect/account%2fquestions%2flist 3 August Announce the Cat B and Cat C bowling center with the most participation per lane bed and inform them that their league champion will be receive the UFC prize package. 1 September Bowling Center managers of the centers earning the grand prize for their league will turn in their final league standing sheets to AFSVA via MyPers Link: https://gum-crm.csd.disa.mil/app/login/redirect/account%2fquestions%2flist. In the event of a tie for the championship, teams must bowl a one game tie breaker to determine the championship. 7 September Winning CAT B and CAT C teams announced. 14 September AFSVA will contact winning league team members to process travel arrangements. Finance forms will be provided prior to making travel arrangement. 14 September AFSVA will gather winning center shirt sizes. October AFSVA mail out UFC T-shirts to winning bowling centers. 4 P a g e
