Official Entry Application Entry must be filed on an official entry application, or on-line for those sections which have on-line entry available. Entry by phone will not be accepted. Please photocopy the application and give to your friends to apply, play and have fun. Entry Fee Completed entry application, together with the applicable entry fee, must be received by the PGA Section contact as noted on this application prior to the entry deadline. Make your check payable and Please mail it to the applicable PGA Section office. Application for entry is subject to acceptance by the PGA Section and the PGA of America, the administrator of the Championship. Applicant agrees that, in the event of acceptance, he/she may be rejected at any time and stage of the championship. PGA reserves the right to limit entry, adjust dates, format and other details to fit conditions prevailing at the time of the event. Section Championship Format All boys and girls must have fun and compete in one of the Divisions below. Championship competition is scratch, stroke play. Age Division Ages 12 & Under Ages 13-15 Ages 16-18 Holes Played 9 holes (min) 36 holes (over 2 days) 36 holes (over 2 days) Only boys and girls who compete in the older age division will be eligible to qualify for the National Championship. Any junior who wishes to qualify for the Junior PGA Championship must play 36 holes from the same tees as the 16-18 year old age division and pay the same entry fee. Caddies Players are not allowed to have caddies at any stage of the Championship. Section Champions are awarded on the lowest scratch score in the older age division. 2007 CALLAWAY GOLF PGA JUNIOR SERIES EXEMPTIONS The boy winner (ages 16-18) and overall girl winner from Events 1-6 of the Callaway Golf PGA Junior Series will be exempt into the Junior PGA Championship. In consideration of the applicant s participation in The PGA Section Championships, Qualifiers and The Westfield Junior PGA Championship: 1) the applicant and the parent/guardian hereby release The PGA of America, The PGA Section, PGA Tournament Corporation, Inc., Westfield Insurance, Westfield Group Country Club, host sites of PGA Section events and all of each company s respective officers, directors, employees, agents, subsidiaries and affiliates from any and all liability associated either with acts or omissions of such parties, or with accidents or injuries sustained by the applicant or parent/guardian in connection with the tournaments; 2) the applicant grants and assigns his/her individual media/photo rights respecting participation in the competition, without exception to the sponsors, PGA of America, PGA Tournament Corporation, Inc., and the PGA Section. Applicant information will be used for internal purposes only in conjunction with PGA of America activities and programs. Additionally, The PGA of America will provide event sponsors of the Junior Series and Junior Championship with the names and mailing addresses of its participants. It is a condition of participating in this event that the competitor is not permitted to use a motorized golf car during the event competition. Notwithstanding the foregoing, The PGA of America is committed to conduct golf competitions within the parameters of the Americans With Disabilities Act. Players who request relief under the Americans With Disabilities Act and seek a reasonable accommodation for their disability by the use of a motorized golf car during the event competition shall be required to submit their request in writing to the Special Board on or before the entry deadline date for review and determination.
