LOGAN HIGH SCHOOL CHEERLEADING TRYOUT PACKET 15-16

Dear Prospective Cheer Participant, Congratulations in taking the first step towards trying out for the new 2015-2016 Logan High School Cheer Squad. The following pages contain information regarding the expectations of cheerleaders, the schedule for tryouts, and also requirements that must be met in order to try out. First and foremost, I understand that trying out is a scary experience and you're courageous for taking the first step in taking this packet. My goal is to make this tryout experience fun and rewarding for both you and myself. The atmosphere of the tryouts will not include negative feelings, intimidation or inadequacy. Each participant is equally important and everyone has the same chance of making the squad. you will learn all the material and routines that will be used during tryouts. No private practices will be allowed from the advisor or senior cheerleaders. Practices with others trying out are strongly encouraged. I encourage you to bring materials to take notes regarding the cheers. The squad will be chosen by a combined judge's scores (65%), teacher's evaluations (30%) as well as the copy of your pep rally (5%). Your pep rally will be judged on creativity, completeness, and neatness. The judge's hired have absolutely no relation to Logan High School. Good luck during the week of tryouts. Make sure you read through the packet thoroughly and recognize that there are signature forms that are required to be turned into Cheryl Brown. All pages that must be returned with a parent signature have a star at the top. Online athletic registration must be completed by March 13th.The entire packet (except for teacher evaluations) are due to Cheryl in the office by Friday March 13th at 2:30. Please read this completely and carefully This is your try-out packet. The following pages need to be filled out completely and promptly by you, your parents, and SIX of your teachers you had 3 rd Quarter. (This cannot include Seminary teachers.) In order to try out for the Logan High Cheerleading Squad you must have a 2.5 accumulative academic grade point average, no F's or I's from 3rd Quarter. Your entire tryout packet must be turned in on March 13th no later than 2:30 to Cheryl Brown. (Otherwise your participation in the clinic or tryouts will not take place!) CHEERLEADING SQUAD TRYOUT AGREEMENT FORM AND APPLICATION must be signed by your parents and yourself CHEERLEADING CONSTITUTION is next. Please read this along with your parents/guardian and sign the signature form. The signature form needs to be handed in. Please keep the constitution for your information. Logan High School's Agreement to participate, Athletic Insurance, Authorization/Verification of other insurance, Render medical Treatment Form and Physical Examination forms need to be completed on the on-line athletic registration website. DO NOT PAY ATHLETIC FEE OR WORRY ABOUT THE PHYSICAL. YOUR REGISTRATION WILL SHOW INCOMPLETE BUT I WILL BE ABLE TO SEE THAT EVERYTHING ELSE IS DONE. https://home.registermyathlete.com/login/ PLEASE NOTE THAT ALL STUDENTS WHO PARTICIPATE IN ANY PHYSICAL EXTRACURRICULAR ACTIVITIES AT LOGAN HIGH ARE REQUIRED TO HAVE THESE PARTICIPATION FORMS ON FILE. THESE ARE TO BE ACCOMPANIED WITH THE PHYSICAL EXAMINATIONS FORM DOCUMENTED BY A PHYSICIAN. TEACHER FORMS: Choose SIX teachers from 2 nd Quarter only and ask them to complete the TEACHER EVALUATION FORM. The teachers must be given the forms by March 2nd in order for them to complete and return the evaluation to my box in time for judge's tryout. These scores will be totaled and WILL become a part of the judges' score. (It will be to your advantage to make sure you teachers have received, filled out and returned the forms.) Copy of 2 nd Quarter report card PARENT MEETING: This is a MANDATORY meeting for you and your parents. It will be held in the Media center on February 25th at 6:00pm. All of the above items will be discussed and questions

will be entertained. CLINIC will be held from March 16-18 (3:00-5:30). All sessions will be held in the crimson gym. Mock tryouts will be held the last 30 minutes of clinic on the 18th. TRYOUTS will be held on Thursday March 19th at 3:00pm. Tryouts are closed to the public. Only the Judges, Tabulators, Advisors, and Administration will be allowed. You will need to pay a $10 tryout fee. This will be due with your packet and covers the cost of judges. Finalists for the Cheer Squad will be announced to the candidates Friday March 21st at 2:00 in the auditorium. A letter will be given to all participants at 1:15. If you are selected to be on the new cheer squad you must meet back in the auditorium at 1:30 to get ready for the assembly. YOU WILL BE ALLOWED TO ONLY NOTIFY YOUR PARENTS OF THE RESULTS! The new squad will be introduced to the school in the auditorium following the practice.

LHS CHEER SQUAD 2015-2016 Application * NAME: BIRTHDATE: ADDRESS: GRADES LAST QUARTER (BY HOUR): 1 2 3 4 5 6 7 Cheerleaders must maintain a 2.5 GPA during the school year to remain eligible to participate. (Cheer squad begins immediately following tryouts and selection.) Good citizenship is imperative. It is mandatory that all cheerleaders have good attendance habits. Members must have 90 % attendance to all classes to be eligible. Cheer squad members will adhere to all rules, policies, and regulations of Logan High School, the athletic region, and the Utah High School Activities Association. It is required that ALL MEMBERS attend safety clinic (usually in April) and summer camp as a team. Camp dates and location are still to be determined but most likely will be in June. ALL SCHOOL YEAR PRACTICES ARE MANDATORY!!! During May practice will be held after school form 3:00-4:30. During the following school year practices will be held at 6:00 am through first hour. ALL SUMMER PRACTICES ARE MANDATORY!!! The summer practice schedule is Monday-Thursday during June and August. Two weeks before camp and two weeks before school starts through Homecoming practices will held twice a day. Times are subject to change but most likely will be from 7-10am and 3-5 pm during two a days. Cheer members responsibilities MUST take precedence over all other school activities and jobs. The estimated cost of camp and uniforms is approximately $1500 total. A $300 deposit is due by Thursday, April 10. A physical examination slip must be on file with the advisor and trainer before the first game is held. As with any physical activity, there are risks of injury. Cheerleaders dance, jump, tumble, and stunt. If you are uncomfortable with participating in these activities you are discouraged from trying out. Cheerleaders will be required to attend the following sports: Football, volleyball, basketball (girls and boys) and wrestling. Other sports may be assigned during region and state competitions. My parents and I have read through this together, and understand the foregoing statements. I agree to live by and respect thee statements if I am elected to a position of cheerleader at LHS. Logan Senior High School Cheerleading Squad Constitution Article I- Name The official name of the group is Logan High School Cheerleaders. Article II- Purpose and Activities The main purpose of the group is to promote school spirit develop leadership, good character, responsibility, good citizenship, sportsmanship and to establish crowd control. The main activities will include cheering at assigned sports, conducting pep rallies, participating in halftime and other performances, and initiating other activities to promote school spirit. Article III- Membership The squad may include up to 18 cheerleaders. Article IV- Tryouts and eligibility for membership 1. All prospective members of the cheer squad must have a grade point average of 2.5 from the previous quarter. Each candidate must meet the eligibility requirements by LHS; full schedule- no F's or I's during any part of the school year. Members will be benched during a quarter if they have an F in any class until it is made up and a teacher's note is delivered to the advisor. Tryouts will take place 3 rd Quarter of each year. If any squad members fall below eligibility requirements during the year (cheer squad year begins immediately after selection) they will be removed from the squad 2. Because of the costs, conflicts in the schedules, length of participation, opportunity of participation for a variety of students, ect. It is no permissible for a student to be a member of the Cheer squad and the Hi-Lo's during the same school year. 3. A cheerleader will be allowed to play one other team sport in the fall, winter, or spring. However, if the cheer member participates in a team sport, they will not be allowed to cheer during the season that sport takes place. Once the team sport has ended the cheerleader will then be allowed to cheer for the remainder of the season. 4. Cheerleaders who also play a sport will be allowed to welcome the team onto the field/court, perform at half time and quarter breaks. 5. All prospective member must attend all tryout clinics or meetings conducted by the current senior cheerleaders or the Advisor/coach. 6. The prospected cheerleading squad members will tryout in front of a panel of qualified judges chosen by the advisor/coach. The cheerleading squad will be chosen by accumulation of scores to include- 65% judges scores, 30% teacher evaluations, and 5% pep rally idea. 7. All those trying out must be familiar with the constitution and agree to abide by all of the rules outlined. Must be signed by both the prospected cheerleader and a parent/guardian. 8. All potential participants must have parent permission to try out.

9. Upcoming juniors and seniors are eligible to tryout for the cheerleading squad. 10. Any cheerleader missing 2 or more practices while preparing for a performance may not be allowed to perform with the cheer squad. CHEER MUST COME FIRST! Whether or not the cheerleader performs will be left up to the coach/advisor. Article V- Officers and Duties Officers will be chosen by the advisor/coach. 1. Head Cheerleader(s) A. Help conduct meetings and practice. B. Works closely with the Advisor. C. Will help order emblems, uniforms, shoes, ect. D. Conduct uniform checks before each event. E. Enforce the rules and regulations with the advisor/coach. F. G. Other assignments as needed under the direction of the advisor. 2. Other members may be assigned as needed by the advisor. Article VI- Uniforms 1. School uniforms must comply with the standards set by the school. Purchases and uniforms must be approved and ordered by the advisor. Uniforms, including shoes and socks should be clean and in good condition: hems in place, no holes or missing buttons, clean emblems, ect. 2. Cheer Squad are to wear proper uniforms at all times. No jewelry or glitter is accepted on the floor during games. 3. Uniform checks will be conducted by head cheerleader(s) or advisor before each event. 4. Uniforms will always be worn to school on home game days to help promote school spirit. 5. Failure to pay for all uniforms and cheer costs will result in the entire uniform being returned to the school. Article VIII- Standards Cheer Squad members should observe the following rules and regulations at all times. 1. Cheerleaders should realize their main purpose is crowd control and creating school spirit among the student body. They should have total game awareness at all times and perform in an acceptable manner, keep your personal problems at home, no talking to friends in the crowd and no entertaining other people. Cheers should be practiced at practice, and not at the game. 2. Members should set an example by conducting themselves in a sportsman like manner at games and all other activities. NEVER at any time should an LHS cheerleader make fun of other school's cheerleaders. 3. In the case of smoking, drinking, and engaging in any activity that threatens their reputation or

the reputation of the squad, the member will be automatically removed from the squad. 4. Any members showing disrespect or poor attitude toward the advisor, coach, head cheerleader, or any squad member will be in jeopardy or suspension/ dismissal as determined by the advisor/ coach. 5. For no reason, except illness or extreme emergency, may a member miss a game, pep rally, activity or practice. If a member is not going to be present to an activity, they must contact the advisor ahead of time. CHEER MUST COME FIRST! 6. Hair must be pulled back and off the neck at all times during practice and at the games. Assigned hair accessories must be worn! 7. Additional practices will be called by the Advisor- in which everyone must attend. In case of sickness or injury, members will not be expected to participate, but must attend. 8. Members cannot allow their jobs, outside activities, or other school activities to interfere with cheerleading. Excuses must be passed by advisor for missing an activity or practice before hand. CHEER MUST COME FIRST! 9. No gum chewing when involved in any cheer activities including practices. 10. Members will travel to away sports. They must ride to and from games as a group on the bus. In some cases it may be permissible to ride home from an activity with their parents (not friends, boy friends, or other parents) with written permission 24 hours prior to the event. 11. Members MAY NOT go off at any time alone at home or away games! 12. No cell phones, I pods, or other electronic devices are allowed during practices, games, and pre game activities. Article VIX- Demerit System (see attached demerit page) Members may receive a demerit for the following which relates only to eligibility to cheering a games: -Being late to any games or assigned activities -Not wearing assigned uniform to school and any cheer functions -Not bringing assigned accessories to the game or practices such as poms, shoes, binder, and else assigned. -Any attitude problems toward the advisor/coach or other team members. 1-4 demerits- no penalty 5-9 demerits- 1 game probation 10 demerits- REMOVAL FROM SQUAD These demerits accumulate through the entire time on the cheer squad! (Spring, Summer, Fall and Winter) In certain instances demerits may be made up with an agreement with the coach. GRADES

Grades will be based on all assignments given (pep rally's, posters, community service, etc.) Each assignment partially completed or not completed will result in a demerit. A B C D 0-4 demerits 5-6 demerits 7-9 demerits 10 demerits Each quarter grading demerits will start over to calculate grades. **

Article X- Probation 1. Probation is served by sitting in uniform with the advisor/coach, but not participating with the rest of the group on the floor at any time during the game. Failure to appear at appointed probationary game will be removed from the squad. 2. Any classes missed on the day of a game (excused or un-excused) will result in probation that night. 3. A member who is put on probation three times will be removed from the squad on the third offense. 4. Report card will be shown to the advisor after grades are given out. Each member should meet Logan High eligibility requirements. If unable to do so automatic dismissal will result. 5. Random grade checks will be done throughout the year. Any one currently failing a class will not be allowed to cheer until grade is brought up and a signature from the teacher is obtained. Article XI- Ratification of the constitution Any changes in the constitution will be made by the advisor, or two thirds majority vote. The advisor has veto power. The advisor and principal can make any changes as deemed necessary, whenever. I have read and understand the foregoing statements. I agree to live by and respect this constitution if I am elected to a position of cheerleader at Logan High. I understand that the failure to meet these requirements could result in suspension. Pep Rally Ideas ** (This will be counted for 5% of your score) Make up a pep rally that you think would help school spirit before a game and entertain the student body. Pep Rallies should include everyone in the school. There should be a main theme that is followed throughout the entire assembly. Some examples are: JUMP into Spirit Eighties Army or Halloween Be creative with your own theme! Activities such as games, skits, etc. should be entertaining and tasteful. Anything that does not apply to school rules will not be allowed in the assembly. Remember there are two main season for assemblies, so all sports should be involved. This part of the packet will be counted towards your overall score. TAKE TIME TO TYPE THEM UP! BE SURE TO INCLUDE THE FOLLOWING SOMEWHERE IN YOUR PEP RALLY: NATIONAL ANTHEM (1) CHEER DANCE (1) HILO DANCE (1) CHEERS (1) DECORATIONS (2) THEME (4) RECOGNITION OF SPORTS (5) GAMES (5) ITEMS NEEDED FOR GAMES (5) PUT YOUR NAME ON BACK OF PAPER BE CREATIVE!!!
