The purpse f this standard is t ensure that KUCC emplyees and cntractr persnnel are affrded prtectin in the event f a fall regardless f where, r n what jb, they are wrking. 16.20.2 DEFINITIONS 16.20.2.1 An At-Risk Wrker means a persn wh is required t wrk at six feet r mre abve grund level, and is nt wrking n a standard railed platfrm, r where a risk assessment highlights a danger f falling. 16.20.2.2 A "cmpetent persn" means a persn wh has cmpleted fall prtectin training, testing, and is capable f identifying hazards r dangerus cnditins in the persnal fall system r any cmpnent theref, as well as in their applicatin and use with related equipment. 16.20.2.3 A "qualified persn" means a persn with a recgnized degree r prfessinal certificate and extensive knwledge and experience in the subject field wh is capable f design, analysis, evaluatin and specificatins in the subject wrk, prject, r prduct. An emplyee wh is underging n-the-jb training and wh, in the curse f such training, has demnstrated an ability t perfrm duties safely at his r her level f training and wh is under the direct supervisin f a qualified persn is cnsidered t be a qualified persn fr the perfrmance f thse duties 16.20.2.4 "Anchrage pint" means a secure pint f attachment fr lifelines, lanyards r deceleratin devices that is independent f the means f supprting r suspending the persn. 16.20.2.5 "100% tie-ff" means that a persn must be prperly tied-ff at all times while expsed t a fall hazard. The use f tw lanyards is required if a persn must mve r relcate a tie-ff pint while expsed t a fall hazard t ensure a psitive tie-ff cnnectin at all times during the mve. 16.20.2.6 Fall Prtectin means a persn is secured with an apprved full bdy harness, shck absrbing lanyard, self lcking snap hks and secure anchr pints. 16.20.3 REQUIREMENTS References: OSHA 29 CFR, 1910 & 1926. Whenever a risk assessment highlights a danger f falling r persnnel are required t wrk abve 6 feet a Wrking at Heights Permit must be cmpleted by a cmpetent persn (See Exhibit 16.20.1) 16.20.3.2 Wherever practical, a safe wrking area must be prvided by means f apprved handrails, wrk platfrms, r scafflds t minimize the risk expsure. In such cases a Wrking at Heights permit is nt required. In all ther cases fall prtectin (100% tie-ff) must be used. This includes situatins in which wrk is being carried ut frm an elevating wrk platfrm r manlift. 16.20.3.3 Where verhead wrk is being cnducted, there must be a system in place t prevent tls and equipment frm falling and barricades must be erected arund the wrk area t prtect thers belw frm falling bjects. (See KUCC Safety and Health Standard 16.3) 16.20.3.4 Full bdy safety harnesses are required fr use n KUCC prperty tgether with lanyards that prvide shck-absrbing prtectin in the event f a fall. T ensure that all fall prtectin equipment is prperly tested and certified, all fall prtectin equipment must meet r exceed OSHA and ANSI Z359.1 requirements. Befre each use, the harness and lanyard must be inspected by the user fr cuts, breaks, lse rivets, trn threads, excessive wear, etc. Fall Prtectin equipment that has shwn evidence f excessive wear r mechanical malfunctin r has been expsed t a fall must be remved frm service in a manner t ensure it cannt be used again. At n time shall a harness r lanyard that appears unsafe be used. A prperly sized and apprved full bdy harness must be wrn tight enugh t prevent the wearer frm slipping ut. The lanyard shuld be adjusted s as t restrict any fall t n mre than a six-ft vertical drp and must ensure that in the event f a fall the wrker will nt cntact any lwer level. Lanyards must be attached t a secure anchrage pint. There must be a system fr ensuring that anchrage pints are tested and apprved by a qualified persn t ensure that they are secure and can take the required lad. Anchrage pints shuld, where practical, be abve the head f the at risk wrker, and ensure that in the event f a fall the wrker will neither swing nr cntact any lwer level. Handrails that are nt designed t anchrage pint specificatins may nt be used as an attachment pint. With regard t testing, apprving, and certifying anchrage pints and fall prtectin these functins must be carried ut by a qualified persn r authrity wh has received apprpriate training r wh is suitably certified / qualified. This may include engineering analyses t verify that supprt structures can supprt the necessary lads; rutine inspectins and re-certificatin f anchrage pint cmmensurate with the envirnment and the duty f anchr pint installatins; and sample testing f anchr pints t cnfirm integrity. The results f such inspectins and tests shuld be dcumented. Regular inspectin f the cmplete fall prtectin system, including the anchrage pints, remains the respnsibility f the users f fall restraint systems and cmpetent inspectin / testing persnnel. 16.20.3.5 Wrk platfrms and scafflds must have cmplete flrs, guardrails and te-bards and safe access and egress must be prvided. All scafflds must be installed by cmpetent persns wh fllw the requirements listed in OSHA 29 CFR 1910, 1926 and KUCC Safety and Health Standard 16.7. Persnnel engaged in assembling scafflds shall wear apprved fall prtectin equipment (safety harness and lanyards) when wrking six feet r mre abve grund level. Fall prtectin will be used until wrk platfrms with prper handrails are installed. Safe access (ladders, etc.) t upper levels will be installed as part f the assembly prcess. Revisin N. 4 Standard N 16.20 06/2004 Page 4 f 8 16.20.3.6 A persn may climb r descend a ladder withut Fall Prtectin prvided that they use bth hands and legs, face the ladder, and use ne step at a time. Straight ladders must be lcated s as t prevent slipping and must be lashed r secured. (See KUCC Safety and Health Standard 16.21). Emplyees n ladders wrking six feet r mre abve grund level must wear apprpriate fall prtectin equipment and must be tied ff prir t starting their task. When ascending r descending a ladder, tls must nt be carried by hand. 16.20.3.7 Suspended wrk platfrms and manbaskets shall be used nly when all ther means f access are determined t be less effective. The use f manbaskets is permitted nly in unique wrk situatins where their use results in the least hazards. Jint apprval must be btained fr each use f a manbasket and a Persnnel Histing Permit must be cmpleted and apprved. (See Exhibit 16.20.2) Persnnel wrking frm a suspended platfrm r manbasket must wear apprved fall prtectin equipment and must be tied ff t a lifeline attached separately frm the wrk platfrm, histing cables r attachment, als there must be systems in place t prevent tls and equipment frm falling. 16.20.3.8 Persnnel wrking frm platfrms r buckets n any vehicle-munted manlift, bm, JLG, scissrs lift, etc., shall wear an apprved safety harness with the lanyard attached t the platfrm, r platfrm / bm attachment pint. A persn must be designated t cntrl the wrk platfrm, scissr-lift r man-lift (the basket), wh is trained and cmpetent t d s and qualified as required under lcal regulatins. Where practical, the designated persn shuld be in the basket. There shall als be a system fr ensuring the design, cnstructin, certificatin, maintenance and inspectin f elevating wrk platfrms and man-lifts. (See KUCC Safety and Health Standard 16.5) 16.20.4 TRAINING 16.20.4.1 In rder t effectively implement the fall prtectin prgram, in-depth training is required. There are three types f training: The At-Risk Wrker, The Cmpetent Persn, and The Qualified Persn. All training shall be dcumented. Re-training is required whenever there is a change in the fall prtectin system r equipment. 16.20.5 RESPONSIBILITIES 16.20.5.1 Each individual is respnsible fr using adequate fall prtectin equipment as described in this standard. 16.20.5.2 The Area Supervisr must: Cmplete a Wrking at Heights Permit whenever a risk assessment highlights a danger f falling r persnnel are required t wrk abve 6 feet. Cmpleted all prvisins set ut in Persnnel Histing Permit prir t using a suspended wrk platfrms r manbaskets. 16.20.5.2 Each Area Manager is respnsible t ensure that the prvisins f this standard are met including ensuring: There is a practice in place fr prperly identifying and apprving anchr pints. That a cmpetent persn prvides fall prtectin training fr all persnnel expsed t a fall hazard. That there is a system in place fr prviding emergency rescue fr fall victims. Exhibit 16.20.2 KENNECOTT UTAH COPPER CORPORATION PERSONNEL HOISTING PERMIT (Standard 16.20) Permit will be psted at wrk site until wrk is cmplete r a new permit is issued. This permit must be reviewed every shift and reissued if a change in cnditins r wrk scpe has ccurred.
