Remove wellhead flange and install manual master valves and flow crosses on University Land 09#1001H, 1002H and 1003H. 09#1001H to be treated first in conjunction with 09#1004H on adjacent pad. Run GE 5-1/2" 10k frac sleeve in wellhead; run in lock down screws and torque gland nuts to secure. Replace blind flange and torque to secure well in preparation for bond log run. Activity Date: 9/11/2014 Days on Compl: 3 TD (TVD): 0' Discussed overhead loads with crane, pinch points, and using good communication and hand signals between crane operator and personnel at wellhead. Rig up crane and flange up to wellhead. RIH as far into the curve as possible to a depth of 8562', approx. 73 degrees. Log out from where tool stopped at 8562'. Tied into short joint at 7439'.Top of cement at approximately 714'. Rig down and over to #1002. CBL completed. Activity Date: 9/12/2014 Days on Compl: 4 TD (TVD): 0' O-Tex on location with two pumps and crew. RU to frac tanks and 3"" tie-in on flow cross. Wait on roustabout crew to complete double-valving annular valves on 1003H before pressuring up and pumping. Break down shoes on 09#1001H and 09#1002H. Pressure test iron to 8500psi; good. Open wing valves and test flow cross against master valve to 8500psi; good. Open well and walk rate up to 5bpm. Fill void in casing and pressure up. Saw breakover at 5680psi and dropped off, indicating shoe breakdown. Slowly walk rate up, pressure immediately climbed - pumps pressured out at 11bpm (kicked out at 8200psi). Unable to stabilize pressure higher than 6.5bpm, pressure sitting at 7200psi. Flush 350bbls to clear wellbore. Shut down pumps; pressure bled off to 2950psi before stabilizing. Shut in well. RD O-Tex pumps and iron. Well shut in and secure. Prep for tractor run to set plug and fire first stage guns. Activity Date: 9/13/2014 Days on Compl: 5 TD (TVD): 0' Page 1 of 19

Discussed trapped pressure, making sure pressure is bled off before opening or closing valves. Also, discussed multiple third party contractors working in the same area at once, and making sure they are clear of one another's working area unless absolutely necessary and communicated. Rig up lubricator, rehead wireline, test wireline tractors, stab onto the well, pressure test lubricator, equalize to well, and run in hole with wireline tractor, ccl, 5 guns, and bridge plug. Tie into short joint at 7439', and trip in until unable to get any further into the curve, which was 8619'. Turn on tractor and trip in until tool stopped and stalled out at 12760'. Pulled up 25', turned tractor on and began moving again. Tool made it to 12936' and stalled out again. We were able to pull up 25', then start going down again. Tractored down to 13080', tractored stalled. This time, unable to pull up at 1500 #'s over line weight. Turned tractor back on to attempt to move tool down and immediately tool stalled out without moving. Attempted surging the well under tension 1500 #'s over to free wireline. Still unable to move tool. Discussed max safe pull with wireline engineer, and decided to pull up that that. Tool pulled free at 3200 #'s, which was 2200 #'s over line weight. Tool drug up hole with significant resistance for approximately 1500' before beginning to smoothly. It was decided to POOH and forgo attempting another tractor run due to hole conditions. POOH. Rig down wireline tractors and guns. Attempted to establish injection again by pumping fresh water with the mindset of pumping acid for formation breakdown to obtain necessary injection rate for pump down. Unable to inject at a rate of 5 bpm without pumps kicking out. Made 3 unsuccessful attempts, pumping a total of 81 bbls fresh water. Well shut in, lining up coil tubing. As a result of the leak two joints of treating iron were replaced, and re-tested to 8000psi. (Good) During the RU/RD 09-1003 was opened up to the OT tank at a rate of 7BPM for 350bbls. An injection test of 450bbls at 20BPM of fresh water was completed to ensure the formation was still broken down.
