1 11/17/ /18/ /19/2011 AFE dated 11/18/ pull tbg out of hole and make a mill run. Run packer on tbg and attempt to reestablish production from Devonian Formation. MIRU Great Basin Pulling Unit (Operator - Baldamar). ND plunger lift equipment. ND Wellhead & install BOP. Dig & line 175 bbl workover pit. Pick up on tubing & had approx 66,000 lbs string weight. SWI & SDON. Prep to POH with tbg in am. DWC - $5600 CWC - $5600 POH with 349 jts of 2 3/8 tbg (tally out). Recovered SN with seating cups off standing valve and tbg stop. Jt below SN had numerous corrosion holes. (1 to 2 in diameter). Unload and assemble 6 1/8 OD string mill and prep to RBIH in am. Close blind rams and SWI. SDON. DWC - $4600 CWC - $10,200 RBIH with 6 1/8 OD string mill. Tag at 4,282. Rotate and work mill thru and continue in hole. Tag again at 4316 and rotate and work mill to Continue RIH unobstructed. Tag up on liner top at 10,992. RU kill truck and reverse circulate 7 casing with 90 bbls 2% KCL wtr (broke circulation after pumping 55 bbls). Did not circulate anything of note out of hole. RD kill truck & POH. Had intermittent drag for the first 1800 off bottom. At 4,337 had drag again and had to work out of hole. At approx 4,282 freed up and continued out of hole without problems. SWI & SDOD. Prep to make another run with mill Monday am and then run PKR & RBP to test casing. 2 11/23/ /24/2011 DWC- $6,200 CWC - $22,550 Pick up 4 ½ OD Arrow SET II PKR & RIH on 2 3/8 tbg (one jt of tbg with mule shoe guide below packer & SN above packer). Worked PKR thru tight spot at 4285 with only minor effort. Continue in hole and set PKR at 11,058 with EOT at (10,000 lbs compression on PKR). RU swab, bypass production separator and commence swabbing into fiberglass water tank. IFL = surface. Made 14 swab runs. FFL = 600. Total recovery in 4 hours = 45 bbls wtr. SWI, SDON. Prep to remove BOP & NU wellhead in am and resume swabbing. DWC - $6550 CWC - $29,100 SITP = 45 psig. ND BOPs, NU wellhead. Land tbg on dount with 2,000# tension on 2 3/8 tbg string. Resume swabbing. IFL = 500. Made 26 swab runs, recovering 117 bbls. Total recovery 172 bbls. Checked backside & had no sucking or blowing suggesting PKR holding. FFL = 900. SWI & SD for Thanksgiving Weekend. DWC - $4050 CWC - $33,150 11/25/ SDFWE 11/26/ SDFWE 11/27/ SDFWE 11/28/ SDFWE 11/29/2011 SITP = 0 psig. RIH with swab, 1,800. Made 13 swab runs lifting approximately 1300 each run. 2,000. Daily recovery = 68 BW. Total recovery = 240 BW. SWI, RDPU. Wait on swab unit. Note: recovery = 100% black dirty water. 12/02/2011 DWC - $3025 CWC - $36,175 SITP = 45 psig. MI&RI Hamilton Well Service swab unit. RU flowline & start swabbing. IFL = 2,500, pulled from 4,000 & recovered 100% black water. Made a total of 10 runs & recovered approx 57 BW. EFL = 2,700. The casing is standing full and it appears that there is a small water flow. Installed gauge in casing but pressure was TSTM. Will install a smaller gauge in am in attempt to get SICP reading. DWC - $4300 CWC - $56,175 SITP = 0 psig. 4,200. Made 20 swab runs, lifting 1,500 on each run. FFL = Recovered 88 bbls. 100% black water. SWI, SDON. Prep to continue swabbing. Continued RIH to 6,200 unobstructed. POOH & LD string mill. PU polish bore mills, RIH to 4,000 & left swinging. Unload, rack, drift, clean & strap 4 ½ casing. SI & SDFD. RU Express Casing Crew, PU seal assemble and orifice collar, RIH with 4 ½ P-110 casing. Tagged 4,225 worked down 4,255, 4,295, & 4,300. Unable to work casing down any further. LD 3 jts casing & SDFD. 02/26/2012 DWC - $15,000 CWC - $130, Pull & lay down 4 ½. Found O.D. of seal assembly heavily marked. DWC - $15,000 CWC - $145,000 02/27/2012 SDFWE. Thoughts: In addition to bad & or parted 7 this also may be an area of crooked hole. I recommend we clean the area up again with string mill then run csg inspection log. Then possibly recover 7 & either repair & rerun or dress 7 stub & run 4 ½. 02/28/2012 WOO. RIH with open ended tubing, work tubing past bad 7 to 4,378 where we tagged up. PUH above bad 7 & worked back down 2 more times and were unable to work past 4,378. LD 135 jnts tubing. SI & SDFD. 02/29/2012 DWC - $157,000 (includes cost of 4-1/2 ) CWC - $302,000 SICP = 150 psig, bled off pressure (all water). ND BOP & C section, NU old WH & BOP. Work mills down to 4,288 & start milling. Mill to 4,299, circulate clean & SDFD. Daily Cost $ 8,000 CWC - $320,000 04/23/2012 ( ) and ( ) SDFWE 04/24/2012 ( ) Opened well - 0 psi. WIH with 6" lead impression block and tagged up at 4218'. POOH with block. Half-moon impression of parted 7" casing on block. WIH with 5.5 OD flat bottom mill. Tagged parted csg at 4218, lowered down and tagged at 4293' parted csg. Rotated through and tagged at 4313'. Pulled up dragging 20K over weight and came free at 4293'. It appears that we are outside casing at 4293'. POOH with 5.5 mill. WIH with used 6-1/8 rock bit, tagged parted csg at 4218' and rotated through. RU Swivel. Tagged at 4293' with 10K down and attempted to rotate into casing. Went down and tagged at 4310, torqueing up and bouncing up and down. Again, appears to be outside casing. Picked up several times and tagged at 4293', but could not rotate inside 7" casing. Kept going outside casing to 4310', dragging 20K to pull up into casing at 4293'. RD Swivel and POOH with 6-1/8 bit. Closed well in for night. 04/25/2012 DWC - $16,800 CWC - $ 336,800 ( ) Opened well 300 psi on 9-5/8 casing. Bled to 0 psi, flowing a little fresh water. ND 11" 5K casing head for 7 casing. RU rotary csg jacks and speared into 7" csg with Weatherford spear grapple. Pulled 200K on spear and 7" csg. Pulled casing free from hanger weighing 120K. Pulled up 5 and sat on slips. RD csg jacks and LD Spear. NU 11" 5K Weatherford Hydril. LD 2 jts of 7" casing. Elevators came open while laying down second jt causing the welder #1_reports.doc 7
8 to jump off the 5 rig floor. Sent the welder to doctor and he is ok. RU Premian Casing crew with tongs and elevator and slips but could not back out csg. RD 7" csg tongs. Shut Well in. Waiting on 10-3/4 high torque tongs. 04/26/ /27/ /28/2012 DWC - $20,100 CWC - $ 356,900 ( ) Opened well - 0 psi and flowing a little fresh water. RU Permian High Torque Tongs. POOH Laying down 7" casing. LD 95 jts of casing, parted at 4219'. Pipe extremely tight and had to cut 10 jts with a cutting torch. Pipe Weight 120K. It also had split collar on bottom jt of casing. Lost 5 bow springs and 1/2 centralizer in hole. Bottom 5 jts are pitted on OD and ID. Closed well in. DWC - $17,300 CWC - $ 374,200 ( ) Opened well - 0 psi. WIH with 4-3/4 OD tapered mill, 1 jt of 2-3/8 tbg, 6-1/8 OD string mill and 2-3/8 tbg. Tagged 7" csg at 4220'. Rotated into csg and tagged at 4285'. RU swivel and stripper and washed down to 4335' and torqueing up and dragging 20K over weight. Seems to be outside csg. POOH with mills. WIH with 7" csg spear with grapple and tagged at 4220'. Stung into csg to 4223' and pulled 2K over weight. POOH with spear and 61' of 7" csg parted 15' below collar. Csg pitted on OD and ID. WIH with same tapered mill and sting mill and 2-3/8 tbg to 3900'. 9-5/8 surface pipe at 3955'. Closed well in DWC - $15,300 CWC - $389,500 ( ) Opened well - 0 psi. WIH with 4-3/4 tapered mill, 1 jt of 2-3/8 tbg and 6-1/8 OD string mill and 2-3/8 tbg. Tagged at top of 7" csg at 4282'. Rotated tagged outside csg at 4335'. Pulled up. RU swivel and tagged at 4282'. Rotated inside. WIH to 4535'. Pulled up to 4250'. WIH and tagged at 4282' but could not get back in csg. POOH with mills. WIH with 7" csg spear with grapple and 2-3/8 tbg. Tagged csg at 4282'. Speared into csg to 4285'. Pulled up to 50K on csg. Speared pulled free. Pulled out of 7" csg and string not parted. POOH with Spear. Bent 2-3/8 tbg above tapered mill. Rotated inside csg. WIH to 7050' with 175 jts of 4-1/2 csg. Closed well in. DWC - $14,500 CWC $436,500 (5-3-12) Opened well - 0 psi. WIH picking up 101 jts of 4-1/2 11.6# P-110 LTC csg to 10946' and circulated down 4-1/2 and out 9-5/8 csg. Stung into PBR at 10953' to 10963'. Tested seals to 1000 psi. Bled to 0 psi. 10 P-110 pup jt and loaded BJ Cement Bulk Unit with 1200 sks of Class H cmt. RU BJ Cement Pump Truck and pumped 20 bbls of fresh water spacer. Mixed and pumped 1145 sks of 16.3 cmt, 220 bbls. Dropped wiper plug and pumped bbls of water behind plug. Did not bump wiper plug over displaced.5 bbls. Stung into PBR with 5-1/4 seals with 3500 psi, bled to 0 psi and had no flow back. Pressured up to 1000 psi and closed well in. Stung into PBR at 10,953 to 10,963' with 10K down on seals. DWC - $95,260 CWC - $545,960 05/06-05/07/2012 Shut down for the weekend. 05/08/2012 (5-7-12) Opened well - 0 psi on 4-1/2 csg and 9-5/8 csg. RD BJ Cementers. ND Weatherford 11" 5K Hydril. Set 4-1/2 csg slips with 100K on slips. Cut csg off with welder and beveled with grinder. Installed 11" X 7-1/16 csg head and successfully tested to 5000 psi. NU 7-1/16 dual Graco 5K BOPs. RU floor and tongs. WIH with 3-7/8 4 bladed concaved junk mill and 166 stands of 2-3/8 tbg. Tagged cement at 10895'. Drilled to plug and baffled orifice sub at 10930'. Drilled out of cement at 11025' and washed down to retrievable plug at 11070'. Circulated out rubber, cement and sand off top of plug. Circulated clean. Pulled up to 11030'. Closed well in. 05/09/2012 DWC - $25,000 CWC - $570,960 (5-8-12) Opened well - 0 psi. Tagged plug at 11070'. No fill. RD swivel and stripper. POOH with 169 Stands of 2-3/8 tbg and 3-7/8 bladed mill. WIH with 3-3/4 OD retrieving tool for receivable bridge plug and 169 Stands of 2-3/8 tbg. RU swivel and stripper. Rotated over top of plug and circulated out sand, metal and cement off plug. Latched onto plug, pulled 20K over weight, opened by-pass and well on vacuum. Pulled plug loose, went down to 11090'. RD swivel and stripper. Plug dragging through csg collars. Pulled 15 stands. Closed well in. Loaded up Hydril BOPs and closing unit and sent to Weatherford. DWC - $11,400 CWC - $582,360 05/10/2012 (5-9-12) Opened well - 0 psi.
