Dance One Studios Inc. Hold Harmless Agreement In consideration for my child being permitted to participate in the Dance One Studio Inc. program (referred to as the Studio ), I/We, parent(s) and/or legal guardian(s) of, agree to the following provisions: Acknowledgment of Risks I/We understand that there are numerous risks and hazards associated with participating in dance activities, including those risks present during classes and activities, on the premises before, during, and after various classes and events, as well as in travel to and from the Studio and various venues of performance. Some of the specific risks include the placement of unusual stresses on the body, falls and tripping which may lead to accidents resulting in, but not limited to, mild to severe bodily injury. I/We understand that the Studio cannot be responsible for any injuries or damages experienced by my/our child during her/his participation in such activities. I/We affirm that my/our child is covered by adequate medical insurance provided by myself and I certify that my/our child is physically fit to participate in the dance activities. Medical Care In an emergency situation, when parental permission is not available, I/We herby grant and give my/our permission for a staff member at the Studio to seek emergency medical treatment for my/our child. In my/our absence or inability to communicate with emergency and hospital personnel, I/We hereby grant the Studio authority to release for the purposes of providing medical treatment, my/our child to the care of medical personnel or physicians as the Studio determines as reasonably appropriate. I further authorize the treating physician to provide all proper medical treatment and care, hospitalization, surgery, medication, injections and anesthesia for my/our child, in the event such treatment and care is required. Media Relations I/We understand that the studio obtains media attention (television, news and all various print media resources) in order to promote the Studio. I/We understand that my/our help is necessary to further this goal. By signing this Agreement and Release, I/We consent and give permission to the Studio, and those acting under its authority, to use the name or likeness of my child in newspaper articles, print ads, television, radio broadcasts, videos, DVD, Dance One Studios website, as well as any other media in connection with activities undertaken under the

Dear Parents/Guardians, We would like to extend a warm welcome to the new and returning members of the 2018-2019 Dance One Studios family. As most of you know Dance One Studios offers instruction in all techniques and varieties of dance, both group and private instruction, in a relaxed, safe, and clean environment. Through creative activity and valuable dance experiences we will foster artistic expression and provide your child with a meaningful extracurricular activity. For serious students, the training they receive will provide a solid foundation, as well as, exposure to the cultural and historical evolution, terminology, scientific basis and theory; a complete dance education. At Dance One Studios, we take the time to evaluate each and every student so that their class placement will improve confidence and ensure their personal and technical growth. Whether it is participation in an age-based or level-based class, for all students, their year at Dance One Studios will result in personal growth in a fun and supportive atmosphere. In addition to our regularly scheduled classes, there are many exciting activities to broaden your child s education at Dance One Studios. We encourage all to participate in these great events: Dance One Contemporary Ballet Company - This pre-professional company explores a fusion of classical ballet and modern dance through various performance opportunities. DOCBT has performed throughout the Tri-County area, as well as, at Carnegie Hall, Lincoln Center, the Kennedy Center for the Performing Arts, and the National Cherry Blossom Festival. DOCBT II, the apprentice company, has performed at Hershey Park, varied local venues, and performs in one piece in the company concert. DOCBT II is required to participate in all fundraisers. Auditions for both companies will be held Saturday, September 22nd at 1:00 pm for students at the Ballet III level and above. You must be enrolled in a ballet and modern class to audition. DO TAP (Dance One Tap Company) - Experience rhythm and Broadway tap through numerous performance opportunities. DO TAP is the only pre-professional tap company in the area and has performed throughout the Tri-County area, as well as at Carnegie Hall, the Philadelphia Tap Festival, the Carnival Cruise line, Lehigh Valley Dance Exchange, and at Lincoln Center. Auditions will be held for Advanced Beginner Tap level students and above on Friday, September 21st at 4:00 pm. You must be enrolled in a tap and jazz class to audition. DO CREW (Dance One Hip Hop Company) - This pre-professional company ignites as they perform the latest street dance and urban funk influenced by today s hottest video choreographers. DO CREW has performed throughout the Tri-County area, as well as, at Carnegie Hall, Lincoln Center, the National Cherry Blossom Festival, and in a RAW performance in New York City. Auditions will be held Saturday, September 22 nd, at 3:00 pm. You must be enrolled in either an Intermediate or Advanced Hip Hop class to audition. DC s Army- A chance to experience a taste of the energy, camaraderie, and teamwork of what it would be like to be in Do Crew, our senior Hip Hop Company. Highlights of the year would include a guest performance at the Company Concert in March and performing in local events and parades. No audition necessary,

commitment of two rehearsals per month; must be enrolled in Kid s or Beginner Hip Hop to participate. Dancin in Disney - Dance One has made a commitment to provide a lasting memory by Dancing in Disney every other year. We will perform in Disneyworld Spring, 2019- different class levels have been invited to perform next year-check the schedule to see if your child qualifies! Artists in Residence and Master Classes - Master teachers and choreographers from all areas of technique are invited to share their expertise with Dance One students. Dance One Studios began this phase of growth with the very successful Darrin Henson workshop (choreographer for N Sync), Ruth Andrien (former soloist with the Paul Taylor Dance Company), Jeff Shade (dance captain under Bob Fosse, original cast of Sweet Charity), Patricia Brown (former soloist - Joffrey Ballet), jazz legend Luigi, Finis Jhung (NYC Ballet), Dormeisha Sumbry- Edwards (Bring in Da Noise - Bring in Da Funk), Stephen Vaughn of Open Category Dance Co. (formerly of Parson Dance Co.) and Daniel Mayo of Ballet X. Special Classes and Lectures - broaden your child s knowledge and appreciation of dance with these special events; college preparatory lecture, history of dance slide presentation, and somatic studies to name a few. Dance One Studios Terms and Conditions 2018-2019 Each child is expected to dress appropriately for dance class. Any leotard and tights/capri-length yoga pants are acceptable for technique classes with hair pulled back, with the exception of ballet class where everyone must wear black, pink, or white leotards and tights (in any combination). Additionally, all female ballet students must have their hair in a tight bun. No child will be permitted to wear pants (jazz pants included) or street clothes in technique classes (we have had students injure themselves tripping on pant legs!). NO EXCEPTIONS. If your child forgets to bring appropriate dance gear, he/she will have to observe class. Proper footwear is also required as designated below: Ballet- pink ballet shoes, Male; black ballet slippers Pointe- pink pointe shoes (properly fitted) (for Pre-Pointe: please make sure you have your pointe shoes for the first day of classes. Instructions on sewing ribbons and elastic will be given during the first class- please do not sew them on prior to the first class), hair in bun Creative Kids- pink ballet slippers, tan tap shoes (Mary Jane- strap style), leotard (with or without an attached skirt), tights Combination Classes- * All 5-7 classes: pink ballet slippers, tan tap shoes (Mary Jane style), leotard, tights & shorts or capri leggings. * All 7-9 classes: tan oxford (ties) jazz shoes (not available at Curtain Call), tan tap shoes (Mary Jane style), leotard, tights & shorts or capri leggings. * All 9-11: tan oxford (ties) jazz shoes (not available at Curtain Call), tan tap shoes (oxford style), leotard, tights & shorts or capri leggings. Jazz- split sole tan jazz boots without ties, leotard, tights & shorts or capri yoga pants, hair pulled back Tap- black oxford tap shoes, leotard, tights, & shorts or capri yoga pants, hair pulled back Modern- barefoot, leotard, tights & shorts or capri yoga pants, hair pulled back Hip Hop- comfortable clothing, no jeans or long pants (cut-off sweat pants or yoga pantsnothing longer than calf length- harem pants accepted), BLACK hip hop sneakers only (other sneakers of any kind or color will not be permitted), hair pulled back, no sweatshirts or hoodies African- barefoot, leotard, tights & shorts or capri yoga pants, hair pulled back Adult Classes - comfortable clothing, shoes - appropriate to technique Attendance and punctuality are expected for all scheduled classes. Dance One Studios will be closed on the following dates: November 21 st - Nov 25 th, 2018 Thanksgiving (classes resume Nov 26 th ) December 19 th - January 1 st, 2019 Winter Recess (classes resume Jan 2 nd ) January 21 st, 2019 Martin Luther King Day April 15 th April 21 st, 2019 Spring Recess (classes resume April 22 nd ) May 27 th, 2019 Memorial Day July 4 th, 2019 Fourth of July

Emails will be sent and facebook and Instagram messages will be posted for closings due to inclement weather. We will not make-up classes unless snow days become excessive (more than 2 snow days) for a particular class because we have included two snow days per class when determining your yearly tuition. If class is cancelled on the teacher s behalf there will be a make-up class scheduled, otherwise there are no make-up classes. We have an open door policy regarding parental observation. Please feel free to observe any class from the waiting area. Once a student reaches enrollment in a one hour level-based technique class they will move up to the next level only upon recommendation of their teacher. Exceptions can be made at the sole discretion of the Director. All intermediate and advanced level technique classes are by special invitation only after a class evaluation. Any student who would like to enroll in a private class must also enroll in a regular class of the same technique. Private lessons are allowed 2 absences that can be rescheduled for a make-up only if we are aware of their absence ahead of time. If a student does not contact us prior to their absence, that private cannot be made up. After two absences of any kind, there will be no make-ups. No exceptions! Any student enrolled in Pointe class must also be enrolled in a ballet class. Any student enrolled in Advanced Ballet must also be enrolled in the Ballet Tech class. We reserve the right to cancel or move any class with an enrollment less than five students. Students should be dropped off no earlier than 15 minutes before their first class and picked up no later than 10 minutes after their last class. The tuition at Dance One Studios is a yearly tuition. There is no per class fee, nor monthly class fee. Tuition is not based on how many classes are given per month. For your convenience we have provided you with a payment book where you can make ten, five, bi-annual, or annual payments. Please mail the appropriate coupon so it is received on time or place it in an envelope with the appropriate coupon and drop it into the drop box located at the reception desk. There will be a $5.00 late fee if the payment is delinquent. A student will not be able to participate in a class when payment is three weeks past due. Class participation may resume when past due payments and late charges, as well as the next payment are paid in full. For clerical purposes please make sure to record your check number and dancer s name on each tuition coupon. Visa and MasterCard are now accepted and can be scheduled to automatically pull each month when a payment is due. Cancelled checks will serve as your receipt unless otherwise requested. There will be a $25.00 fee for returned checks. Post-dated checks are not accepted. Full payment for recital costumes is required during the month of November, 2018. Costume payments must be made by check or cash (no Visa or MasterCard). Our recital is tentatively scheduled for the end of May 2018. Tickets are $18 each and if you have a child in both recitals you ll get a set of tickets for the second recital for free. Everyone must purchase a ticket with the

exception of a Dance One Studios dancer. The Tiny Tots Revue is for all Creative Kids classes through 7-9 yr old-1st year, Mini Hip Hop, Kid s Hip Hop, Beg Ballet and will perform the following: Creative Kids I and II (tap dance) 5-7, 1st yr (tap dance) 5-7, 2nd yr (jazz dance) 7-9, 1st yr (tap dance) Mini Hip Hop Kid s Hip Hop Beg Ballet The Main Recital begins with the 7-9 yr old-2nd yr (jazz dance), Beginner Hip Hop, Ballet I through all advanced level classes It is our hope that these terms and conditions will contribute to a better experience for our students and parents.
