SITE SAFETY PLAN AND NEW EMPLOYEE SAFETY ORIENTATION LOCATION: TSMF REVISED: 09-11-09 The following summarizes the orientation requirements for new employees at the facility. This plan augments the company safety program for conditions at this facility and does not replace the detailed safety training. EMERGENCY ACTION PLAN: Procedures S635 Emergency Procedures Policy and S620 Emergency Action Plan Procedure cover the main aspects of the emergency policy and is available for review. Emergency Contact Information is posted Main door to shop Forman s office on way to time clock Emergency Evacuation Map and Egress Paths maps are posted at the doors to the main office and all entrances to the main shop. Please review and become familiar. These paths should always be kept clear. Rally Points for Emergency at the north section of the parking lot in front of the main office First Aid & CPR -All COMPANY supervisors are trained in First Aid and CPR. The training is given to stay in compliance with OSHA standards of a quick response for medical care. First Aid Kits & Eye Wash Facility First aid kits and mobile eye wash supplies are located on site at the entrance to the shop Forman s office. Fire Prevention Program Fire extinguishers are located by the restroom in the main office, at four evenly spaced locations in the shop SITE SPECIFIC SAFETY RULES: The following rules are specific to this fixed location and in addition to the COMPANY Health, Safety and Environmental Manual. Training All COMPANY employees receive comprehensive safety training. The Site Specific Safety Training Summary covers the training schedule at the site. In addition, the overall training program is provided in S055 Employee Safety Training Program Summary. Please contact your supervisor if you would like to review a written copy of the company s Health, Safety and Environmental Manual. Site PPE Requirements Note that in addition to the company s regular PPE requirements, the following requirements apply at this facility: o Hard hats are NOT required o Steel Toed shoes/boots are NOT required o All Grinding will be conducted with double protection (face shield in addition to safety glasses) o All weld slag chipping will be performed with double protection (either a face shield or a welding hood with a flip lens). o Hearing Protection is required in all main shop areas COMPANY supplies most PPE to the employees (ie, safety harness, lanyard, gloves, hard hats, hearing protection, etc.) Fall Protection employees should note the following site rules regarding fall protection: o Employees shall wear full-body harness and lanyard when working in any JLGs, manlifts or scissor lifts. Equipment Notes o Overhead cranes are pendant controlled 5 ton capacity. Be aware of overhead lifting around you. Employees are never allowed to ride hooks! o This facility operates rolling stock consisting of two propane forklifts, two transport trailers. Employees need to be aware of their surroundings as noise levels and activity can cause you to tune out hazards. Near Miss and Incident Investigations All near misses and incidents are to be reported to your supervisor for proper investigation. Any injuries should be reported immediately to your supervisor. Visitor Identification visitors may be identified by the blinking light on their beanie. Subcontractors all subcontractors working in a Company facility shall have a Company employee assigned as the responsible liaison while on site. The liaison is responsible for ensuring that subcontractors undergo a review of their safety program in addition to basic site orientation prior to work. It is expected that all subcontractors will comply with Company Safety Rules and conduct regular tool box meetings as well as JSA s where applicable. In addition, the liaison shall serve as the point of contact regarding any safety issues or violations that occur. Building/Facility Notes o Walking Paths Safe areas for travel around the shop are designated by yellow lines on the floor. Please keep travel within these areas for safety purposes. o Out-of-Bounds NA o Multi-floor Hazards: be aware of workers who may be above you as you enter the maintenance building 2.0 SAFETY COVERAGE & ACCOUNTABILITY COMPANY believes that ALL personnel on site are responsible for their safety as well as those they work with and around. If an employee witnesses an unsafe act or condition, they should handle it as follows: If another employee is involved, notify the employee of the unsafe act or condition. If this does not resolve the condition, notify their supervisor Visitors and Contractors Working on Site All visitors and contractors working on site are expected to follow company safety policies and procedures. All visitors and contractors on site shall have a company representative assigned to them and responsible for ensuring that work is performed safely and per company safety policy. Should a visitor or contract be notified by an employee of a potential safety issue which is not addressed, the employee should notify their supervisor who may in-turn notify the company representative assigned to investigate the issue. 3.0 EQUIPMENT OPERATOR CERTIFICATIONS & TRAINING Only properly trained employees are authorized to operate heavy machinery and equipment. This includes: Overhead Cranes (O-OH) Forklifts (O-Fork) Shuttlelifts and Marine Lifts (O-SHUTT) JLG Manlifts (O-JLG) As well as other equipment. Employees caught improperly operating equipment without certification are subject to disciplinary action. Certification will be done by a formal training program conducted by the company and documented in writing. If you are unsure of whether you are certified for a particular task, notify your supervisor before operating the equipment. 4.0 DRUG & ALCOHOL PROGRAM The company maintains a zero-tolerance policy with regards to drugs and alcohol. As part of the Company drug and alcohol program, employees may be subject to pre-employment, post-accident, random and reasonable suspicion testing. Prior to testing, employees will have one opportunity to self-disclose in accordance with the terms of the Last Chance Agreement. Site Safety Plan (Fixed) & Employee Orientation Page 2 of 5

5.0 SITE SPECIFIC SAFETY REQUIREMENTS (SST) Electrical Safety Program The electrical safety program shall be maintained and managed as it is stated in the COMPANY Corporate Safety Manual. Mike Bolton is responsible for maintaining the facilities written Lock-Out, Tag-Out (LOTO) plan. Assured Grounding Conductor Program All electrical power tools, cords and power supplies shall be inspected and cleared and must comply with COMPANY ground fault applications. The Company utilizes GFCI s at all locations. Welding, Cutting and Hot Work It is imperative that only fully trained and certified employees be allowed to perform hot work at this location. If the task analysis indicates a possible flammable material within 35 feet of hot work operations it is the responsibility of the area supervisor to issue a hot work permit before welding or cutting operations may commence. Employees are required to make proper use of the provided fans, hoods, smoke evacuation devices or respirators as required by the engineering and process control procedures at all times during hot work operations. Compressed Gas Cylinders All compressed gas cylinders shall be stored as stated and required by the COMPANY policy. All compressed gas cylinders will be stored in a designated bottle cart, designated storage container or properly tied off to a secure object. Gas cylinders are NEVER to be stored or transported horizontally (lying on its side). {always use the bottle certified bottle carrier when using overhead crane to move bottles} Hearing Conservation Program COMPANY hearing conservation requires that all employees are to wear hearing protection at all times during {times of loud activity in the shop}. The only exception to the policy is at any time, safe verbal communication cannot be rendered due to wearing hearing protection. As soon as the verbal message is delivered, the hearing protection must be re-inserted. Confined Spaces Confined space training is required for entry or attending a confined space. Although not an exhaustive list, the following confined spaces are common at this facility: Shop-Tanks during fabrication are considered non-permit required confined spaces as long as all entry is proceeded by a confined space permit and air monitoring. All tanks must be forced air ventilated, regardless of entry and purpose. See S820 Working Inside Shop Built Tanks for more information. Access to water supply by the Forman s office door is a permit required confined space Heat/Stress Prevention Program COMPANY employees have been made aware of indicators of heat/cold stress awareness. The heat/cold stress awareness section can be located in the COMPANY Safety Orientation Workbook. Housekeeping All COMPANY employees are responsible for housekeeping practices. COMPANY employees are aware of the dangers of improper housekeeping. Good housekeeping eliminates the potential for possible tripping and slipping hazards. In respect to fire prevention standards, housekeeping also reduces the ability for acting as a fuel that would allow a fire to spread rapidly. Hydrogen Sulfide (H2S) Awareness Program (SST) COMPANY does not foresee any exposure to H2S at this facility. However, all COMPANY employees have been made aware of the dangers and cut off limits with brief descriptions of H2S (Hydrogen Sulfide) through the usage of the COMPANY Safety Orientation Workbook. Additional and more extensive training is supplied to personnel that will work around H2S on a more frequent basis. Excavation and Trenching COMPANY does not foresee any need for excavation, trenching or shoring at this facility. Site Safety Plan (Fixed) & Employee Orientation Page 3 of 5

Asbestos Awareness Program (SST) COMPANY does not foresee any exposure to friable asbestos at this facility. However, should a worker become aware of asbestos in the facility, they should contact their supervisor immediately. Workers who do work with asbestos will be certified as Asbestos Mngmnt/Maint Worker (W-ASB) prior to working with material. Benzene Awareness Program (SST) COMPANY does not foresee any exposure to benzene at this facility. However, should a worker become aware of benzene in the facility, they should contact their supervisor immediately. Workers who do work with benzene will be certified as Benzene Worker (W-BENZ) prior to working with material. Hexavalent (VI) Chromium Program (SST) It is possible that welding of stainless and other materials will require that employees be certified as Chromium (VI) Worker (W-HEXCHR). Please be aware that prior to welding or cutting this type of material, that certification is required. Please contact your supervisor if you have any questions. Spill Prevention and Control (SST). {the primary possibility of spills is from the forklifts, oil absorbent material is available by the welding supply cabinet} In the event that an environmental issue occurs, COMPANY personnel are to follow HAZWOPER Level One responder terms. Level one responders are to notice and report to their immediate supervisor only. If a leak or spill is detected, field personnel are to immediately notify the competent person (foreman). The foreman is then required to report any spill or leak to an on-site, customer representative. Operations are to cease until a qualified one/or authorized person gives the all clear and a permit is reissued for the continuation of resumption of work. 6.0 HAZARD COMMUNICATION / PROCESS SAFETY MANAGEMENT Site Hazard Analysis If potential site hazards have been identified, they will be discussed or communicated to the designated crew. A site-specific hazard identification section will be listed under the safety section of the foreman s job letter. Although verbal awareness steps will be taken, written notification will also be a form of communication. Again, this will be included with the foreman s job letter for awareness purposes. Forms of written communication will include, but not be limited to, the following topics: 1. Chemical Exposure; HAZCOM; MSDS 2. Hearing / High Noise Exposure 3. Caught between; Struck By; Pinch Points 4. Fire Hazard(s) 5. Confined Space 6. Fall Hazards; Fall Protection; Scaffold Usage 7. Crane Rigging and Lift Plans 8. Radiation Pre-Cautions 9. Continuous Air 10. Atmospheric Monitoring 11. Additional PPE 12. Site-specific Emergency Evacuation Plan. MSDS A complete binder of all substances in the form of an MSDS manual will be on-site in the foreman s trailer. The MSDS binder is readily accessible to any and all employee s working on the site and is located {at the door to the Forman s office}. All COMPANY employees have been trained, at an awareness level, in HAZCOM. Fueling & Refueling Fueling of company equipment will be done by certified operators of that equipment only. Storage of fuel is to be placed in a metal container and properly labeled as to the contents. Site Safety Plan (Fixed) & Employee Orientation Page 4 of 5

7.0 REQUIRED POSTINGS The following state required postings are located at {the lunchroom}: Regulatory posters or logs Emergency contact telephone numbers Local emergency or clinic information Equal Employment Opportunity Commission Poster Any pertinent information required for site-specific emergency notification 8.0 OSHA INSPECTIONS When possible, the company participates in voluntary OSHA inspection programs. In the event that an OSHA representative arrives to inspect the site and working conditions, then the local site manager and shop manager will be notified of the officer s arrive and assist them in their tour of the facility. 9.0 DISCIPLINE The company s disciplinary policy may be found {in the lunchroom}. 10.0 SITE SPECIFIC SAFETY INSPECTIONS Shop Inspections All shops are required to conduct a site-specific safety audit on their job on, at least, a weekly basis. Equipment Inspections - Prior to use, equipment is to be inspected. Potential pieces of equipment that could possibly used on the job site, but are not limited to, are cranes, skid steers, JLG/man lifts and forklifts. The equipment is to be inspected by a competent person and all unsatisfactory findings are to be documented and properly reported. The following is a list of items or equipment that must be inspected (if applicable) and the required frequency of inspection requirements.
