Newtown Memorial Day Weekend Tournament Registration Instructions Congratulations on your acceptance to the Newtown Soccer Club's 2014 Memorial Day Weekend Tournament. In order to complete your registration into the tournament your team will need to complete the following instructions and documents. Online Registration Your team needs to complete the online registration using the link provided (http://www.eyouthsportsusa.com/cgi-win/ebap.exe?nt106317)
2 TYPE THE DOCUMENT TITLE If you finished that portion then you can move on to the filling out your team history. This is important when the Newtown Soccer Club is creating the brackets. Please describe any conflicts you may have with double rosters, team strengths or what ranking your team would like to be placed in. To access this section, please use the login link (http://www.eyouthsportsusa.com/cgi-win/ebap.exe?pw106317) And login with your userid / password you used in the online registration portion. Once logged in you will be presented with a menu:
Newtown Memorial Day Weekend Tournament Registration Instructions Please make sure the following sections are completed: Important Info Entry Fee Payment Team and Contact Info Team History Email Registration Once the online portion is complete, teams can finish their registration via email. The following items will need to be submitted via email A copy of the State Approved Roster A copy of each US Youth Soccer Player Passes (ONLY front must be copied) A copy of each guest players US Youth Soccer Player Passes (ONLY front must be copied) Teams outside of Region 1 need to provide a signed permission to travel form. Check with your State association for the appropriate form. (Region 1 teams include: ME, NH, VT, MA, NJ, RI, PAE, PAW, NYE, NYW, CT, MD, DE, VA, WV) NSC Medical Release form for all players (you can use your clubs release form) Print, read and sign NSC MDW Rules/Policy Document Each of these items needs to be scanned into either an image file (JPG, JPEG, GIF, PNG) or into a PDF file. Please Zip all of the documents and attach to an email and send to the following address: newtowntourney@outlook.com The subject line should contain the following: Division- Gender Club Team Name ex: U11-Girls-Newtown-Force The body of the email should contain the attachment of the file and team rep contact information.
4 TYPE THE DOCUMENT TITLE Onsite Registration Onsite Registration will take place at the following location: Newtown Youth Academy 4 Primrose Street Newtown, CT 06470 Friday May 23, 2014 6:00 PM 9:00 PM Teams will not be permitted to check in on Saturday or Sunday WHAT TO BRING FOR ON-SITE REGISTRATION: Two (2) copies of your State Association, League or US Club approved roster. Current Player passes for each participating player. Guest players information should be added to the bottom of the official roster(s) being submitted. NSC Medical Release form for all players (you can use your clubs release form) Printed signed copy of the NSC MDW Rules/Policy Document REGISTRATION FAQ If I checked my team in via email, will I receive an email confirming that my roster has been approved? Yes - all teams will receive an email confirming their roster has been approved. This will be your official tournament roster. You must bring this to the tournament with you and may be asked to show it along with your original player cards to the Field Marshal (FM) prior to every game. This email will be sent no later than one week prior to the tournament start date. If a player is not attending the event do I leave their name on the roster? No, if a player is not attending the event; you need to cross their name off of the roster.
Newtown Memorial Day Weekend Tournament Registration Instructions What is the roster size and number of guest players a team can bring to the event? U-9 through U-11 teams are allowed a maximum of 15 players. U-12 through U- 14 teams are allowed a maximum of 18 players. Teams may have up to two (2) guest players per team. How do I add a guest player? Step 1: Obtain the guest players current player card and medical release form. Step 2: Copy your original roster. Please handwrite the guest player s full name, unique jersey number, player identification number and birthday below the bottom of the existing rostered players. Note: No roster may be comprised of players with different passes from different organizations (no mixed rosters ). How do I add a guest player after I emailed my registration information? Due to the size of the tournament, teams will not be allowed to resubmit their roster on-line after the deadline. Instead teams will be allowed to resubmit their roster during the on-site check-in dates and times. If your team resubmits their roster they must follow the directions for on-site registration. Where can I find the NSC Policy Document? You can download the medical release form from this link (https://s3.amazonaws.com/files.leagueathletics.com/text/documents/6311/4744 2.pdf) or you can use your clubs. How can I submit medical release forms? You can scan each of the signed release forms and add to the email registration package. If you do not have access to a scanner, onsite registration will take place Friday May 23rd 6-9pm. If a player does not have a medical release form they will not be allowed to participate in the tournament.
6 TYPE THE DOCUMENT TITLE What if I cannot make onsite registration? If a team rep cannot make onsite registration, we have the email registration set up. If both options are not able to be performed, please contact the VP of Travel, Joshua Drew at vptravel@newtownsoccerclub.org as soon as possible to make other arrangements. What is the deadline for using online/email registration? The deadline is May 9 th, 2014. If your teams information is not submitted prior to that date you team will be removed from the tournament. What if I did not pay yet? If you or your club made prior arrangements then you do not need to do anything. If you have not paid, you can either complete payment online through the team menu or send in payment to the following address: Newtown Soccer Club P.O.
