I have received and agree to all the rules and regulations governing cheerleader / dance team activities. I also understand that campus rules may require that a minimum level of proficiency be demonstrated by each member before participation in each routine. I agree to accept any and all financial responsibilities that may incur if the above-named student is selected as cheerleader / dance team member. Cheerleader / Centurionette Clinic Permission Slip I herein grant permission for Mrs. Cindy Sandoval and Ms. Jeneva Tilley, the John Paul II High School Cheerleaders and Centurionettes Directors, to act on my behalf in matters of discipline and obtaining medical attention should either become necessary. I acknowledge the authority of the director to send my child home by public conveyance at my expense should the behavior of my child be detrimental to an educational experience. I will not hold John Paul II High School of the sponsors responsible in case of an accident. It is understood that John Paul II High School student policy will be in force for the duration of the clinic. There is a $40.00 cash fee due no later than 2/21/14 for cheerleaders and 3/3/14 for dance team. Girls should dress comfortably; wear shorts, t-shirts, and comfortable shoes. JPII Cheer/Dance Demerit System In order to keep disciplinary actions consistent for everyone, we will be using the following demerit system. Demerits will be distributed accordingly for any behavior that the coaches feel is unacceptable. Please note that we may add or remove penalties throughout the year as necessary, but written notice will be given if a change is made. All merits and/or demerits will be recorded and dated in a notebook. Demerits do not re-start at the end of the semester. Demerits earned under the Appearance, Grades and Attendance categories can be canceled out by acquiring 5 merits for every 1 demerit. Merits cannot cancel demerits earned for misconduct. We have read the cheerleader/dance constitution and understand our obligation to encourage our child to accept the responsibility, commitment and pride, which are the foundations of the John Paul II cheerleading/centurionette organization. We understand all cost obligations associated with this program are to be met per the schedule of the organization. We understand the inherent financial obligations of this program, which includes the cost of uniform, camp oufits, camp, transportation costs, additional technique classes (tumbling for cheerleaders or dance classes for dancers) and additional personal items and we are willing to provide these things as they become necessary. We agree to accept the results of tryouts without complaint and we agree that if our daughter is chosen as a cheerleader or dancer we agree to accept performance decisions and decisions made by the Director at any game or performance with out complaint. Additionally we agree to comport ourselves with dignity at all games and performances. I have read and support the regulations under which my daughter will be governed as a cheerleader or dancer.
