(2) RED ROSE FIGURE SKATING CLUB 2013-14 MEMBERSHIP CATEGORIES Home Club Membership $60.00 $ **Membership from March 1 st through June 30 th will be pro-rated to $50.00** This membership is for active skaters who wish to represent The Red Rose FSC. Benefits include attendance privileges at monthly meetings, voting privileges at annual club meetings, reduced USFS testing fees for RRFSC sponsored test sessions, quarterly newsletters, ability to purchase official club apparel, optional reduced club ice fees, participation in annual ice show(s), and participation in Ice Theatre*. This membership also includes annual dues for ISI or USFS. If you would like both memberships, please increase the fee by $15.00. Subsequent Skating Family Member: $40.00 $ Introductory Membership $40.00 $ This is a full Home Club Membership that is only for First Time Members and can only be used one time per person. This does not apply to club transfers. Affiliate Membership $80.00 $ This membership is for active skaters who wish to skate on Red Rose FSC sponsored ice, but already represent another USFS club. Benefits include optional reduced club ice/test session fees and participation in annual ice shows. Coach Affiliate Membership $35.00 $ Collegiate Membership (4 years) $120.00 $ This membership is for active skaters who have just graduated high school and beginning a 4 year college program, and wish to continue to skate on Red Rose FSC sponsored ice. A college ID is required for this membership and subject to approval by the Executive Committee. Non-Skating Home Membership $45.00 $ This membership is for non-skaters who wish to support the Red Rose FSC. Benefits include attendance privileges at monthly meetings, voting privileges at annual club meetings, quarterly newsletters, and ability to purchase official club apparel. This membership includes annual dues for USFS. Board of Directors Membership $20.00 $ This membership is for secondary family members who serve on the Board of Directors for the RRFSC. This membership is a home club membership that includes annual USFS dues and all home club membership privileges. *Required tests must be passed for both Jr. and Sr. Ice Theatre participation

(3) The Red Rose Figure Skating Club Waiver and Release of Liability, Assumption of Risk and Indemnity Agreement ( Agreement ) In consideration of participating in activities, I represent that I understand the nature of figure skating activities ( activity ) and that I am qualified, in good health and in proper physical condition to participate in such activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that this activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the activity, the conditions in which the activity takes place, or the negligence of the releasees named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the activity. I hereby release, discharge, and covenant not to sue the Red Rose Figure Skating Club, United States Figure Skating, it s directors, officers, administrators, sponsors, volunteers, agents, employees, staff, instructors, trainers, other participants and if applicable, owners and lessors of premises on which the activity takes place (each considered one of the Releasees herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the releasees or otherwise, including negligent rescue operations; and I further agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the releasees from any loss, liability, damage, or cost which any may incur as the result of such claim. The Red Rose Figure Skating Club has the right, but not the obligation, to provide rules, regulations and/or ice monitors for Club Ice. We hereby acknowledge that the Red Rose Figure Skating Club shall not be responsible for the supervision of the members at Club Ice. I have read this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect. In addition, I/we give permission to The Red Rose Figure Skating Club to use any of the above mentioned in its publications including, but not limited to, the club s website. I/we are aware that the below named participant(s) may be part of videos, DVD s, and/or photographs that will be sold to parents, family members, and customers. I/we understand that I/we will make no claim for compensation for use of these materials. I/we also agree to release The Red Rose Figure Skating Club from all claims and demands of any nature whatsoever arising from or with respect to the use of any such materials. 6/2010 U.S. FIGURE SKATING & RED ROSE FSC PARENT/SKATER CODE OF CONDUCT Codes of Conduct give everyone a guide to what is expected of us if we are part of an organization, participating in a sport, or as spectators or representing you or your child s organization. By signing below, I hereby agree that: 1. I will encourage good sportsmanship by demonstrating positive support for all skaters, coaches, and officials at every practice session, competition, test session and show/event either sponsored by the home club or any other club. 2. I will place the emotional and physical well being of my child ahead of my personal desire to win. 3. I will myself, or encourage my child to skate in a safe and healthy environment. 4. I will never ridicule or yell at my child or other participants or spectators for making a mistake or place blame on teammates, coaches, parents for placement in competitions. 5. I will teach my child that doing one s best is more important than winning, so that my child will never feel defeated by the results of a competitive event, test session or show/event. 6. I will do my best to make skating fun and will remember that me or my child participates in sports for my/his/her own enjoyment and satisfaction. 7. I will ask and reinforce the importance of treating other skaters, coaches, fans and officials with respect regardless of the situation. 8. I will applaud a good effort in both victory and defeat, emphasizing the positive accomplishments and learning from the mistakes. 9. I will/i will teach my child to, resolve conflicts without resorting to hostility or violence. 10. I will be a positive role model for my child and other skaters. 11. I will respect my or my child s coach and refrain from coaching my child or other skaters during competitions, tests, exhibitions, practices because it may conflict with the coach s plan or strategy.

I will respect the decisions of officials and their authority during competitions and test sessions and teach my child to do likewise. 13. I will show appreciation and recognize the importance of volunteers and club officials at ANY and ALL types of events where I/we are representing the RRFSC. 14. I will support all the opponents in my child s competition and respect the rights of all skaters to participate. RED ROSE FIGURE SKATING CLUB/ ICE RULES 6/10 1. Skaters should make every effort to avoid collisions. Skaters should use caution and be aware of other skaters at all times, especially tots. Parents of tots and beginner/low level skaters should ensure that their child practices safely. 2. Skaters at a level of Freestyle 4 and below should try and stay as close to the boards as possible when not in a lesson. This is a safety issue. 3. Skaters taking a private lesson have the right-of-way. Every effort should be made to stay out of the way of skaters doing their program with music. Skaters should not, however, expect to perform their program without interruptions on the ice. 4. Skaters and parents may NOT interrupt another skating lesson. 5. Parents must refrain from coaching their skater from the doorways or sides of the ice during freestyle sessions. 6. Only coaches, skaters, and ice monitors are allowed in the Visitor/Home hockey boxes and music area during freestyle sessions. 7. The use of any portable audio device is prohibited by all skaters. 8. If there is a crowd of 10 or more skaters, a skater may not have their program played more than twice in a session. This includes those in a lesson. 9. Coaches that are not choreographing a program should try to stay as close as possible to the wall while teaching to lessen unneeded traffic. 10. Many freestyle moves require space. Extra caution and common sense should be used by the skater to ensure adequate clearance when doing these moves. 11. There should be no loitering on the ice. If you wish to have a conversation with a friend, please remove yourselves from the ice. 12. Possession of and/or consumption of alcohol or illegal drugs is strictly prohibited. On the ice, skaters must not: kick, stomp feet or drag toes, stand, sit or lie in one spot, curse or use inappropriate language, race or play tag (unless instructed by a coach during group lessons), chew gum, smoke, throw objects, sit on boards, use a cell phone, eat or drink while skating (drinks being kept on the boards is acceptable) 13. Skaters and parents not wearing ice skates are prohibited from being on the ice during a freestyle session. (coaches carry their own liability insurance covering this) 14. The fundamental rules of courtesy, good sportsmanship and common sense apply at all times. The first infraction of a rule, skaters will be issued a warning. The second infraction, skaters will be asked to leave the ice for the session. Should a concern arise, skaters must speak to their own coach first and then to the monitor or Skating Director. The monitor or member of the RRFSC coaching staff has the authority to correct those violating the rules. The Board reserves the right to enforce such discipline as it deems necessary for the safety and well-being of other skaters.

DROP OFF POLICY In the interest of ensuring the safety and supervision of our skaters, both on and off the ice, the following drop-off policy will be enforced for all skaters under the age of 18 when participating in any RRFSC sponsored programs and freestyle ice sessions: 1. Skaters under the age of 12 may NOT be dropped off at the rink without a parent or other responsible adult accompanying them. This is in no way meant to prevent parents of younger skaters from forming car pools, or arranging with one another for the care of their skaters; however, a responsible adult must remain at the rink at ALL times with skaters under the age of 12. 2. Skaters from age 12 through 15 may be dropped off at the rink provided that a parent (or another responsible adult authorized by the parent) signs the skater in and out in the warm room. A drop-off log will be kept alongside the ice time sign-in sheet for this purpose (see attached for a sample log sheet). Parents are not to drop-off/pick-up skaters in front of the rink, but must enter the building to sign their skater(s) in and out. When signing in, the parent/responsible adult must leave a contact phone number at which they can be reached during the time that the skater will be at the rink. Parents are also asked to provide an approximate pick-up time, so that the adults/coaches remaining at the rink know when to expect their return, and can contact them should all adults/coaches need to leave the rink prior to the expected pick-up time. 3. Skaters from age 16 through 18 (many of whom drive themselves to the rink) need not be accompanied into the building by a parent or other adult, but may sign themselves in and out on the drop-off log. These skaters must also provide a contact number at which their parent can be reached during the time that the skater will be at the rink. 4. No skater should be dropped off if their coach is the only adult present in the building. In order to safeguard our coaches, as well as our skaters, a second coach or another non-skating adult must be present for skaters to be dropped off. 5. A skater s drop-off privileges will be revoked should that skater remain at the rink without a ride on three separate occasions at the end of any scheduled ice time or activity and/or when all other adults are ready to leave the building. Please keep in mind that this drop-off policy relies on the good will and cooperation of all club members. It is expected that parents will remain at the rink with their skater(s) whenever possible. At the same time, we wish to provide parents who cannot stay, or who need to run occasional errands or to care for other children, the freedom to do so. Obviously, all coaches, parents, and other adults at the rink will do whatever they can to assist a skater who has been dropped off, should the need arise. However, parents must remember that these adults cannot be expected to continuously watch skaters who have been dropped off, nor do they assume full responsibility for these skaters.

FUNDRAISING OPTION WE ARE REQUESTING THAT EVERY MEMBER, UPON RENEWAL, PAY AN ADDITIONAL FEE FOR FUNDRAISING for the entire skating year. MEMBERS WHO OPT TO PAY THE FEE NOW WOULD NOT BE REQUIRED TO PARTICIPATE IN ANY FUNDRAISERS THAT WE OFFER (WITH THE EXCEPTION OF THE AUCTION) AND COULD APPLY ANY DOLLARS RAISED DIRECTLY TO THEIR PERSONAL THEATER FEES. THOSE THAT DO NOT CHOOSE TO PAY AT RENEWAL, WOULD HAVE PERSONAL FUNDS APPLIED TO THEIR THEATER PAYMENTS AFTER THE CLUB RECEIVES THE FIRST $35.
