Arizona State University Club Cheer 2016-2017 Tryout Packet Checklist Tryout Application/ A recent photograph Cheerleading Agreement A copy of your health insurance card $50 cash or check made payable to ASU Club Cheer Arizona Heat waiver form (http://n1media1.files1.jamspiritsites.com/78397/1085604514552219c400271.pdf) The waiver is necessary for any possible injuries you might sustain in tryouts or future practices. If under 18, you must have a parent sign the Minor Release form, ASU will not cover any expenses from injury for incoming or returning candidates. Please bring all the above to in person tryouts on May 8 th 2016 or send along with video submission no later than May 2 nd 2016 to: ASU Club Cheer c/o AZ Heat All Stars 3164 S Country Club Dr, Suite 11 Mesa, AZ 85210 Date and Time- Sunday May 8th, 2016 from 12:00-3:00pm Location- Tryouts will be held at AZ Heat All Stars at 3164 S Country Club Dr. suite 11, Mesa AZ 85210 Who is eligible to try out? High school seniors with proof of admission to ASU Undergraduate or graduate transfer students with proof of admission to ASU Full-time undergraduate or graduate ASU students in good academic standing with the University Format of tryouts- This year we have opted for a full one day tryout. We will be focusing on tumbling and jumps only. During the warmup s you may ask for a spot on advanced tumbling, but during actual tryouts spots will not be allowed. Candidates will perform tumbling and jumps

in front of the panel of judges in groups of 3-4. Once a candidate has performed they are allowed to leave. The final decision will be made on May 11 th and posted to the team s website www.sundevilcheer.com by 10:00am. Time commitment- All cheerleaders are expected to be at every practice unless excused by the Head Coach (details in cheerleader agreement). Currently practices are Tuesdays and Thursdays 8:30pm-10:30pm; this may eventually change but it will remain within a similar timeframe. There may also be additional weekend practices as competition becomes closer. Tryout Attire- Please wear all black athletic shorts and shirt/sports bra, and cheer shoes. Hair must be up in a ponytail with a bow, no jewelry, no fake nails; NO EXCEPTIONS! Any violation of these dress code rules will result in deductions during tryouts. All potential cheerleaders are expected to portray a collegiate image when trying out for ASU. Body piercings and jewelry of any kind, extreme hairstyles and/or colors are not acceptable at tryouts or during the year as an ASU Cheerleader. All cheer candidates must remove all piercings and other jewelry prior to arrival at tryouts. Required skills for tryout: Standing Tumbling: Standing back tuck Jumps to back 2 to tuck Running Tumbling: Running tumbling ending in layout or higher Specialty pass through to tuck RO BHS layout **We will not be looking at stunting skills at tryouts, focusing on tumbling and jumps ONLY **

ASU Cheerleading Application Agreement (Please Print Clearly) I,, submit this application to the ASU Cheer Program for a position on the Club Competition Cheerleading Team. For the 2016-2017 year, my academic classification will be. By submitting this application, I understand and agree to the following: I will be an ASU student, taking a minimum of 9 undergraduate or graduate credit hours at Arizona State University for the entire 2016-2017 fall and spring semesters. I must maintain a 2.0 GPA and will hereby release my academic transcripts to the Sports Club Association. I understand that I must be in good standing with Arizona State University or my high school and not on any type of academic or disciplinary probation. If selected, I understand that I am required to be covered by personal primary medical insurance throughout my time as an ASU Cheerleader. I have no health or physical defects, which would hinder my ability to perform as an ASU Cheerleader or which might cause performing as an ASU Cheerleader to be unsafe to my health or the health and wellbeing of my team mates. The physical demands of being an ASU Cheerleader require that I maintain a high level of physical fitness and that I pass a complete physical examination administered by my physician or a physician at ASU. If selected and I accept a cheer team position, I understand I will be required to attend all scheduled practices, classes, performances, summer camps, events, competitions, team meetings, parades, and other functions or duties designated by the Sports Club Association and Coaches. I understand I am agreeing to a FULL YEAR commitment (fall AND spring semesters and summer obligations). Being an ASU Cheerleader is a major personal commitment of my non-academic time and cannot be placed secondary to any other non-academic activity. I understand that, with the exception of academics, my commitment to the ASU Cheer Program takes FIRST priority over all other activities (sorority, fraternity, clubs, organizations, work, etc.) Furthermore, I understand the last minute appearances/events may arise and cheerleaders must be present. I am responsible for the care and upkeep of all components of the cheerleading uniform. The ASU Cheerleaders play an important role in representing the university and its club sports program. As such, I will always know that I am an official representative of Arizona State University, its students, faculty, staff, alumni, and cheer program. I will always conduct myself in a manner to uphold that character, tradition, integrity, and spirit of Arizona State University. I agree to follow all safety guidelines deemed necessary by the coaches and AACCA. I understand that I must maintain an email account and check it DAILY.

I understand that I must obtain a note from family or school physician to be excused from any assigned function, unless approved in advance by the coaches. I understand and accept that failure to meet any of the requirements listed above will result in my disqualification from consideration for a position.
