Donaghue / PTO 2019 Ski Program at Bradford SIGN UP /PAPERWORK AND PAYMENT DEADLINE: OCTOBER 26, 2018 NO EXCEPTIONS The following information outlines the Donaghue Ski Program offered at Bradford Ski Area. The program for 2019 will begin on Thursday, January 3,2019 and continue for six consecutive Thursdays thereafter weather permitting (1/10, 1/17, 1/24, 1/31, 2/7,). If Bradford cancels the program because of weather or lack of snow, the session will be made up sometime after February vacation. If we cancel as a group, we do not get a make up week. If there is no school in the district, there is no ski program and the week will be made up. If your child is ill and misses a week, there is no refund and/or make-up day. Other refunds are detailed on Bradford Ski s Enrollment form (available on sign-up night). Please read these rules carefully so that there will be no questions later. Thank you! Program Options: Option 1: Bus & lift ticket only = $180 Option 2: Bus, lift ticket & ski/snowboard lesson = $250 Option 3: Bus, lift ticket, lesson & rental = $360 Option 4: Bus, lift ticket & rental = $300 Helmet rented separately = $30 Payment is required with completed forms BEFORE OCTOBER 26, 2018 Please submit the following in an envelope marked with students name and Ski Club to The Donaghue School: -Completed Forms -Check made out to Merrimac PTO Lessons Beginner lessons will be scheduled first, approximately ½ hour after arrival. The lesson will be at the same time each week. A label will be issued to each participant indicating name, class number, and class time. It s the participant s responsibility to be on time for class each week. The maximum class is twelve and the class is taught by the same instructor each week when possible. Rental Information Refer to Ski Bradford s enrollment form. There is no refund for rental equipment once the program starts. Additional Logistical Information Students ski/snowboard equipment must be dropped off at the school on Thursday mornings; no equipment can be transported to school on the bus in the morning. Skis/boards go in the cart in the main lobby. 1
The school dismisses the ski program participants at 3:00 p.m. at which time they will gather their equipment and head straight for the designated bus(es). Once they arrive at Bradford Ski Area, students should go to the lodge to locate the table reserved for PENTUCKET/DONAGHUE Ski Program, as the passes and rental forms will be handed out then. Rental equipment must be returned every week (except helmets, which you will keep for the 6 week program duration). At least one parent volunteer will be in the lodge during the ski program for questions or concerns and one or two ski monitors will be on the hill. The kids get very hungry and thirsty; please provide a snack and drink for your child to enjoy at Bradford. You may also provide your child with money to buy food, but keep in mind that ski area food is expensive. To minimize equipment loss, it is recommended you buy a lock for skis or snowboard. Also, please ensure that the child s name is on everything. Many hats, gloves, goggles, ski pants, etc. are lost each week. The volunteers/monitors/school are not responsible for lost or stolen equipment. Bus/transportation is provided from the school to Bradford Ski Area only. It is the parent s/guardian s responsibility to arrange transportation home. The program ends promptly at 6 p.m. An adult must sign out the child/ren that they are picking up inside the lodge at the Pentucket/Donaghue table. Parent volunteers will leave Bradford at 6:15pm. Volunteers are not permitted to leave participants unattended; late pick-up twice will result in participant being dismissed from program with no refund. If an unforeseen emergency arises that prevents you from picking up your child/ren, please contact a volunteer immediately. If a participant loses his or her pass, Ski Bradford charges $10 for a replacement. Behavior Any student found drinking, stealing, bullying, possessing a controlled substance, using vulgarities, skiing or boarding in a reckless manner, or anything else that Bradford or the parent volunteers consider disruptive, dangerous or illegal, will be dismissed from the program without a refund. Proper behavior in the base lodge also includes: No skis or snowboards in the building, picking up after yourself, removing trash from tables and returning cafeteria trays, and keeping doorways clear at all times. The Donaghue Ski Program s primary communication is via email any cancellations, changes, or updates will be emailed to you. Please check your email frequently for updated information. 2

I have provided the necessary information to make contacting successful. If I am unable to be reached, I authorize the Donahue PTO parent volunteer in charge to call the physician indicated above and follow his/her instructions. If it becomes difficult or impossible for the doctor to be contacted, I GIVE THE DONAGHUE PTO PARENT VOLUNTEER IN CHARGE PERMISSION TO MAKE WHATEVER ARRRANGEMENTS BECOME NECESSARY IN THE BEST INTEREST OF MY CHILD. Additionally, I recognize this form and the other application forms as a permission slip for my child to participate in the Ski & Snowboard program. I understand that skiing and snowboarding can be a dangerous activity and I agree to indemnify and hold completely harmless, the staff, administrators, and monitors, and to absolve them from any and all liability arising from my child s participation in this activity.
