1 CTC Risk Assessment Form How to use this sheet: Step 1 Using the instructors guide and this sheet note any potential hazards, and add any hazards you identify yourself. In particular survey the route you intend to use and identify physical locations where risks occur Step 2 Consider the likelihood of the hazard causing an incident Remote Never been known to happen during training Low Has occasionally been known to happen during training Medium Has sometimes been known to happen during training (1-2 times annually) High Causes known incidents during training Step 3 Consider the usual severity of an injury of an injury arising from an incident None Slight Serious Severe May cause the rider to stop training Minor sprain, graze, upset Injury requiring professional care Long term patient care or death Step 4 Consider how to reduce the risks, especially the high ones and more likely ones. This may simply be giving advice to riders. You may wish to warn riders of specific hazards, but remember that you cannot control events on public roads. Therefore you must not give the impression that they can take any less care than they would when riding normally on the roads because they are being trained. Remember, this is a practical activity in a public place; it places a premium on common sense and reasonable, usual behaviours. Clients are learning to cope with such conditions and the training will not be effective if normal road activities do not take place! Page 1 of 9

Instructors check all bikes at the start of the course are fit for purpose, including their own. Take note of useful shelters near training areas. Reminder about safe stopping distances after rain Begin on quiet junctions and roads and only move to busier when trainees have demonstrated sufficient skill to progress Pupils are taught basic safety checks and practice before each ride. Instructors assist with minor adjustments to make bikes safe. Page 5 of 9

7 Hertfordshire CC Blank Site Specific Risk Assessment Form To be completed and returned to. by completion of course. Try to start somewhere quiet and progress to busier roads. You may be limited by the location and time available. If both instructors are riding you can cover a greater area, but remember to allow time to return to school before the end of the session. Sites 1 Quiet, clear road to practise starting and stopping 2 Road with cars parked on both sides to practise overtaking and holding the lane 3 Main road with a side road with give way markings to practise passing a side road 4 A T-junction to practise left turns in 5 A T-junction to practise right hand turns out, preferably with give way markings and hazard lines to show centre of side road 6 A wide main road with T-junction to practise right hand turns in. Wide enough for cars to pass a cyclist waiting in the middle of the road 7 A junction with cars parked nearby on the main road so that it is necessary to go over the give way markings to see 8 A suitable road to practise U turns 4 locations should provide enough variety as you may be able to use them for more than one manoeuvre. If you need to travel to reach your chosen site, sketch or describe route taken. For each site used for training name both roads, say if there are road markings, estimate, volume of. For both route and site note any situation worthy of comment. Remember that road situations can vary with time and day. A site that you have used during the morning may become too busy or have too many towards the end of the school day.
