QUALIFIED OPERATOR: PERSON DESIGNATED BY EMPLOYER WHO HAS APPROPRIATE OFFSHORE EXPERIENCE AND TRAINING, SHOULD BE QUALIFIED TO SAFELY OPERATE CRANES ON WHICH THEY HAVE BEEN TRAINED, SHOULD BE QUALIFIED TO PERFORM PRE-USE AND MONTHLY INSPECTION, IS CONSIDERED A QUALIFIED RIGGER. 2 QUALIFIED INSPECTOR: PERSON DESIGNATED BY EMPLOYER WHO HAS SUCCESSFULLY COMPLETED COURSES ON CRANE AND HOIST MAINTENANCE AND TROUBLESHOOTING; SHOULD BE QUALIFIED TO PERFORM THE INITIAL, QUARTERLY, AND ANNUAL INSPECTION. 3. 3 OPERATING PRACTICES AND THE QUALIFIED OPERATORS (SECTION 3.1) 3 THE QUALIFIED OPERATOR WILL: 01. BE RESPONSIBLE FOR THE OPERATIONS UNDER DIRECT CONTROL 02. STOP AND REFUSE TO HANDLE LOADS AS SAFETY DICTATES 03. BE AWARE OF CRANE S OPERATING CHARACTERISTICS 04. BE FAMILIAR WITH THE EQUIPMENT AND ITS CARE 05. OBEY AN EMERGENCY STOP SIGNAL GIVEN BY ANY PERSON AT ANY TIME 08. VERIFY THAT APPROPRIATE LOAD CHARTS ARE IN PLACE FOR THE CRANE CONFIGURATION IN USE 09. SECURE THE CRANE AGAINST SWINGING WHEN NOT IN USE 10. WILL NOT OPERATE CRANE IN PROXIMITY OF HELIDECKS WHILE HELICOPTER IS LANDING OR TAKING OFF 11. INSURE THERE IS SUFFICIENT LIGHTING WHEN CRANE IS USED AT NIGHT 12. WILL NOT ALLOW FIELD WELDING ON LOAD HOOKS OR SLING HOOKS 12A. THE OPERATOR WILL MAINTAIN LOG OF PRE-USE INSPECTION BEFORE STARTING THE CRANE THE QUALIFIED OPERATOR WILL ENSURE: 1 PRE-USE INSPECTION HAS BEEN COMPLETED 2. ALL CONTROLS ARE IN OFF OR NEUTRAL POSITION 3. ALL PERSONNEL ARE IN THE CLEAR BEFORE LEAVING THE CONTROL STATION FOR PROLONGED PERIOD, THE QUALIFIED OPERATOR WILL: 1. LAND ANY ATTACHED LOAD 2. DISENGAGE THE MASTER CLUTCH 3. SET ALL LOCKING DEVICES 4. PUT CONTROLS IN THE OFF OR NEUTRAL POSITION 5. STOP THE PRIME MOVER 6. ASSURE CRANE WILL NOT INTERFERE WITH HELICOPTER OPERATIONS
4 OPERATION (SECTION 3.2, 3.4 AND APPENDIX B) 4 THE LOAD 1. THE OPERATOR WILL KNOW THAT THE HOOK LOAD IS WITHIN THE CRANE S APPLICABLE LOAD AT THE LOAD RADIUS 2. THE LOAD CHART WILL BE VISIBLE TO OPERATOR AT THE CONTROL STATION ATTACHING THE LOAD 1. THE LOAD WILL BE ATTACHED TO THE HOOK BY SLINGS OR OTHER SUITABLE DEVICES 2. THE LATCH WILL BE CLOSED TO SECURE LOOSE SLINGS 3. THE HOIST ROPE WILL NOT BE WRAPPED AROUND THE LOAD MOVING THE LOAD 1. DO NOT APPLY A SIDE LOAD TO THE BOOM 2. MINIMIZE PENDULUM ACTION OF SUSPENDED LOAD WHEN SWINGING THE CRANE 3. CRANES WILL NOT BE USED FOR DRAGGING LOADS 4. AVOID SWINGING LOADS OVER PERSONNEL 5. BLOCK OR CRIB SUSPENDED LOADS BEFORE PERSONNEL ARE PERMITTED TO WORK BENEATH OR BETWEEN THEM 6. TAG LINES SHOULD BE USED WHEN NECESSARY 7. ONE QUALIFIED OPERATOR WILL BE RESPONSIBLE FOR THE OPERATION OF A TWO CRANE LIFT BEFORE STARTING THE LIFT VERIFY 1. THE LOAD IS FREE TO BE LIFTED 2. MULTIPLE PART LINES ARE NOT TWISTED AROUND EACH OTHER 3. THE HOOK IS BROUGHT OVER THE LOAD TO MINIMIZE SWINGING 4. THE ROPE IS PROPERLY SEATED ON THE DRUMS AND IN THE SHEAVES. THE CORRECT SLINGS HAVE BEEN ATTACHED FOR THE WEIGHT TO BE LIFTED DURING LIFTING CARE SHALL BE TAKEN 1. ACCELERATION OR DECELERATION OF LOAD IS ACCOMPLISHED IN A SMOOTH MANNER 2. LOAD, BOOM, OR OTHER MACHINE PARTS DO NOT CONTACT ANY OBSTRUCTION PERSONNEL TRANSFER 1. ALL HOOKS USED FOR SUPPORT OF PERSONNEL WILL HAVE A SAFETY LATCH (LATCH WITH REDUNDANT LOCKING METHOD) 2.
