1 BASKETBALL Section 1 General 1. The Basketball tournament to be held as part of the 2015 ORLEN OLYMPICS a competitive sports event for employees of the ORLEN Group, designed to foster the corporate values and promote the image of PKN ORLEN S.A. and companies of the ORLEN Group is organised by Agencja Allegro Sp. z o.o. Sp. k, with its registered office at ul. Słowackiego 19a in Warsaw ( Organiser ). 2. The rules of participation in the 2015 ORLEN OLYMPICS are set forth in the Rules of the 2015 ORLEN OLYMPICS ( Rules of the Event ). 3. The Basketball tournament of the 2015 ORLEN OLYMPICS shall be a strictly amateur event. 4. Polski Koncern Naftowy ORLEN Spółka Akcyjna, with its registered office at ul. Chemików 7 in Płock , shall be the administrator of the personal data to be processed in connection with the Basketball tournament of the 2015 ORLEN OLYMPICS, within the meaning of the Act on Personal Data Protection of August 29th 1997 (Dz.U. of 2014, item 1182, as amended). Provision of personal data shall be required to participate in the Basketball tournament of the 2015 ORLEN OLYMPICS in accordance with these Rules. Persons who have provided their personal data shall have the right to access and correct them, as well as revoke their consent to the processing of their personal data at any time. The administrator shall process the personal data together with the Organiser, whom the administrator shall authorise to process the personal data for the purposes of the 2015 ORLEN OLYMPICS pursuant to Art. 31 of the Act on Personal Data Protection.
2 Section 2 Participation 1. Only employees of PKN ORLEN S.A. of Płock and employees of other ORLEN Group companies may participate in the Basketball tournament. An employee shall mean a person employed at PKN ORLEN S.A. or an ORLEN Group company under an employment contract. 2. PKN ORLEN S.A. and each company of the ORLEN Group may enter any number of teams in the Basketball tournament. 3. Prior to the Basketball tournament, participants shall present to the Organiser a document confirming their identity, such as a valid employee ID card. A person who fails to present such document on the day of the competition shall not be permitted to participate. 4. Participants shall observe public order, follow the instructions of safety coordinators and treat other participants of the 2015 ORLEN OLYMPICS with respect. The Organiser shall have the right to remove from the premises of the tournament any person who in any way disrupts public order or the 2015 ORLEN OLYMPICS, or who acts in violation of these Rules or the Rules of the Event. 5. Participants in the 2015 ORLEN OLYMPICS shall declare in writing, or by checking the appropriate box if they register online through that they agree to observe these Rules, thus acknowledging and undertaking to respect their provisions. 6. Participants in the 2015 ORLEN OLYMPICS shall declare in writing, or by checking the appropriate box if they register online through that they agree to participate in the competition at their sole responsibility, fully aware of the dangers and risks inherent in such events, including the risk of injury and death. Only persons in good health, and without any medical contraindications against participating in the Basketball tournament of the 2015 ORLEN OLYMPICS, shall be allowed to enter the Basketball tournament. 7. Participants in the Basketball tournament shall declare in writing, or by checking the appropriate box if they register online through that they consent to the processing of their personal data, including placement of such data in online and printed communications relating to the 2015 ORLEN OLYMPICS. 8. Participants in the Basketball tournament shall declare in writing, or by checking the appropriate box if they register online through that they agree to submit to the verification process which will take place at the Event Office on June 12th 13th During the verification, each participant shall be required to submit again the written declaration referred to in Section 2.6 above.
3 9. The declarations referred to in Section above are included in Schedule 1 to these Rules. 10. Team captains shall deliver, in the manner described in Section 8 of these Rules, declarations made in accordance with Schedule 1 to these Rules, signed by members of their respective teams. 11. Participants may collect their starter kits at the Event Office located on the premises of Wisła Płock Stadium (Stadion Wisły Płock) at ul. Łukasiewicza 34 in Płock, on June 12th 2015 between 7am and 12 midnight, and on June 13th 2015 from 7am until the end of the competition. Section 3 Competition rules 1. Players taking part in the 2014/2015 season games organised by the Polish Basketball Association (Polski Związek Koszykówki) and Regional Basketball Associations (Okręgowe Związki Koszykówki), in the Third and higher leagues (or their equivalents in the case of foreign companies), may not participate in the Basketball tournament of the 2015 ORLEN OLYMPICS. A team in which any such player is found shall be disqualified. 2. The rosters of teams entered in the Basketball tournament shall be published on 14 days prior to the competition start date. 3. Any protests regarding the rosters may be filed by team captains within five days of their publication. A protest must contain the first and second name of the person concerned, along with a concise description of the nature of the complaint. Protests filed after this deadline shall not be accepted. 4. Protests regarding team composition must be submitted to the Organiser, either in person or in writing at the Organiser s office. 5. After receiving a protest, the Organiser shall immediately notify the captain of the relevant team and take appropriate steps to resolve the situation. 6. The captain of the team whose composition has been objected to shall submit a written explanation to the Organiser within two days of being so notified. If the team captain fails to furnish any explanations, the Organiser may disqualify the relevant team member or the entire team without taking any steps to resolve the situation. 7. Participants in the Basketball tournament may file protests regarding any matters that may arise in the course of a match. Such protests must be submitted to the Head Referee for Basketball immediately after the match. The Head Referee for Basketball shall issue a final opinion immediately after the match. If a participant is not satisfied with the Referee s final decision, they may file a protest using the form available from the Event Office, which shall be reviewed by June 19th 2015.

5 Section 4 Competition schedule 1. The Basketball finals of the 2015 ORLEN OLYMPICS shall be held on June 13th 2015 in Orlen Arena, at Plac Celebry Papieskiej 1 in Płock, from 2pm to 6pm. 2. Team fixtures shall be determined by draw four days before the start of the finals. 3. If a large number of teams are submitted, the Organiser reserves the right to hold qualifiers, having notified all the participants. Section 5 Game rules 1. The matches shall be played in compliance with the rules of basketball and these Rules. 2. All players shall be required to wear sports attire, which they must procure by their own means. The starter kit shall only include a shirt. 3. Sports shoes that do not mark the floor shall be required. 4. There shall be five players per team. 5. A team shall start a match with a line-up of five players and shall finish with at least two players. Otherwise, the match shall be declared forfeit. 6. Players shall be supplied with distinctive markings (vests) to help distinguish between the teams. 7. Duration of a match: 2 x 7 minutes with an interval to swap ends of the playing field. 8. The result of a match must be conclusive, in the event of a tie a two-minute overtime shall be played to determine the winner. 9. The matches shall be played on a court in a sports hall. 10. The basket rings shall be set at the height of 3.05m from the floor. 11. The ball may be thrown, passed and dribbled with hands. 12. A player may move with the ball only if the ball is dribbled (bounced on the floor). 13. Dribbling shall be performed with one hand. Once a player catches the ball, he/she cannot dribble again. 14. Play should begin within five seconds from the referee s whistle. 15. If a player does not pass, throw or dribble the ball within five seconds from the whistle, it shall be considered a five-second violation. 16. Each team shall have 24 seconds to attempt a field goal and 8 seconds to leave their own half of the court.
6 17. A player shall not remain for more than 3 seconds in the 3 second field of the opponent when the ball is in control of his/her team. 18. A free throw shall be awarded for a foul committed against a player in the act of shooting; if the shooter is fouled and the goal is successful, the goal shall count and in addition a free throw shall be awarded. 19. Details of the competition shall be announced by the Organiser through corporate media, with a copy sent to the Ambassadors of each company, promptly after the list of participants is closed. 20. The final position in the qualifiers shall be determined by: the number of points (2 points for a won match, 1 point for a lost match); the result of a direct match between two teams; the ratio of goals scored to goals lost; a draw. 21. The Basketball matches shall be overseen by referees designated by the Organiser. 22. The matches shall take place in accordance with the schedule. A team that is not ready to play at the start of a match shall be disqualified. Section 6 Sportsmanlike conduct 1. For an infraction of the game rules or unsportsmanlike conduct, the referee may assess a technical or disqualifying foul to a player. 2. A player shall be ejected if he/she commits three infractions and free throws shall be awarded after three fouls are committed by a team. 3. A team member may be sanctioned by expulsion for: wilfully kicking, hitting or attempting to hit an opponent; any violent, aggressive attack; using offensive or vulgar language with respect to the referee or other players, showing dissatisfaction with the referee s decision with a word or gesture; committing another offence that the referee considers to be a manifestation of particularly unsportsmanlike conduct. 4. Only the referee shall be authorised to interpret conduct.
7 Section 7 Health care 1. The Organiser shall provide Personal Accident Insurance for participants of the Basketball tournament. 2. The Organiser shall provide first medical aid in the case of any injuries suffered by players during play; the Organiser shall not be held responsible in any manner whatsoever for participation in the tournament of persons with any medical condition (anything that causes them not be in full health) and for any resulting accidents or their consequences. Section 8 Registrations 1. Registrations shall be accepted until May 25th Registrations may be submitted: - electronically, using the online registration form, in accordance with the participant s instructions, or on paper, using the Organiser s original forms, delivered: a. by post, to the following address: PKN ORLEN, Biuro Komunikacji Korporacyjnej/Corporate Communication Office, Płock, ul. Chemików 7, with the following additional note: ORLEN OLIMPIADA 2015; b. personally to PKN ORLEN S.A. in Płock, ul. Chemików 7, building 07, room 804 or 805; 2. Filled in registration forms, sent in scanned form by , to the following address: olimpiada@orlen.pl, shall also be accepted. 3. Registrations submitted otherwise than as provided in Section 8.2 above shall not be considered. Section 9 Dissemination of image 1. Persons participating in the tournament agree to the use and dissemination (including public use) by PKN ORLEN S.A. and by the Organiser (to the extent of its cooperation with PKN ORLEN on the 2015 ORLEN OLYMPICS) of their image recorded in photographs taken during the 2015 ORLEN OLYMPICS, in all fields of use: in video footage prepared for internal publication on the
8 Intranet, on ORLEN Studio screens, in photographs intended to be used in inhouse media, as well as in brochures, advertising leaflets, or printed corporate materials, e.g. calendars (including also parts of such photographs or their lower resolution or compressed versions) for an unlimited period of time. 2. Participants of the 2015 ORLEN OLYMPICS are required to submit a declaration, in such form and manner as stipulated in Section 2.7 above, to the effect that they agree to the processing of their image and that the right to authorise dissemination of their image is not restricted by any third-party rights. When using such photographs, PKN ORLEN S.A. and the Organiser shall have the right to give the names and surnames of participants of the 2015 ORLEN OLYMPICS. 3. The photographs and other derived materials may be used by PKN ORLEN S.A. and the Organiser in PKN ORLEN S.A. s advertising and promotional materials, including materials distributed over the Internet and Intranet, at exhibitions, in brochures and booklets concerning their own work, or in the mass media, at any time and place, whether in Poland or abroad. 4. The rights specified in Section 9.1 above may be transferred by PKN ORLEN S.A. to other entities working with PKN ORLEN S.A. on organisation of the 2015 ORLEN OLYMPICS. Section 10 Miscellaneous 1. The Organiser may change the time of play if technical or organisational considerations so require. 2. These Rules shall come into force upon being published on 3. The Organiser reserves the right to change these Rules. The Organiser shall directly notify all participants of any such changes, as well as post an appropriate notice on 4. In matters not regulated in these Rules, the Rules of the Event shall apply. 5. In matters relating to the Basketball tournament which are not regulated in these Rules or the Rules of the Event, final decisions shall be made exclusively by the Organiser.

Chemików 7, KRS No ( PKN ORLEN ), and Allegro Sp. z o.o. Sp.k. as the Organiser, to the extent of its cooperation with PKN ORLEN on the 2015 ORLEN OLYMPICS, of my photographic, video or other images taken in connection with my participation in the 2015 ORLEN OLYMPICS, in all fields of use (including parts of such photographs or their lower resolution or compressed versions, as well as other modifications) and for an unlimited period of time. The above consent to use my image refers to the use by PKN ORLEN and the Organiser of the above-mentioned materials (photographs, video, etc.) in advertising and promotional materials, including materials distributed over the Internet and Intranet, at exhibitions, in brochures and booklets concerning PKN ORLEN or its activities, at any time and place, whether in Poland or abroad. I further declare that the consent expressed in this Declaration shall extinguish all of my claims that I could otherwise have against PKN ORLEN and other entities involved in organising the 2015 ORLEN OLYMPICS in connection with my personality rights. I further declare that my right to authorise dissemination of my image is not restricted by any third-party rights... first name and surname (in legible script) I hereby consent to the processing of my personal data contained in the registration form by the Administrator of personal data within the meaning of the Act on Personal Data Protection of August 29th 1997 (Dz.U. of 2014, item 1182, as amended), i.e. Polski Koncern Naftowy ORLEN S.A., with its registered office in Płock at ul. Chemików 7, for the purposes of organising the 2015 ORLEN OLYMPICS.

I am aware that I am prohibited from participating in the 2015 ORLEN OLYMPICS under the influence of alcohol or narcotics. I further represent that I have read the Rules of the 2015 ORLEN OLYMPICS, rules of the competition, and rules of the facilities where the competition will be held, and that I fully accept these rules and agree to observe them.
