1 Grayslake Fire Protection District Request for Self-Contained Breathing Apparatus (SCBA) Bid The Grayslake Fire Protection District invites interested and qualified parties to submit a bid for a Self-Contained Breathing Equipment (SCBA) package. Bid packages will be available for pick up at the District office located at 160 Hawley Street, Grayslake, Illinois beginning Monday, November 12, Bid packages may also be downloaded from the District website or ed by contacting Deputy Chief Dan Pierre at dcpierre@grayslakefire.com Two (2) original bid packages in the prescribed format must be addressed to Deputy Chief Dan Pierre in a sealed envelope clearly labeled SCBA Bid Package. The outside of the envelope must include the name, phone number, and address of the designated contact person. The envelope is to be delivered to the Grayslake Fire Protection District, Attn: Deputy Chief Pierre, 160 Hawley Street, Grayslake, IL by 12:00 noon (CST), on Monday, December 3, Submittals that do not meet the minimum requirements or deadline will not be considered. Bids shall be submitted in the same sequence as specifications for ease of evaluation, comparison, and checking compliance. Bid price shall not include any federal or state taxes. Bid price shall include shipping costs.

2 All questions about this bid package must be submitted via as follows: Deputy Chief Dan Pierre All requested clarifications to the bid package must be provided in writing via as indicated above, unless otherwise provided. No other responses whether verbal or otherwise, shall be deemed official unless in writing by the Grayslake Fire Protection District. All submitted questions shall be redistributed with response attached to all representatives who request a bid package. The safety of our personnel, warranty, service, customer care, and staff survey results will all be deciding factors in the bid selection process. The district reserves the right to reject any and all bid packages or waive any technicalities. Documents to be submitted with the Bid Package. 1. Copy of SEI certification to NFPA Copy of all warranties provided by the SCBA manufacturer. 3. A detailed and itemized list of the items being offered including the quantities and unit cost. 4. Detailed specification of the SCBA being offered. 5. A list of any exceptions to the specification. For each exception, please explain the details of the exception and offer, if available, an alternative feature. 6. Reference list providing a minimum of five (5) Illinois fire departments that have purchased the bid SCBA within the last 12 months. 7. Description of service and repair program, facility, and services including the scheduling or repairs and the annual maintenance program. 8. Description of the training that would be provided. 9. 3 SELF-CONTAINED BREATHING APPARATUS (SCBA) BID PACKAGE FORM GRAYSLAKE FIRE PROTECTION DISTRICT The undersigned vendor hereby proposes to furnish new, commercially produced Self-Contained Breathing Apparatus (SCBA) in accordance with the Grayslake Fire Protection District s specifications and the vendor/manufacturer s detailed specifications incorporated herein. Any exceptions to the Grayslake Fire Protection District s specifications are listed on the following exception page(s). It is agreed by the undersigned that the signing and delivery of this bid package represents the bidder s acceptance of the terms and conditions of the attached specifications and provisions and, if awarded this contract, will represent the agreement between the parties. Failure to deliver all equipment in the time quoted above, without the expressed permission of the purchaser will constitute a breach of contract and said agreement may be voided.

4 Compliances: All SCBA and accessories shall be approved by the National Institute for Occupational Safety and Health (NIOSH), under 42 CFR, Part 84 for chemical, biological, radiological, and nuclear protection (CBRN) with a 45-minute rated service life and compliant with all requirements of the National Fire Protection Association (NFPA) 1981, Standard on Open-Circuit Self Contained Breathing Apparatus (SCBA) for emergency Services, 2013 edition and NFPA 1982, Standard on Personal Safety Alert Safety Systems (PASS), 2013 edition. Detailed Specification: The following represents the District's minimum requirements for the purchase of Self- Contained Breathing Apparatus (SCBA). Any alternate products offered must meet or exceed these requirements. Vendors interested in proposing alternative products shall submit detailed documentation on the proposed equipment with their bid package submittal. The fire district will have the final decision as to whether alternate equipment is acceptable. Delivery and Documentation: The order is to be shipped to the Grayslake Fire Protection District, 160 Hawley St., Grayslake, IL with the vendor paying all shipping costs. The order is to be delivered in entirety as one package on the same date the Fire District will not accept partial shipments. Delivery must be during the District s normal office hours of Monday Friday between 8:00 am - 4:00 pm Documentation to be supplied at the time of delivery includes a quality assurance test report for each individual SCBA delivered and four (4) copies of the following: User manual for each component. Care and maintenance instructions for each component. Warranty for each component. The District reserves the right to inspect any and all materials, components, equipment, supplies, services or completed work specified herein. Any of said items not complying with these specifications are subject to rejection at the option of the District. Any items rejected shall be removed from the premises of the District and/or replaced at the expense of the vendor. Training: The vendor shall provide training for all members of the District. The training shall instruct all Fire District personnel in the operation and maintenance of the SCBA and shall occur for each of the three (3) shift battalions on days mutually agreeable to both the bidder and the Fire District. The vendor will also supply training for (2) two persons at the Grayslake Fire Protection District Fire Station One (160 Hawley Street, Grayslake, IL 60030) by a certified technician of the supplied SCBA units and facepieces. The technician will instruct Grayslake Fire District designated employees on how to perform basic level repairs that can be completed in house. The date will be mutually agreeable to both bidder and the Fire District but must be within 180 days of receipt of equipment unless extended or waived by the Fire Chief.

5 The Grayslake Fire Protection District intends to purchase the following quantities of SCBA, spare cylinders, face pieces, and accessories: (36) Thirty-Six Self-Contained Breathing Apparatus (SCBA) sets consisting of 1 complete and fully operational SCBA unit (including battery if applicable), two (2) cylinders, and one (1) Facepiece. The cylinders must be 45-minute, 4500 PSI carbon fiber cylinders with quick connect. Custom Grayslake Fire Protection District logos monogrammed into each of the Carbon Fiber bottles. The Grayslake Fire Protection will provide the artwork in a high-quality, electronic format. Unique numbers monogrammed into each of the carbon fiber bottles in the same location for all bottles to utilize for tracking purposes (26) Twenty-Six additional Face pieces with cloth/kevlar style head harness (65) Heavy duty mask bags (6) Quick connect adapters for threaded connection filling stations (If Applicable) (1) Fit test adapter for OHD Quantifit 3000 fit test machine (2) Tool kits with an assortment of items used to service user serviceable items on the SCBA s and Face pieces in house OPTIONAL EQUIPMENT TO BE INCLUDED (If applicable or available) (3) Charging stations for rechargeable batteries (9) Spare battery packs (1) Radio Frequency ID tag/id card reader (40) Radio Frequency ID tag/id card

6 SPECIFICATION Must check either Compliant or Exception for each line item 1. NFPA Standard: COMPLIANT: EXCEPTION: All requirements of NFPA 1981 Standard on Open-Circuit Self Contained Breathing Apparatus for emergency services 2013 Edition are listed in this document by reference. All SCBA units in their entirety (including facepieces, if applicable) must be made compliant with NFPA Edition no longer than 120 days after the release of the new 2018 standard. The update will be made by factory authorized service technicians on-site with no additional charges. The price to make these 2013 Edition SCBA systems complete will be included in the bid. 2. SCBA Model: COMPLIANT: EXCEPTION: The proposed SCBA model and facepiece must be the latest and best model offered by the manufacturer and must still be in production. 3. Facepiece: COMPLIANT: EXCEPTION: Facepiece will be offered in multiple sizes, according to fit test results done prior to order. 3. Voice Amplification COMPLIANT: EXCEPTION: While SCBA is worn, every user will have voice amplification capabilities. 5. Cylinder: COMPLIANT: EXCEPTION: All SCBA bottles in the bid will be 45-minute 4500psi, carbon fiber cylinders. All cylinders will have a quick connect fitting and be adaptable to fit current SCBA fill stations 6. Backframe: COMPLIANT: EXCEPTION: The SCBA backframe will be rigid and made of composite or metal material for strength and durability. 7. Cylinder Retention: COMPLIANT: EXCEPTION: All carbon fiber cylinders will be attached onto the SCBA backframe with a metal locking band, or webbing style strap and composite constructed lock.

7 8. Indicator Lights: COMPLIANT: EXCEPTION: All operation indicator lights will be visible to the user from all peripheral angles at all sides of the SCBA. 9. Power Supply: COMPLIANT: EXCEPTION: For ease of maintenance and troubleshooting, the power supply for ALL electronics will be in a central location integrated into the backframe or mounted on the backframe. The energy cells will be rechargeable or disposable and all cells will be of the same size and type. 10. Waist Belt/Lumbar: COMPLIANT: EXCEPTION: For quick removal, the waist strap must be a seatbelt style, quick release buckle. With consideration to user comfort, the lumbar section will be of a swivel type. 11. Shoulder Straps: COMPLIANT: EXCEPTION: Shoulder straps should connect to the backframe, not the waist strap. Low pressure hoses and any electronic wiring will run through the shoulder straps. A hooking D ring should be provided on either the right or left shoulder strap to hook accessory items. Shoulder straps should be field removable in order to be laundered in the Districts current extractor gear washing machines nd Stage Regulator: COMPLIANT: EXCEPTION: The second stage mask mounted regulator will be equipped with a dual action design and have a quick disconnect hose coupling to separate from the frame assembly for ease of changing. 13. Shoulder Gauge: COMPLIANT: The shoulder air pressure gauge/pass alarm will have an analog pneumatic gauge. In the event of an electronics failure, the user will have the ability to check air pressure using the pneumatic gauge. No exception. 14. EBSS Buddy Breathing: COMPLIANT: EXCEPTION: The SCBA should be equipped with an EBSS buddy breather system that is secured to the backframe or lower harness with a heavy-duty pouch utilizing snaps as a minimum.

Heads Up Display: COMPLIANT: EXCEPTION: The heads-up display will consist of a series of different colors and number of lights easily visible and understandable to the wearer giving a visual indication of available air pressure remaining. The lights should be located on the second stage regulator or facepiece. 16. Warranty: COMPLIANT: EXCEPTION: A minimum of a 10-year warranty is required on all components included in the SCBA system (except consumables). 17. Vendor/Service: COMPLIANT: EXCEPTION: For quick service of emergency repairs, the Bidder shall have a service or repair facility within 50 miles of the Grayslake Fire Protection District Station 1, located at: 160 Hawley Street, Grayslake, Illinois The bidder shall offer on-site repair as an option for quick turnaround of SCBA removed from service. Loaner SCBA shall be provided by the bidder if SCBA s are removed from service due to manufacturer defects. 18. Fit Test Adapter: COMPLIANT: EXCEPTION: (1) One adapter should be provided to connect the face pieces to an Occupational Health Dynamics 3000 Fit Test machine 19. Mask Bags: COMPLIANT: EXCEPTION: (65) Sixty-five heavy duty mask bags will be provided that properly and securely hold provided face pieces. These mas bags should have a metal, self-locking clip for attaching to turnout gear or SCBA harness. 20. Custom Monograms: COMPLIANT: EXCEPTION: Each air cylinder should be custom monogrammed with the Grayslake Fire Department patch. Each air cylinder shall also include unique numbers monogrammed into the cylinder in sequential order beginning with 01. The numbers shall be located in the same location for each cylinder.

Tool Kit: COMPLIANT: EXCEPTION: (2) Two tool kits should be provided that include various pieces that are user serviceable and can be repaired by fire department personnel trained by the vendors certified technician 22. Cascade Adapter: COMPLIANT: EXCEPTION: N/A: (6) Six quick connect adapters to connect threaded cascade system fittings to SCBA
