2 Section 1 Regulations and Guidelines Any person, organisation (diver or not) employed by, subcontracted to or working in conjunction with Diving Services UK will at all times adhere to the following: 1. Good safe working practice. 2. Conform to HSE at Work Regulations Conform to relevant ACoPs and HSE diving regulations. 4. Will be qualified, fit and competent to carry out a required task. 5. Will inform the supervisor or Mr. David Shaw if, for any reason, they are unable to carry out a task. 6. Will report immediately any defective equipment to the supervisor. 7. Will comply with Mr. David Shaw s instructions. Section 2 Letter of Appointment Mr. David Shaw is the proprietor of Diving Services UK and on behalf of the company Mr. David Shaw can also act as a Diving Supervisor at any time. Diving Services UK will act as the Diving Contractor. Mr. David Shaw may appoint a nominated diving supervisor other than himself at any time. Mr. David Shaw may at any time appoint any member of the dive team, who has the correct qualifications and experience, as the diving supervisor. The diving supervisor may not be changed during any dive but may be changed during any one day. Section 3 Letter of Appointment from client To conform to HSE DoWR, Diving Services UK are required to be formally appointed as Diving Contractors. This necessitates a Letter of Appointment to be completed for each job and should include the below: Client details Confirmation of Diving Services UK to be the appointed diving contractors Confirmation that all diving operations will conform to DOWR and ACoPs HSE - Heath & Safety Executive ACoPs - Approved Codes of Practice DOWR Diving Operations at Work Regulations 1997 DSUK Company Rules 2

3 Section 4 Diving Supervisor will: a. Be responsible for the safety of all of the members of the diving team. b. Conform to current diving regulations and ACoPs, ensuring that qualifications are correct and in-date. c. Be responsible for ensuring that the site-specific risk assessment has addressed all hazards and that the project plan is carried out. d. Fill out dive sheets and log books. Their signature on the dive sheets will be proof that they are the appointed Diving Supervisor. All log sheets must be signed and dated by the supervisor. e. Ensure good working practices and communications are maintained with other departments or industries that may influence or jeopardise the safety of the diving team or are involved within the diving operation. f. Ensure that other departments or industries, if they are to be used during any dive, are fully aware of their involvement and have been briefed prior to any diving operations. g. Ensure that all required equipment is available (diving, rescue and first aid) and that all equipment is certified and in good working order. h. Have read, understood and have a copy of the HSE Diving Operations at Work Regulations that is applicable to the type of operation. i. Use standard US Navy air decompression tables and ensure that during any decompression dives the divers carry out an additional stop at five metres for three minutes. j. Not exceed 10 metres per minute ascent rate. k. Not exceed dive time of 240 minutes per diver per day. l. If at any time, circumstances change during any dive, the Diving Supervisor may terminate the dive until the problem has been solved. NOTE: The diving supervisor is responsible for the health and safety of the dive team. NO OTHER PERSON, CORPORATE BODY OR ANY OTHER ORGANISATION CAN IMPOSE DURESS ONTO THE DIVING SUPERVISOR FOR TERMINATING A DIVE. The Diving Supervisor will ensure that: i) If modified equipment is to be used by any member of the diving team it should be thoroughly tested and the diver properly trained in its use. It should be of sound construction and incorporate a secondary independent redundant gas supply that should be available to the diver in case of any malfunctions with the primary. 4 ii) iii) iv) All dives are recorded in daily dive log and divers log books are filled in each day. The Diving Supervisor has complete control or lock out over any differential pressure areas (gates, weirs, propellers, cutting, burning, welding, etc.). Divers are protected from possible overhead or submerged injury. v) Good communication systems are established and maintained throughout each dive. Although due to the nature of the specific type of work, communications and life lines may hinder or reduce the ability of the diver to carry out a task (ie. Rescue), it must be thoroughly investigated if the divers are not going to use communications. Adequate alternative arrangements must be put in place. vi) Divers must have an adequate air supply to carry out a task. Although, due to the nature of the specific type of work, it may not be possible for the diver to have an independent redundant alternative air supply. vii) All the diving team and any other organisations involved are familiar with any rescue or emergency techniques and protocols that may be required and the dive team has thoroughly practised the required procedures with any other organisations that may be involved. 5 Section 5 Divers All divers will comply with the following: 1. Any diver who is employed by or subcontracted by Diving Services UK ( DSUK ) must have their own personal injury/disability/liability insurance policy and produce it before commencing work with DSUK. 2. DSUK will not be responsible for any claim of loss of earnings, etc. howsoever caused. 3. Divers must conform to Diving Supervisor s instructions at all times. 4. Divers must inform the supervisor if they are not capable or are unsure of any task asked of them, whether diving or other. 5. Divers must ensure that they are fit to carry out heavy manual work. 6. Divers must ensure that their original qualifications are valid and available as below: Diving Qualification HSE Diving Medical First Aid At Work Certificate Others (task-specific) 7. Divers must record every dive in their log books and have it countersigned by the diving supervisor. DSUK Company Rules 5

6 Section 6 Equipment 1. Diving Services UK is responsible for equipment used by DSUK staff. Any equipment hired to other parties falls under their responsibility for the duration of the hire. Any crew using equipment must be appropriately qualified and/or familiarised with the correct usage. 2. Diving Services UK will familiarise Rentors on the proper use of buoyancy equipment, water protection and SCUBA equipment if required. Diving Services UK is not responsible for the supervision of hired equipment during an on-hire period. It is the responsibility of the Rentor to inspect and be familiar with all equipment prior to use. 3. All equipment must be returned to Diving Services UK in the same condition under which it was hired. Any damages or losses must be reported at the time of off-hire and will be charged accordingly. 4. Diving Services UK is not responsible for any personal injury, property damage or wrongful death resulting from renting and/or using the equipment, however caused, including but not limited to product liability or negligence of Diving Services UK whether passive or active. Section 7 Credit Diving Services UK endeavour to provide the best services for productions water projects helping to ensure they are safe and efficient. 7 Section 8 HIRE OF U STAGE U Stage is an underwater filming facility and there are certain special considerations to take into account prior to shooting: 1. The staff at U stage are on site to advise and assist production. 2. We strongly recommend tech recces for all productions to the stage. 3. There are specific health and safety requirements in the stage which production must be aware of: STAGE Downstairs is a dry area. Access to tank level is upstairs. This is a wet area. All electrical systems must have risk assessments and project plans completed by an appointed competent electrician. Working at height guidelines apply. DIVING The diving supervisor is in charge of all activities in the water regarding safety and all activities that may affect persons in the water. All diving activities fall under the Diving At Work Regulations of Operations fall under HSE guidelines of a maximum dive time of 4 hours in total per diver per day. Dive teams work a 10 hour day from call time Monday to Friday. Any time outside these hours is charged at Time and ½. Artists using SCUBA must have in date HSE Diving Medicals and must be allowed a sufficient amount of time for training variable depending on the task prior to shooting. A post-shoot no-fly time of 18 hours must be allowed for artistes using SCUBA. 4. U stage water There are 1.2 million litres of clean clear water heated to 30 degrees centigrade in the tank. The water turns around in the ultra-violet filtration system every 4 hours Charges may be incurred if the water is contaminated and the quality of the water is reduced during a shoot. Productions should take into account the fact that the water quality will be reduced if fabric, construction materials and plant are introduced into the water during a shoot or if there are a lot of different rigging set-ups during a shoot day. At the request of production during a shoot the dive team may work overtime at the end of the day to clean the water for the next day s shooting. We also recommend setting up for the next day s shoot the night before when possible. DSUK Company Rules 7
