1 00935 UCSF Mount Zion Campus November 4, 2011 MR. BERT LUISTRO PROGRAM MANAGER UCSF - MOUNT ZION CAMPUS OFFICE OF ENVIRONMENTAL HEALTH & SAFETY 50 MEDICAL CENTER WAY San Francisco, CA Wastewater Enterprise Collection System Division 3801 Third Street, Suite 600 San Francisco, CA Telephone: (415) Fax: (415) SIC/ID: 8062/00935 SUBJECT: WARNING NOTICE Dear Mr. Bert Luistro: The recently collected grab and composite samples from five waste streams which discharge from UCSF Mt. Zion Hospital into the S.F. sewage collection system. All of the sampling and analyses were performed in accordance with procedures established by the Environmental Protection Agency (EPA) pursuant to Section 304 (g) of the Clean Water Act, and contained in 40 CFR Part 136, and amendments thereto. The enclosed summaries of analytical results show that on one day (Oct. 6, 2011) during the monitored period, the discharge from Site D was out of compliance with the City and County of San Francisco s discharge limit for lead (1.5 mg/l). Those limits are contained in Section 123(a) of Chapter X (Public Works Code) of Part II of the San Francisco Municipal Code, Article 4.1, and in the City s Department of Public Works Order No You are hereby required to ensure that your facility immediately ceases the discharge of wastewater with pollutant levels in excess of the applicable limitations. If you have any questions about the information provided, please contact Dr. John Gregson at Sincerely, Bruce Seale, Acting Manager Pretreatment Program Wastewater Enterprise / Collection System Division Enclosures
