1 2015 SCA Cheerleader Camp McNeese State University, Session II June 5,6,7 Thank you for choosing SCA for your cheerleader camp training. We are looking forward to working with you and your squad. Please read the following information carefully and make copies of this form for each camp participant. EVALUATION/COMPETITION (IMPORTANT) Please understand that the preparation of a cheer and/or dance for SCA camp is optional. You may choose to bring one, both or neither. As an advisor/coach SCA encourages you to simply do what works for you and your squad. OPTIONAL: Cheer evaluation first night of camp, perform one home cheer. Building, jumps, and tumbling may be incorporated. The home cheer will be evaluated only on the first night and will not be repeated for final competition on the last day. Please keep this is mind as you prepare your home cheer. OPTIONAL: Dance competition first night of camp, must perform dance routine to music not to exceed 1 minute 15 seconds. Tumbling and building are not allowed. BALANCE PAYMENT Final payment is due 14 days or two weeks prior to camp. Payments must be made with a single school check, money order, or cashier s check.please mail a copy of your invoice along with your balance payment. Make certain that SCA has received your camp application form by fax or so that we have all the necessary contact information. Each adult attending camp will receive a name badge, please make certain that we have all of the names of the adults attending. REGISTRATION/HOUSING Report to the Residence Life Club Room, Friday, 10:30-12:30 p.m. Commuters report to the MSU Recreation Center at 1:00 p.m. for the opening session. One key deposit of $45.00 will be collected at camp, however; a charge of $45.00 will be assessed for each lost key by the university. SCA will accept checks. One designated person should collect all keys and turn them in ASAP during check-out. Any charges for lost keys will be collected at check-out! The dormitory is air-conditioned and you are required to bring linens. The dormitory is air-conditioned and you are required to bring linens. The suites are 6-person and 2-person suites. Each suite has complete bathroom facilities. The hall has washers and dryers as well as vending machines. IMPORTANT: MSU has limited resident housing space and we must get maximum use out of the space available. For example, please understand that if your squad has a total of 13 participants they would get two 6-person suites and an extra mattress would be added for the 13 th person The hall is supervised by MSU resident assistants as well as SCA staff. SCHEDULE Opening session, 1:30 p.m., classes immediately following. All classes in the MSU Recreation Center. SCA encourages you to bring creative signs to showplace in the Rec Center! See page 2 for a detailed schedule. MEALS Six meals are served during camp, starting with dinner the first evening and ending with lunch the last day. EMERGENCIES: Campus Police (337) GUESTS No dates or over-night visitors. Guests may only visit camp at the specified times. Make sure this is fully understood with friends and relatives before your arrival. Violation of this regulation may lead to your dismissal. WHAT TO BRING Linens,sheets,pillow,blanket,towels and washcloths.practice clothes: See reverse side of this form. Water bottle, poms, spending money, change for vending machines. Do not bring jewelry or valuables. It s a good idea to pack a SQUAD FIRST AID KIT : Band-aids, ace-bandage,zip-lock bag for ice or instant cold pack, aspirin, and throat lozenges. MASCOTS: Bring your official mascot suit if possible, a one minute skit which defines your character through pantomime and antics. Bring markers, poster paper, and a CD player and any potential props! It is imperative for all mascots to come prepared with all material! Your mascot home routine will be performed on the first night. ADVISOR/COACH PROGRAM: Please bring a copy of your tryout score sheet and any DVD of pep rallies or any special performances. If your squad competed this year please bring a DVD of the performance. CAMP STORE T-shirts, sweatshirts, shorts, and patches are a few of the items that will be available for your camp mementos. Also, SCA will again be producing a CD with the 2015 SCA cheers, sideline chants, and music for the SCA dance routines. PAID BIDS AT SCA SUMMER CAMPS FIRST PLACE TEAMS in the Junior High and High School Home Cheer and Dance divisions, will be awarded exclusive PAID BIDS to the 2016 BATTLE AT THE CAPITOL Championship; a unique end-of season event. The Junior High and High School winners in both the Home Cheer and Dance divisions at summer camp will receive paid bids to bring their routines to BATTLE at the CAPITOL! So bring those routines and BATTLE hard because the 2016 BATTLE AT THE CAPITOL is January 10 th!

2 ATTENTION: ADVISORS, COACHES, CHOREGRAPHERS, PARENTS PARENTS/SPECTATORS Parents and spectators are invited to watch the home cheer evaluation and dance competition at 6:45 p.m. on Day I. On Day III guests are invited to watch the All American competition beginning at 3:00 p.m. followed by the squad final evaluation/competition beginning as early as 3:30 p.m. CHECK OUT (FOR RESIDENT PARTICIPANTS)Dorm check-out is scheduled for approximately 1:00 (after lunch) on Day III. If your squad transportation is not available at that time luggage may be stored in the advisor room until after the awards ceremony. That way all keys except one will be turned in during lunch. The advisor key will then be returned individually at the end of camp. Most squads like to wear their uniform on the first night for the dance competition/cheer evaluation and on the last evening for final competition/evaluation. You may also choose to wear your uniform the evening of Day II for evaluation as well but this is totally optional! Plan on two or three matching outfits If a squad is planning on purchasing an SCA camp t-shirt you may want to include this as one of the matching outfits. Do not worry if your squad will not have a uniform available for camp, many squads do not and will just need more matching outfits. Also, washing machines and dryers are available so your squad can wear an outfit more than once. Please remember that this is just a suggestion. You may of course choose to do something differently.

3 PARENT S MEDICAL RELEASE FORM: Each student attending camp must bring a parent release form giving the medical center permission to treat the student should an accident occur. SCA does not provide special camp insurance for participants and will not assume any financial responsibility for injuries. MCNEESE STATE UNIVERSITY RELEASE FORM CONSENT, WAIVER, RELEASE AGREEMENT I, the undersigned parent and/or legal guardian of, allow my child to participate in the activities of McNeese State University and/or (camp name), including but not limited to on campus events and scheduled off campus events. I do hereby release and discharge McNeese State University and/or (camp name) representatives from any and all damages on account of any injuries or illnesses sustained to or by my child while engaged in such activity at McNeese State University and/or (camp name), whether related or not to the activity enumerated above. I understand the risk of injury may be similar to sport types of injuries like heat exhaustion, falls, pedestrian accidents or even death. This agreement shall constitute a bar of any recovery by the undersigned individually or brought for and on behalf of the child, and said agreement may be urged and used by McNeese State University as a bar to any recovery by the undersigned or by the child in any suit or claim instituted on account of any injury or illness sustained by my child while engaged in the activities of McNeese State University. HOLD HARMLESS AND IDEMNIFICATION AGREEMENT I, the undersigned, release and discharge McNeese State University and/or (camp name) representatives from any and all liability from any and all claims or damages from any accident or illness sustained to or by my child while engaged in the activities of McNeese State University and/or (camp name). I agree to hold harmless and indemnify McNeese State University and or (camp name) representatives against any loss, damages, or cost of whatsoever nature including expenditure of attorney s fees which may be suffered as a result of any action, claim or demand by my child or my child s heirs, by me, my heirs, successors, or assigns, or by any other person on his/her own behalf or for the benefit of the child. MEDICAL RELEASE FORM AND INDEMNITY AGREEMENT I, parent or guardian of, hereby acknowledge that as a part of the activities of my child, attending (camp name), that there is the possibility my child may need to receive medical attention due to injury or accident I understand that McNeese State University and/or (camp name), or its representatives will make a reasonable effort to contact me in the event of injury or accident to my child based on the circumstances. In the event that McNeese State University and/or (camp name), or their representatives are not able to contact me, or if the need for medical care appears to be immediate, then I instruct and authorize McNeese State University and/or (camp name) representatives to consent to and authorize reasonable and necessary medical treatment for my child. I further agree to release McNeese State University and/or (camp name) and their representatives from any liability for their efforts to secure reasonable and necessary medical treatment for my child as stated above. I, the undersigned parent or legal guardian shall assume full responsibility for all medical bills, including doctor and/or hospital bills incurred by my child that are not covered by the (camp name) accident policy. I further agree to reimburse McNeese State University and/or (camp name) and their representatives who may incur expenses in the treatment of an accident or illness of my child.
