Minimal equipment is required and rental equipment is available. Organized and instructed by former professional Mudbugs hockey staff. The fun begins May 15th and runs until mid August. 2 Rules & Information All players must fill out and sign a permission and waiver form. No exceptions. Minors must have parents or legal guardians sign permission form and waiver. When and Where: Sunday afternoons. Adaptive Floorball 12:00pm Youth ages :00pm Junior ages :00pm Adults 3:00pm. 3 League Structure: We have one league: Recreational and Competitive combined into recpetitive. This league is designed for everyone from the beginner to the skilled veteran. This league is instructional at times and skilled players will assist. There is absolutely no body checking allowed. This is laid back and strictly for fun. Veteran skilled players will be allowed to play each other hard but will not be allowed to take advantage of beginning players. Slap shots are allowed. Helmets and shin guards are required. Adult (ages 15+) Ages 13+ are allowed to play with adults with parental permission and if the player is large enough and skilled enough to compete. The Junior Division is recommended for players Each team should expect to play once per Sunday. Required Equipment: Must wear a certified ice hockey helmet. Any player under age 18 must wear a full cage or full visor. A half visor for adults is recommended. Gloves Ice hockey, street hockey or lacrosse gloves Stick any regular ice/roller hockey stick. Running or Court shoes Shin guards soccer or street hockey type. Ice hockey shin guards are fine. Not Required but recommended safety equipment Protective cup Elbow and knee pads Mouth piece Normal indoor athletic wear is recommended such as shorts or sweat pants and t- shirts. Impact 864 has a limited supply of new and used hockey equipment for rent or sale including helmets, gloves, and sticks. For more information contact Tommy Impact 864. We also have sticks, gloves and shin guards for sale. Goalie Equipment Required Certified goalie leg pads for ice or street hockey, chest protector, helmet with a cage, catching glove, blocker, and stick. All goalies must wear a protective cup. Goalies must use a goalie stick. Impact 864 has basic goalie gear on site for anyone to use.
4 Teams: We play 3 vs 3 plus goalies. Teams shall consist of approximately 7-10 players (including goalie(s)). Teams are formed by assignment. All divisions and teams will be co- ed. Family members and couples can request to be together on the same team but no guarantees are made. Any person on a team roster can play goalie. Impact 864 will help provide goalie gear for teams/players that do not have goalie gear. Teams are not permitted to trade players or use substitute players from another team unless cleared by the Impact 864 competition director. Visitors: We encourage people to try the sport. A visiting player can play one game for $10 the first time, $15 the second time, and $20 the third time. If a visitor decides to join the league the season rate will be prorated. No visiting players will be allowed in the playoffs and Rink set- up: We will play one court with 4 players per side (including goalies). Each team will have approximately 8-10 members. The rink is approximately 100 L x 50 W. Goals are standard 6 x 4 Game length: (3) 12 minute periods with two (2) 2- minute intermissions Win = 2 points Overtime or Shootout loss = 1 point Regulation loss = 0 points Tiebreaker will be determined by 1) Regulation/Overtime wins, 2) head- to- head record, 3) goal differential 4) goals for 5) goals against Overtime: In case of a tie game at the end of regulation, after a 2- minute break, the game will resume with a 5- minute, sudden death overtime period. If the score is still tied after the overtime period, the teams will participate in a shoot- out. Shootout Three (3) players from each team will alternate shooting at the goalie. Whichever team makes the most goals wins the competition and their team is awarded the deciding goal. If after one round the score is still tied, the shootout will continue with other players until we have a winner. The Home team decides which team shoots first.
5 Penalties: All minor penalties will result in a 1 minute powerplay. A double minor will result in 2 minutes of powerplay. A player will be ejected for a major penalty (4 minutes) and may be subject to further disciplinary action. Minor Penalties include tripping, hooking, holding, interference, and minor roughing, slashing, high sticking, etc. Major Penalties include fighting, severe roughing, spearing, butt- ending, and any intention to harm another player. Accidental High- sticking resulting in blood will result in a double- minor penalty. Abuse of referees or officials will result in a minor penalty. Continuous abuse will result in a game misconduct, a major penalty, and may be subject to further disciplinary action. CONDUCT: Players: We re all in this together and most of us have played together for years. Keep your emotions in check and remember it s just a game. Respect the game and also respect the rink by cleaning up after yourselves. Fans, Parents, and Friends: Don t embarrass yourself and the players by acting like a fool. Cheer them on and be supportive. I have no problem asking someone to leave who is causing problems. Other information: Players will sweat a lot. Drink plenty of fluids the night before and throughout the game Players are allowed to bring water and sports drinks. The facility does not sell refreshments. Please clean up after yourself. 7 Basic DEK Hockey Rules The object of the game quite simply is to strike the ball with the hockey stick and knock it into the opponent's hockey net (5 feet wide x 4 feet high). Typically, a low- bounce type of ball is used. We use a Mylec or D- Gel regulation ball. For added safety, hockey gloves and helmets are recommended, and in fact are mandatory for play in our member leagues. Any size or type of non- slip flooring can be used as a playing field. Impact 864 league games are usually played at Hot Wheels South in Bossier City on a painted hardwood floor measuring 80 x 50. Extra players are kept on each bench, outside the playing surface, and interchanged with the players on the floor either during play or at a stoppage of play. The following are additional rules: Face- offs are only conducted at the beginning of each period. After a freezing of the ball by the goalie, a goal scored or a ball brought back into play form out of bounds, the team processing the ball has three (3) seconds to bring the ball in play from behind the goal/base line. Opposing players cannot attack the ball until after the three seconds expires or until the ball is played across the bass line. Penalties are called when a player commits a foul. Every penalty will result in a penalty shot. Boxing out and minimal body contact is allowed. Other penalties such as tripping, slashing, cross checking, interference, and roughing will be enforced. No offsides and no icing/flooring. To score a legal goal, it cannot be kicked in; nor struck with a stick above the shoulders; nor pushed in using the hand, nor while offside. Slap shots are legal. (competitive only) A goal can be scored from anywhere in the playing surface. Each team will be allowed one (1) 30 second time- out per game. We will play with a running clock. The clock will only stop for injuries. If the score is within 2 goals, the clock will stop every time play is interrupted in the last minute.

9 WAIVER AND PERMISSION FORM In consideration of my participation in the Impact Dek Hockey League and any related activities (collectively, the "Event"), wherever the Event may occur, I agree to assume all risks incidental to my participation (which risks may include, among other things, muscle injuries, and broken bones, etc.). I hereby release, covenant not to sue, and forever discharge the Released Parties (as defined below) of and from all liabilities, claims, actions, damages, costs or expenses of any nature arising out of or in any way connected with my participation in the Event or any such activities, and further agree to indemnify and hold each of the Released Parties harmless from and against any and all such liabilities, claims, actions, damages, costs or expenses including, but not limited to, all attorneys' fees and disbursements up through and including any appeal. I understand that this release and indemnity includes any claims based on the negligence, action or inaction of any of the Released Parties and covers bodily injury (including death), property damages, and loss by theft or otherwise, suffered by me either before, during or after such participation. I declare that I am physically fit and have the skill level required to participate in the Event and/or any such activities. I further authorize medical treatment for me, at my cost, if the need arises. For the purposes hereof, the "Released Parties" are Impact 864, all Event sponsors or charities, and each of their respective parent, subsidiary, affiliated or related companies; and the officers, directors, employees, agents, members, representatives, successors, assigns and volunteers of each of the foregoing. I further grant the Released Parties the right to photograph and/or videotape me and further to display, use and/or otherwise exploit my name, face, likeness, voice, and appearance forever and throughout the world, in all media, whether now known or hereafter devised, including, without limitation, digitized images, whether for advertising, publicity, or promotional purposes, including, without limitation, publication of Event results and standings, without compensation, reservation or limitation. The Released Parties are, however, under no obligation to exercise any rights granted herein. This Waiver and Permission Form shall be governed by the laws of the State of Louisiana without regard to conflicts of laws principles. Any legal proceeding of any nature brought by either party against the other to enforce any right or obligation under this Agreement, or arising out of any matter pertaining to this Agreement, shall be submitted for trial, without jury, before the First Judicial District Court in and for Caddo Parish, Louisiana. The parties consent and submit to the exclusive jurisdiction and venue of such court. I HEREBY WAIVE ANY RIGHT TO A JURY TRIAL WITH RESPECT TO ANY CLAIM RELATING TO THE EVENT.
