Must have been a 4-H member for a minimum of six years. 2. Applicant must be a graduate of Owen County High School or live in Owen County and have been home schooled. 3. Application must meet the minimum requirement of 200 points. 4. Applicant must plan to carry at least twelve hours and be considered a full-time student. 5. Applicant must submit a complete record of grades as follows: a. Junior year grades and GPA and grades and GPA for three terms of the Senior year. Class standing. b. Grades must reflect a C average. 6. The scholarship shall be awarded to a freshman for one year. Recipient must submit a copy of letter of acceptance to the 4-H Council to receive $ scholarship. 7. Recipient shall be recognized at Owen County High School Awards Ceremony.

2 4-H Scholarship Form I. PARTICIPATION IN 4-H PROGRAMS, PROJECTS AND ACTIVITIES a. Length of Membership: You must be a current 4-H member and have been enrolled in 4-H for six years, including current year: Number of years in 4-H: x 2 points for each year = TOTAL SCORE b. Programs/Projects: List your 4-H projects by Membership year. (Score 2 points for each) Maximum score 30 points. 3 c. 4-H Communication Activities: Identify the project or subject title about which a demonstration/speech/resume was given corresponding with the year of presentation and the number of times presented on each level. Demonstrations and speeches reported here should have been presented to some organized group. Practice times should not be reported. Score 4 points for each time an activity was given at the county (C), area (A), or state (S) level. Maximum score 80 points.
