There is increasing recognition of the problems and the areas where the most impact can be made and Hand Hygiene is top of the agenda. No-touch products have been identified by the NHS as offering significant benefit subject to the reliability of the specified products. This is a very important aspect for all washrooms, but of increased importance for hospitals where cross-contamination needs to be avoided. Reliability DVS automatic taps have been extensively developed and tested to achieve the current high levels of reliability. Dean Hazell of James Paget Hospita commented,l "Dart Valley Systems automatic taps have been totally reliable and are good value for money. Whilst the main consideration is hygiene to avoid cross contamination, automatic taps also make significant savings on water". James Paget Hospital now have over 600 taps and have experienced outstanding performance from DVS taps. The hospital is also the most water efficient in their sector, medium acute outside London. 2 HTM and HBM Compliant Products Hands-Off Approach to Water savings and Associated Infections (HAI) are both Government and NHS Priorities. There are cost-effective solutions that can make significant contributions to improving hygiene and also reduce water consumption. Dorset County Hospital trialled DVS no-touch WC Flushvalves and no-touch taps. The water savings and the respose from the infection control team were very positive. The Trust concluded that it was a worthwhile investment to change all hand wash taps and all WC flushing devices throughout the hospital to no-touch. With the installation of no-touch clinical hand wash taps, the reduction in water costs will pay for the investments which will make an important contribution to reducing HAI. A major proportion of avoidable hospital-acquired infections are transferred to the patient via the hands of carers. The two most important aspects of improving hand hygiene are to increase the frequency and improve the effectiveness of hand washing. No-touch taps will help to achieve both of these objectives. Dr Peter Wilson, consultant microbiologist, University College Hospital, London, said: "Traditionally, elbow-operated taps have been used, but some dexterity is often needed to use them properly. Many still operate taps with fingers, risking spread of pathogens. An innovative solution is the use of infrared operated taps. No physical contact with the tap is then needed and recontamination is avoided. Provided they are reliable, these should reduce cross-infection, particularly in the intensive care unit. The Health Building Note (Number 57) recommends that these taps be installed in all intensive care units." James Paget NHS Trust is one of the hospitals that has recognised the benefits, and has now installed over 600 no-touch taps. The initiative has been keenly supported by the infectioncontrol team, and Linda Hawtin, clinical specialist - infection control, stated: "From an infection-control perspective, the taps are excellent because they reduce the risk of hand contamination after washing, and they are quick to use because the water is warm immediately, therefore encouraging staff to wash their hands more often." Recent trials in large hospitals showed a 65% reduction in the water used per hand wash with no-touch taps compared to leveroperated taps. The number of hand washes doubled, confirming the views that no-touch taps are so much easier to use and encourage hand-washing. Even for retro-fit, where the original taps have to be removed, payback on water savings alone can be as short as three years. There are other products that improve hygiene and save water, such as no-touch, wave-on, electronic WC flushing systems. These are hygienic, easy to operate (which helps the disabled and young children) and use 30% less water. 4 PURGE CONTROLLER Clinical Automatic Taps The Aquarius range of taps are manufactured from stainless steel, and with a solid mm stainless steel ingot for the head assembly. Standard finish for these taps is brushed, but polished is available as an extra. Copper tails for taps can be provided if preferred to flexible connections. There may be certain high risk areas where point of use filters may be considered. DVS have specially designed this automatic tap to accept point-of-use filters. Disposable water filters for taps protect against Legionella and other bacteria in the water supply. The validated 0.2 µm filter membrane protects against Legionella spp, which are known contaminants of hospital water supplies and can lead to nosocomial infections. 5 Maintaining Good Water Quality Automatic Tap Purge Controller The automatic tap purge controller reduces maintenance and the risk of Legionella by purging the taps if they have not been operated for a predefined time. Purge run times are adjustable and delay can be set to detect non-use between 6 hours and 15 days. Purge can be set between 5 seconds and 5 minutes. A periodic purge can be switched on or off. Hot Tap Surgical Purge (Thermal Shock) It is important to avoid touching taps during handwashing to avoid cross-contamination. It is also important to ensure the risk of bacteria in the water is controlled, particularly in areas with high risk patients. There are a number of ways to decontaminate water supplies, however the most common is to rely on having the hot water temperature at 60 c+ up to the thermostatic mixing valve. Some hospitals D615/THERMAL Hot tap surgical purge system have maintenance regimes where they raise this temperature periodically to a much higher temperature, such as 80 c once per month which reduces further the risk of bacteria build up in the hot water systems. Cold water systems require alternative measures to ensure limitation of bacterial growth. Wherever the thermostatic mixing valve is fitted the tap spout and outlet is an area of risk and requires consideration. There is significant awareness of legionella in hospitals but other waterborne bacteria such as pseudomonas auroginosa also need to be given serious consideration. There are common factors such as materials, water temperature and stagnation that affect the growth of many waterborne bacteria. Certain bacteria will proliferate in less favourable conditions, and some form of water treatment and maintenance should always be considered. Cold water supplies and tap outlets are sometimes neglected. DVS have devised a system that ensures all the components of the automatic tap system can easily be decontaminated using thermal shock treatment. The treated areas include the thermostatic mixing valve, solenoid valve, tap spout and tap outlet. This is a maintenance responsibility and a keyswitch is provided to ensure safe operation by a responsible person. The engineer (or an appointed person) should be present during the purging operation, however there is a red warning light that also confirms that hot water is being purged through the tap. 10 Patient Ensuites Low Risk Patient Ensuites Some Trusts may consider that low risk areas do not justify the substantial extra cost for the specialist unbreakable sanitary ware supplied by DVS. Twyford ceramic Sola Spectrum basins and Galerie WC Pans are therefore proposed for these areas. An alternative high security backplate specially designed for the Twyford Sola Spectrum basin is also part of the Low Risk ensuite. Otherwise the proposals are identical for the High Risk and Low Risk ensuites. In every other respect it is proposed that the High Risk and Low Risk safe ensuites are to the same specification. This means that the operation, technology, products and maintenance are standard throughout. Twyford Spectrum Backplate The Spectrum backplate has been designed specifically to match the Twyford Sola Spectrum clinical basin. One area of use is low risk MHU where the porcelain basin is considered acceptable but anti-ligature taps are required. The extra reach on this backplate has also been found to suit other applications where taps have to be removed and replaced with an anti-ligature solution. The design is also quite clinical and may be considered suitable for other areas where a no-touch wave-on solution for warm and cold water is required, such as patients ensuites. Patient Ensuites Sensor Options There are two options for operating the shower and WC Flushvalve. Both are vandal proof and virtually ligature free. D102/AV/B Anti-vandal wave-on sensor with 50mmø chrome bezel for WC Packages 5 and 6 show solutions for patient ensuites which are very similar to the safe ensuites but have an adjustable rail in the shower, shower handset and option of conventional water spout. The spectrum backplate however offers a neat solution. DVS have cooperated with a pod manufacturer to mould this into the pod wall. Most installations use the wave-on sensor but some prefer the push button tactile switch to reduce the risk of accidentally turning on the shower.
