3 1. Gas Cylinder Fire Prevention General Considerations 1. Either the work-piece shall be moved to a safe location for carrying gas-cutting work or Combustible materials and flammable materials shall be removed from the work place 2. Spaces where gas could accumulate shall be ventilated 3. If combustible materials that cannot be moved shall be protected from close contact with flame, heat, sparks or hot slag. Suitable guards or covers such as metal sheeting, mineral fiber boards or fire retardant blankets shall be used; in such case hot work permit shall be obtained 4. Guards or covers shall be used to prevent cylinders from hot particles passing through openings in floors and walls 5. Fire extinguishers shall be kept nearby Prevention of Gas Leaks from Gas Cutting Sets 1. Hoses shall be kept clear of sharp edges and abrasive surfaces or where vehicles can run over them 2. Hot metal or spatter shall not be allowed to fall on hoses 3. Gas cylinders kept in upright position shall be chained to prevent them from falling down or being knocked over. For example, cylinders can be chained in a wheeled hand trolley or against a wall or a suitable support 4. Gas Supply shall be turned off at the cylinder when the job is finished 5. All the equipment shall be kept in good condition 6. All the connections and equipment shall be regularly checked for faults and leaks 7. Taking gas cylinders into a poorly ventilated rooms or confined spaces shall be avoided Special Precautions for hot works 1. Hot work permit shall be taken and measures to be implemented as detailed in hot work permit 2. After the combustible floors have been swept clean, they shall be protected by thoroughly wetting with water, covering with damp sand, sheet metal etc. or such other equivalent material 3. Suitable, incombustible tables, jigs or work places shall be provided for support of small or moderate size work during welding and cutting operations. Such operations should not be undertaken on work resting directly on concrete floors

4 4. Guards or covers shall be used to prevent cylinders from hot particles passing through openings in floors and walls 5. Suitable fire-extinguishing equipment, such as pails of water, buckets of sand, hose lines or portable extinguishers shall be kept in readiness for instantaneous use 6. Firewatchers shall be stationed at the required locations to extinguish the same if any fire breaks out Welding or Cutting of empty diesel or petrol Drums/Tanks 1. Welding / Cutting of oil drums, motor car petrol tanks or any other containers, which might at any time have contained a liquid giving off and inflammable gas shall not normally be permitted. However, in case welding of such container s is absolutely necessary, the following precautions shall be taken before welding them: 2. They shall be thoroughly degreased using an alkaline or any other type of degreasing solution; 3. They shall then be thoroughly rinsed with hot water and allowed to dry for a few days; 4. After drying and before welding, they shall be purged thoroughly for a period of at least five minutes with an inert gas; 5. The inert gas shall be allowed to flow through the container under a positive pressure while welding is taking place. Sprinkler Protection Where sprinkler protection exists, it shall be maintained without interruption while welding or cutting work is being performed. If welding or cutting is to be done quite close to automatic sprinkler heads, sheet asbestos or damp cloth guards shall be used to shield the individual leads temporarily. When Acetylene Cylinder gets heated If an acetylene cylinder is heated or becomes hot accidentally due to excessive or severe back fire from use of faulty equipment, it shall be dealt with promptly as follows: 1. The valve shall be shut. 2. Regulator or other fittings shall be detached. 3. The cylinder shall be taken outdoors. 4. It shall be immersed in water or water shall be applied copiously. 5. The valve shall be opened and the cylinder, kept cool in water until it becomes empty.

Good Practices Power House #6 To increase the night time conspicuity of peoples driving inside the company premises, retro reflective sheet are provided in accident prone areas and also on bicycle of contract workmen who travel long distances. Corporate Safety Highlights Oct 13 Safety audit was completed at Noida Office Building Project (NOBP) site and audit report was released for initiating CAPA. Participated in International Fire India 2013 exhibition at Goregaon, to get updated on best fire & safety practices followed worldwide. Awareness session on Disaster Management Plan & Business Continuity Plan was conducted for operation employees of Trombay. Conducted Training session on CFSA for Wind Operations team. Conducted & witnessed Emergency drill / exercise at CTTL Trombay as Observers and appropriate observations were shared in the post briefing meeting.

Safety training, drills & awards Oct 13 Training on Rigging & Sling handling techniques, Electrical Safety, Emergency Preparedness and Behaviour Based Safety (BBS) were conducted at wind sites Mock drill for Snake Bite and Wild Animal Attack were conducted in Nivade and Sadawagpur wind farms respectively SPANDAN training was conducted at Trombay through Aakar Empowerment Trust Trombay safety received Memento in Unit#7 Outage function

Trombay Liaison: Sanction Proposed Agreement to Lease for Coastal Police Outpost at Mahul, in Tata Power owned plot was handed over to Mr. Khalid Quaiser Addl Commissioner of Police, Eastern Region, Chembur, Mumbai, by Col. P B Dalvi Head - Security, Trombay on 02 October,13 Security. Training: Imparted to Security personnel of Coal Berth Jetty & MGB guards by Security Officers Kenjale/ Bani/ Desai. Meetings: Security EPM Process Review Meet was held at Trombay and was duly attended by Mr. S N Rudrappa Chief Corp Sec & Vigi., Col. P Dalvi, Lt Col. N S Sawant, Mr. G Sridharan and security officers from MO Area.

11 VIP Visit: On eve of Dussera Pooja, Mr. Anil Sardana MD, Mr. S Padmanabhan ED (O) & Mr. Ashok Sethi Chief Com/ED graced the occasion by visiting the Plant on 12 Oct, Reward & Recognition: certificates were awarded to Security Officer P S Bani by Head - & permanent security staff -Trombay for successfully handling Security system for the contractor staff during Unit # 7 Outage and successful handling of MMMS/VMS. II. Maithon MPL Security & Fire team participated in a local cricket tournament at Marurdih village. 32 teams participated and Team MPL WARRIORS played well but lost in semi final. However team has qualified for the third rounds and won the third Prize. A mock drill on checking of vehicle against bomb threat was exercised at 1300 hrs on 22 October A bomb was planted under the bottom of vehicle JH 10 AJ IT was planted in such a way that it was difficult to be detected through passing eyes. The driver of the vehicle was also not aware of the development. While the vehicle was entering through the material Gate a thorough check as usual was carried out using trolley mirror. While checking the SG noticed some- thing new at the bottom of the Car. He immediately informed the SS present at Gate. He verified and announced that it was a bomb. The vehicle was parked at a lonely place & the driver was taken in custody. The SS also informed Control room, Head Security.

12 III. Jojobera Training activities: Security personnel were trained on HOW TO REACT DURING BOMB THREAT IN THE BUILDING". For the purpose a mock drill was conducted on 30th OCT. Awareness classes were for 415 contract workmen during the safety training by the security officers of Tata power to make the contractors workmen aware of the Do's and Don ts in respect to security, while working inside the plant area. Social activities - The whole team of security actively attended Homeopathic camp at Govindpur on 25/10/2013. Reward-Recognition- 1) Mr Kaushik Dutta was rewarded/ recognized by Group Head Security for his effective role during IMS audit 2) Mr A K Srivastava was rewarded / recognized for playing an effective role during mock drill. 3) Five Supervisors/ Guards of out sourced agency SIS were rewarded/ recognized for their outstanding performance. Security Meeting -Head Security attended meeting with ADM, City SP and other Police and IB Officers on 12/10/2013 in connection with emergency arrangement for the threat arising out of PHAILIN.
